Infants born before 32 weeks of gestation are highly susceptible to late-onset sepsis because of immune immaturity, impaired intestinal barrier function, and frequent broad-spectrum antibiotic exposure. Whether probiotics should be continued or initiated during antibiotic therapy remains unresolved and is often conflated with routine prophylaxis in stable infants. This SANRA-guided structured narrative and translational review synthesized clinical, microbiome, resistome, mechanistic, and safety evidence identified in PubMed/MEDLINE, Scopus, and Web of Science through July 1, 2026, across three contexts: stable antibiotic overlap, suspected sepsis, and confirmed or unstable sepsis. Although brucellosis is one of the most common zoonotic infections worldwide, the development of a paradoxical reaction during treatment hasrarely been reported in the literature. We present a 17-year-old patient who developed acute sacroiliitis in the second week of treatment for olecranon bursitis brucellosis, despite her initial clinical findings showing signs of improvement. After excluding other potential etiologies for sacroiliitis, clinical and radiological recovery was achieved by adding anti-inflammatory therapy without modifying the existing antimicrobial regimen. Objective: To investigate predictive factors for seizure recurrence in children with febrile seizures (FS) within a single febrile illness, providing a basis for early clinical identification and preventive measures. Methods: We conducted a retrospective analysis of children with FS admitted to the Department of Pediatrics, Suzhou Hospital Affiliated to Anhui Medical University, from April 2023 to December 2025. A total of 337 children met the inclusion criteria. This study retrospectively analyzed the clinical data of two 19-month-old children diagnosed with Epstein-Barr virus-associated hemophagocytic lymphohistiocytosis (EBV-HLH) who received treatment with a dexamethasone combined with etoposide (VP-16) regimen. Both children met the early remission criteria for body temperature, IL-10, and IFN-γ within 48-72 hours after treatment initiation, but their clinical outcomes were markedly different. In Case 1, the EBV-DNA load decreased significantly during the initial treatment phase, and platelet counts steadily increased after treatment; the patient had a favorable prognosis at the 2-year follow-up. Age-Dependent Variations In Clinical Presentation, Laboratory Parameters, And Coronary Outcomes In Kawasaki Disease: A 10-Year Retrospective Cohort Study Of 480 Pediatric Patients In Eastern China, Zhuying Zhou, Chen Wang, Aole Li, Saeed Saboor, Xiaoli Li, Xiaowei Li, Xianmei Huang, Chunming Jiang

