Marriage readiness is a critical factor in ensuring long-term relationship satisfaction and resilience. In Indonesia, challenges such as rising cases of domestic violence and marital instability highlight the urgent need for more effective premarital counseling programs. However, existing interventions often lack cultural sensitivity, particularly in multicultural societies like Indonesia, which encompasses over 300 ethnic groups. This study examines the effectiveness of a culturally-informed premarital counseling program based on Banjar marital customs in enhancing marital readiness. A randomized controlled trial (RCT) was conducted with 32 engaged individuals divided into an experimental group receiving Banjar-based counseling and a control group participating in …

Methicillin-resistant Staphylococcus aureus (MRSA) remains a significant cause of healthcare-associated infections. Early identification of colonized individuals upon hospital admission is essential for effective infection control. This study aimed to evaluate the performance of pooled MRSA screening using the GeneXpert®system (Cepheid) under two conditions: with and without viral transport media (VTM). The goal was to assess the sensitivity of pooling strategies while minimizing reagent use, processing time, and laboratory workload. Swabs were collected from three anatomical sites, the axilla, groin, and nares, from newly admitted patients. Samples were pooled either with VTM or without VTM and then tested using the Xpert®MRSA …
Biofilm-Associated Infections: Mechanisms Of Formation, Resistance, And Emerging Therapeutic Strategies With Special Focus On Biofilm Inhibitors, Karem Ibrahem
Journal of King Abdulaziz University: Medical Sciences
Biofilms are complex, structured microbial communities enclosed within a self-produced extracellular matrix. This matrix allows them to adhere to a variety of surfaces, including medical devices and human tissues. The formation of biofilms poses a significant challenge in clinical settings due to their inherent resistance to antibiotics and to host immune defenses, thereby contributing to the persistence and chronicity of infections. In recent years, considerable attention has been directed towards the development of biofilm inhibitors as a promising therapeutic strategy to prevent biofilm formation, disrupt established biofilms, and enhance the susceptibility of biofilm-embedded bacteria to conventional antibiotics. The aim of …

Introduction. Diabetic kidney disease (DKD) is a major microvascular complication of diabetes mellitus, characterized by persistent albuminuria and a progressive decline in glomerular filtration rate (GFR), making it a leading cause of chronic kidney disease (CKD) and end-stage renal disease. Vascular endothelial growth factor-A (VEGF-A) is implicated in the pathogenesis of DKD, being overexpressed in the kidneys and contributing to increased vascular permeability, inflammation, and fibrosis. Albuminuria, an early indicator of renal damage in DKD, is a strong predictor of CKD progression and cardiovascular events. The urine albumin-to-creatinine ratio (UACR) is the recommended test for detecting and monitoring albuminuria. …

Electronic medical records (EMRs) are essential to modern healthcare systems, yet their implementation continues to face challenges such as disruption of doctor–patient communication, administrative burden, and documentation structures misaligned with clinicians’ clinical reasoning processes. This narrative review aims to assess the limitations of conventional EMRs and examine the potential of artificial intelligence (AI) to improve documentation efficiency, accuracy, and overall clinical performance. The literature indicates that AI technologies, including machine learning, deep learning, and natural language processing, can automate documentation, streamline workflows, enhance decision support, and reduce physician burnout, thereby allowing clinicians to devote more attention to patient care. Nevertheless, …

Introduction. Heart failure is one of the major challenges in global health due to its significant contribution to morbidity and mortality. One of the pathophysiological aspects is autonomic nervous system dysfunction which can be monitored through heart rate variability (HRV) parameters. Ivabradine is a pharmacological agent that potentially improves the balance of sympathetic and parasympathetic tone. Polycythemia is a condition characterized by an abnormal increase in the total red blood cell mass and is classified into primary and secondary polycythemia. Secondary polycythemia occurs as a physiological response to tissue hypoxia or increased erythropoietin production, without intrinsic abnormalities in erythroid progenitor cells. This condition is rare, particularly when associated with nephrotic syndrome. This report aims to describe a case of secondary polycythemia associated with focal segmental glomerulosclerosis (FSGS). A 20-year-old man presented with generalized edema for five months prior to admission, accompanied by foamy urine. Physical examination revealed peripheral edema and minimal ascites. Laboratory investigations demonstrated massive …

Global changes in gene expression are orchestrated by an array of transcription factors regulated by both extracellular and intracellular signals. The mechanistic target of rapamycin (mTOR) is a protein kinase that ubiquitously mediates signaling networks across every cell type, in two active complexes, mTORC1 and mTORC2. By phosphorylating its downstream targets, these classically cytosolic complexes regulate protein homeostasis, metabolism, cell growth, proliferation and survival. However, evidence from prostate cancer studies reveals mTOR and its partners can occupy the nucleus, where its interaction with chromatin (in complex with transcription factors) has been shown to regulate gene expression at the level of …

MutLα, a heterodimer of MLH1 and PMS2, plays a key role in DNA mismatch repair (MMR), which maintains genomic stability by correcting replication errors. Loss of MLH1 function causes MMR deficiency (MMRd), leading to elevated mutation rates and increased cancer susceptibility. However, MMRd can offer a therapeutic advantage, as high tumour mutational burden enhances the efficacy of immune checkpoint inhibition. MMR also drives somatic expansion of CAG repeats linked to Huntington’s disease (HD) pathogenesis. The C-terminal domain (CTD) of MLH1 contains at least two distinct protein–protein interaction (PPI) sites. The S1 site supports heterodimerization with the PMS2 endonuclease, whereas the …

Occupational resilience (OR) is a novel concept emerging in occupational therapy and occupational science, with significant implications for the field. However, its definition is still evolving and needs clarity and consensus. In this article, we offer a definition inspired by the real-life experiences of refugee immigrants who have been resettled in the United States of America (USA). We define OR as the ability to persist in an occupation, where the duration and intensity of engagement are the essential contributors. Therefore, we propose that three elements, occupation, intensity, and duration, constitute the nature of OR and shape how occupation impacts health, …
