Chronic obstructive pulmonary disease (COPD) is a chronic respiratory disease characterized by various pathologicallesions and an imbalance in the microflora. The Yi-Qi-Xuan-Fei Formula (YQXF) is a clinically effective formula withpharmacological potential to delay the progression of COPD. This study aims to explore the relationship between thetherapeutic mechanism of YQXF and the microflora in COPD. Our study found that YQXF reduces inflammatory injuryand inflammatory cell infiltration in lung tissue, repairs the intestinal mucosal barrier, and enhances immune function.Additionally, YQXF regulates the pulmonary and intestinal flora by increasing the abundance of Alloprevotella, Rose-buria, Oscillibacter, and Lactobacillus, while reducing the abundance of …

Parkinson's disease (PD) is a progressive neurodegenerative disorder marked by dopaminergic neuronal loss,oxidative stress, neuroinflammation, and a-synuclein aggregation. Despite advances, current treatments only offersymptomatic relief without altering disease progression. This review aims to evaluate the therapeutic potential of plant-derived phytochemicals in PD through evidence from both in vivo and in vitro experimental models. Key findingsindicate that rodent models (e.g., MPTP, rotenone) and neuronal cell lines (e.g., SH-SY5Y) help replicate PD pathologyand mechanistic insights. Phytochemicals such as baicalein, morin, ferulic acid, and bacopa demonstrate neuroprotectiveeffects via antioxidant, anti-inflammatory, and mitochondrial-stabilizing pathways. The high content of unsaturated fatty acids in avocado oil (Persea americana) has prompted in-depth exploration across research subjects. This study uses bibliometric analysis to assess global research trends on avocado oil and its fatty acid composition from 2004 to 2024. The search string retrieved 255 articles from the Scopus database and then analyzed using R BibliometriX and VOSviewer software. These tools analyze descriptive structures and collaborative patterns and create network data maps. For comparison, we separated the data into two research periods, namely 2004-2013 and 2014-2024. Efficacy Of Pethidine And Tramadol In The Suppression Of Postoperative Shivering: A Comparative Study, Ahmed Jaber Ibrahim, Hussien Ibrahim Hayal, Ahmed R.Y. Al-Sawad, Shahab Abdulla, Ralela Makline

Postoperative shivering is a common complication that can affect patient comfort and stability. Pethidine and tramadol are widely used to relieve this symptom, but their relative effectiveness and safety remain under investigation. In this randomized trial, 50 patients (aged 25–35 years) with postoperative shivering were divided into two groups: one group receiving intravenous tramadol (n = 25 patients) and one group receiving pethidine (n = 25 patients). Onset of drug action, vital signs, and side effects were systematically assessed. Pethidine had a significantly faster onset of action than tramadol (12.20 ± 0.35 min vs 7.16 ± 0.28 min, p < 0.001). No significant differences were observed between the two groups regarding body temperature, heart rate, oxygen saturation, or blood pressure (P < 0.05). Side effects, such as dizziness (16% vs. 8%), nausea and vomiting (20% vs. 8%), and nausea alone (48% vs. 20%), were more common and showed a statistically significant increase in the tramadol group. Both drugs were effective in suppressing postoperative tremor, but pethidine demonstrated superior efficacy, a faster onset of action, and a better safety profile. These results confirm pethidine as the recommended choice at an intravenous dose of 0.5 mg/kg.

This Doctor of Nursing Practice (DNP) project was a staff education project that addressed the stigmatization of patients with mental illness (PMI) within psychiatric contexts. The project site healthcare organization’s mental health professionals had a high rate of stigmatization of PMI. Stigma is linked to poor medication adherence rates, delays in accessing healthcare, lower healthcare utilization among PMI, psychiatric morbidity and mortality, and poor quality of life. Objectives: The purpose of this study was to evaluate the effect of gingival pigmentations and anterior crown heights in conjunction with the maxillary gingival display on the perception of smile attractiveness assessed by orthodontists, dentists, and laypersons.

BACKGROUND: Sonic hedgehog (SHH) medulloblastoma is the most common molecular group of infant and early childhood medulloblastoma (iMB) and has no standard of care at relapse. This work aimed to evaluate the post-relapse survival (PRS) and explore prognostic factors of patients with nodular desmoplastic (ND) and/or SHH iMB.

METHODS: This international retrospective study included 147 subjects diagnosed with relapsed ND/SHH iMB between 1995 and 2017, < 6 years old at original diagnosis, and treated without initial craniospinal irradiation (CSI). Univariable and multivariable Cox models with propensity score analyses were used to assess PRS for those in the curative intent cohort.

RESULTS: The 3-year PRS was 61.6% (95% confidence interval [CI], 52.2-69.6). The median age at relapse was 3.4 years (interquartile range [IQR], 2.6-4.1). Those with local relapse (40.8%) more often received salvage treatment with surgery …

BACKGROUND: Factors influencing in-hospital mortality in patients with well-differentiated gastroenteropancreatic neuroendocrine tumors (GEP-NETs) remain understudied, highlighting gaps in optimizing acute clinical outcomes. This study aimed to identify sociodemographic and clinical predictors of all-cause in-hospital mortality in this population.