Objective: This study investigated the age-dependent clinical characteristics of Kawasaki disease (KD) in pediatric patients and assessed the influence of age on disease presentation and outcomes. Methods: A retrospective analysis was conducted of 480 children diagnosed with KD who were admitted to Hangzhou First People's Hospital between 2013 and 2022. Subjects were stratified into two cohorts based on age: <2 years and 2-14 years). Demographic data, clinical manifestations, laboratory parameters, comorbidities, and coronary artery abnormalities were comprehensively evaluated across the age groups. Results: Time from symptom onset to diagnosis was significantly prolonged in the older cohort (z=-2.70, P=0.007). The incidence of cervical lymphadenopathy was markedly higher in older children (χ²=15.12, P<0.001), while peripheral extremity changes were significantly less frequent (χ²=5.79, P=0.016). The younger cohort exhibited significantly elevated platelet counts, alanine aminotransferase, and NT-proBNP levels compared to older patients (z=-3.57, P<0.001; z=-2.39, P=0.02; z=-3.59, P<0.001, respectively). Coronary artery dilatation and aneurysm formation occurred more frequently in younger patients (z=7.70, P=0.006; z=14.63, P<0.001). However, coronary artery recovery rates following intravenous immunoglobulin therapy showed no significant inter-group differences (P>0.05). Conclusions: The clinical presentation of KD demonstrates significant age-dependent variation. Clinicians should maintain a high index of suspicion for KD in older children presenting with fever and cervical lymphadenopathy. Online pharmacies improve access to medicines by offering convenient ordering and home delivery, but their growth also creates important risks for medication safety and public health. This review examines the benefits and hazards associated with online medicine supply, including access without a valid prescription, falsified and substandard products, inappropriate medicine use, privacy breaches, and weak cross-border regulatory oversight. The available evidence indicates that illicit online sellers can expose consumers to products of uncertain identity, quality, and dose, while vulnerable users may have limited ability to distinguish legitimate services from unlawful vendors. Background: Chronic kidney disease (CKD) is a progressive disorder frequently complicated by anemia and a persistent inflammatory state, both of which significantly contribute to morbidity and mortality. Reduced renal production of erythropoietin (EPO) is the primary cause of anemia in CKD. Emerging evidence suggests that EPO may exert anti-inflammatory and metabolic effects beyond its hematopoietic role. Cancer immunotherapy has emerged as a transformative therapeutic strategy that harnesses the host immune system to recognize and eliminate malignant cells. This review provides a comprehensive overview of current immunotherapeutic approaches, including immune checkpoint inhibitors (ICIs), monoclonal antibodies (mAbs), cancer vaccines, dendritic cell (DC)-based vaccines, chimeric antigen receptor (CAR)-T cell therapy, and oncolytic virus therapy. The mechanisms of action, therapeutic applications, and clinical benefits of each modality are discussed. Monoclonal antibodies selectively target tumor-associated antigens, whereas ICIs restore antitumor immunity by blocking inhibitory immune checkpoints. Rheumatoid arthritis is an inflammatory autoimmune disease affecting the joints which eventually leads to systemic comorbidities. Its pathogenesis was attributed to disturbances in the Janus Kinase/Signal Transducer and Activator of Transcription signaling pathway. In this research, an innovative lead compound C6 was created and docked toward Janus Kinase1 (JAK1). The docking process was confirmed by re-docking of the co-crystallized ligand MI1 (Tofacitinib). C6 showed a predicted docking score of -9.17 kcal/mol, with enhancement of 1.26 kcal/mol over Tofacitinib. Autonomous artificial intelligence (AI) agents increasingly perform critical tasks, yet their lack of transparency makes it difficult to verify that a model was authentic and uncompromised when a decision was made and to maintain a trustworthy audit trail. The techniques presented herein implement a Cryptographic Agent Provenance Ledger (CAPL) security architecture that uses hardware-backed enclaves, privacy-preserving zero-knowledge proofs, hardware-rooted state continuity, and in-enclave cryptographic batching to create tamper-resistant, forensic-grade provenance for AI decisions. Sex-specific penetrance in autosomal-dominant Mendelian conditions is largely understudied. The neurodevelopmental disorder Pilarowski-Bjornsson syndrome (PILBOS) was initially described in females. Here, we describe the clinical and genetic characteristics of the largest PILBOS cohort to date, showing that both sexes can exhibit PILBOS features, although males are overrepresented. A mouse model carrying a human-derived Chd1 missense variant (Chd1R616Q/+) displays female-restricted phenotypes, including growth deficiency, anxiety, and hypotonia. Orchiectomy unmasks a growth-deficiency phenotype in male Chd1R616Q/+ mice, while testosterone rescues the phenotype in females, implicating androgens in phenotype modulation. Access to health care is a fundamental right of every refugee, yet gaps in implementing Kenya's Refugees Acts of 2006 and 2021 continue to limit access for Somali refugees in Dadaab camp. Existing research has documented resource constraints, movement restrictions, and other barriers to refugee health care, but how policy implementation gaps shape access in Dadaab has not been well explored. The purpose of this qualitative descriptive study was to understand barriers to health care access for Somali refugees resulting from gaps in implementing Kenya's refugee health policies. Effective communication between general practitioners (GPs) and medical specialists is essential for reducing healthcare costs and improving continuity of care. Healthcare leaders are concerned that ineffective communication between GPs and specialists may contribute to delays in care, fragmented services, and inefficient use of healthcare resources. Natural water bodies become eutrophic due to phosphate pollution from industrial and agricultural sectors. This research addresses waste disposal and water pollution at the same time by recycling spent low-temperature shift catalyst (LTSSC) from the fertilizer industry as a sustainable phosphate adsorbent. High surface area (247±5 m2/g), mesoporous structure, and many metal oxide sites (CuO 49%, ZnO 32.2%, and Al2O₃ 13.9%) were shown by characterization using XRF, XRD, BET, SEM–EDX, FTIR, particle size analysis, and zeta potential. Under optimum conditions (contact time 1.64 h, dose 5 g/L, initial concentration 9.58 mg/L, stirring 278 rpm), Response Surface Methodology optimization achieved 92.95±2.8% …

Somatic-type malignant transformation (STM) is a rare but aggressive complication of nonseminomatous germ cell tumors (NSGCTs), most commonly arising from the teratomatous component and frequently occurring in metastatic sites. We report the case of a man in his 20s with a history of mixed NSGCT (50% yolk sac tumor, 40% teratoma, and 10% embryonal carcinoma) treated with left orchiectomy and chemotherapy in 2023, who initially responded to treatment but was subsequently lost to follow-up. He presented with one week of progressive dyspnea and pleuritic left-sided chest pain. Serum alpha-fetoprotein was markedly elevated at 2,966.7 ng/mL. Computed tomography demonstrated a progressive …

The Africa Working Group, under the auspices of the Global Laboratory Quality Initiative of the Association for Diagnostics & Laboratory Medicine (ADLM), conducted several educational workshops in Africa to promote laboratory quality in the region. The challenges faced in maintaining quality in clinical laboratories in low-resource countries may differ from region to region and country to country. To better understand current gaps and inform our engagement with partners in Africa, a needs assessment survey was distributed to 1427 ADLM members in Africa and further disseminated through local partners, resulting in 121 responses from participants across 8 countries in the region. …

Nearly half of Massachusetts residents feel worse off financially than a year ago and expect to be doing about the same a year from now. Those with lower incomes are twice as likely as those with the highest incomes to feel worse off financially than a year ago. Most Bay Staters say their cost of living has increased in the past year, especially groceries, gasoline, and energy.