METHODS: Using 2016 - 2020 data from the National Inpatient Sample (NIS), patients with malignant well-differentiated GEP-NETs were identified via the International Classification of Diseases, 10th Revision (ICD-10) codes. The primary outcome was in-hospital mortality. Sociodemographic and clinical variables (heart failure (HF), malnutrition, Charlson Comorbidity Index (CCI), and tumor site) were analyzed using multivariable logistic regression.

We report direct measurements of changes in diving and movement behavior for 53 goose-beaked whales (Ziphius cavirostris) in relation to experimentally controlled mid-frequency (3–4 kHz) active sonar (MFAS) signals. These signals simulate powerful Navy sources that have been associated with multiple mortal stranding events for this species. We deployed a multi-scale combination of tags to monitor individual whales, including 50 long-duration (weeks), coarse-resolution satellite-transmitting tags and 3 short-duration (hours), high-resolution archival depth, orientation, and acoustic tags. Nanotechnology provides promising options through nanomaterials, including metal oxides, magnetic nanoparticles (MNPs), and carbon-based composites, which show high removal efficiencies of 50–98% for organic contaminants and 80–99% for inorganic pollutants. Many western scientific disciplines adopted Indigenous Knowledge and terminology without deference to or understanding of the original meanings and values attached to Indigenous terms and concepts. This form of scientific appropriation has become a serious barrier to efforts of decolonizing Arctic research. The notion of Alaas is an example of such appropriation by the western science-based system of Indigenous knowledge about human–nature relations. Background Infants born to pregnant women living with HIV (WLHIV) are at greater risk for morbidity and mortality and may also have poorer developmental outcomes as compared with infants who are not exposed to HIV. Nutrition interventions in pregnancy may affect developmental outcomes.

Objectives This study evaluated the effect of maternal vitamin D supplementation on infant development outcomes.

Design We conducted a secondary analysis of a randomised, triple-blind, placebo-controlled trial of maternal vitamin D supplementation from June 2015 to October 2019.

Setting Antenatal care clinics in Dar es Salaam, Tanzania.

Neuropteran insects display a phenomenon called distance segregation during male meiosis I, where the X and Y chromosomes are unconnected from prometaphase I through anaphase I. Here we investigate chromosome number and patterns of chromosome segregation of a previously unstudied Neuropteran: the green lacewing, Chrysoperla rufilabris (Burmeister, 1839). Testes of male C. rufilabris were isolated and prepared following a chromosome squash method. Living meiosis I spermatocytes were also imaged. The data revealed a chromosome number of 2n=12 in males (2n=10+XY) and evidence of distance segregation of X and Y chromosomes in meiosis I spermatocytes.

Pancreatic ductal adenocarcinoma (PDAC) is an aggressive solid malignancy with poor overall prognosis and limited response to standard treatments. Growing interest in the modulation of DNA repair mechanisms, including the homologous recombination (HR) repair pathway, has opened new avenues for therapeutic development. BARD1 (BRCA1-Associated RING Domain 1) plays a complex role in tumor biology, functioning either as a tumor suppressor or as an oncogenic driver, depending on isoform expression, cellular context, and regulatory environment. The mechanistic target of rapamycin complex 1 (mTORC1) integrates growth factor (GF) and nutrient signals to stimulate anabolic processes connected to cell growth and inhibit catabolic processes such as autophagy1,2. GF signalling through the tuberous sclerosis complex regulates the lysosomally localized small GTPase RAS homologue enriched in brain (RHEB)3. Direct binding of RHEB-GTP to the mTOR kinase subunit of mTORC1 allosterically activates the kinase by inducing a large-scale conformational change4. Here we reconstituted mTORC1 activation on membranes by RHEB, RAGs and Ragulator. Cryo-electron microscopy showed that RAPTOR and mTOR interact directly with the membrane. BACKGROUND: Concomitant anterior cruciate ligament (ACL) and medial collateral ligament (MCL) injuries are among the most commonly reported injury patterns in high-energy knee injuries. Concomitant ACL and MCL reconstruction (ACLR + MCLR) is indicated in patients with full-thickness tears and persistent valgus laxity.

PURPOSE: To systematically review the literature to identify studies reporting on patients undergoing concurrent primary ACLR + MCLR to better understand current indications, surgical techniques, complications, and outcomes.

Objective: To review the global impact of the COVID-19 pandemic on stroke care-metrics and report data from a health system in Houston.

Methods: We performed a meta-analysis of the published literature reporting stroke admissions, intracerebral hemorrhage (ICH) cases, number of thrombolysis (tPA) and thrombectomy (MT) cases, and time metrics (door to needle, DTN; and door to groin time, DTG) during the pandemic compared to prepandemic period. Within our hospital system, between January-June 2019 and January-June 2020, we compared the proportion of stroke admissions and door to tPA and MT times.

More than 20 genes expressed in the male reproductive tract have been identified as essential factors for sperm migration to and through the utero-tubal junction (UTJ), and they are divided into ADAM3-dependent and ADAM3-independent pathways. In parallel, sperm having UTJ migration defects also show impaired binding to the zona pellucida (ZP). Herein, we demonstrate that knockout of Galntl5, encoding a sperm surface protein, causes impaired sperm binding with the UTJ and ZP, and null males have severe infertility.
