Multi-Enclave Blockchain Architecture Integrating Useful Work For Member Selection In Committee-Based Consensus, Shawn C. Adams
All ETDs from UAB
Blockchains offer an alternative approach to centralized systems for data and record storage by using independently operated nodes in a distributed and decentralized network. Within a blockchain network, each node maintains an identical version of all data, without being directly controlled or managed by any centralized entity. Nodes maintain the synchronization of data through the network's consensus mechanism, a protocol built into the nodes to determine which node is given the right to append data to the network. Once chosen, the data added by the node is propagated to other nodes which update their local records to maintain synchronization. Common …

Synthetic cannabinoids are synthetic compounds created to mimic the effects of naturally derived cannabis. They are up to 100 times more potent than natural cannabis, and due to their cannabinoid-like activity, they are considered schedule one drugs in Alabama. They are also the most common new psychoactive substance in the state of Alabama. The possible threat to public health and their schedule one status makes it necessary to develop a method for the detection of these substances. BZO-POXIZID and MDA-19 are two synthetic cannabinoids of recent interest. This study was conducted to validate a method for the detection of these …

In recent years, nanobodies have become important theranostic molecules, and research has focused on improving their pharmacokinetics and binding for novel targets. Nanobodies are characterized by their high kidney uptake, leading to potentially unsafe exposure of the kidneys when delivering cytotoxic payloads. Researchers hypothesize that positively charged amino acids contribute to this kidney uptake. To address this, we assessed the impact of substituting lysine residues within the 2Rs15d HER2 nanobody scaffold to alanine on its in vivo pharmacokinetics using positron emission tomography (PET) imaging. We found that nanobodies bearing lysine-to-alanine substitutions maintained their affinity for HER2. Characterization of the K88A …

Evidence-Based Decision Making At The Bedside Process, Roy E. Brown
Evidence-Based Decision Making At The Bedside Process, Roy E. Brown
VCU Libraries Faculty and Staff Publications
Evidence-based practice (EBP) is crucial in healthcare for improving care and organizational outcomes. However, there is a lack of consensus among various health professions regarding the definition and implementation of evidence-based practice in the clinical environment. This lack of consensus centers on whether EBP is project-based or if it occurs primarily at the bedside. This diagram illustrates how evidence is utilized to inform bedside decision-making. It demonstrates that the process is iterative and the intervention or decision made is contingent upon the evidence, one’s clinical experience, and the patient/situation.
The purpose of the diagram is to help those involved in …

Automated Hiv Case Identification From The Mimic-Iv Database, Kai Jiang, Tru Cao
Faculty, Staff and Student Publications
Automatic HIV phenotyping is needed for HIV research based on electronic health records (EHRs). MIMIC-IV, an extension of MIMIC-III, contains more than 520,000 hospital admissions and has become a valuable EHR database for secondary medical research. However, there was no prior phenotyping algorithm to extract HIV cases from MIMIC-IV, which requires a comprehensive knowledge of the database. Moreover, previous HIV phenotyping algorithms did not consider the new HIV-1/HIV-2 antibody differentiation immunoassay tests that MIMIC-IV contains. Our work provided insight into the structure and data elements in MIMIC-IV and proposed a new HIV phenotyping algorithm to fill in these gaps. The …

INTRODUCTION: Autosomal dominant retinitis pigmentosa type 17 (adRP, type RP17) is caused by complex structural variants (SVs) affecting a locus on chromosome 17 (chr17q22). The SVs disrupt the 3D regulatory landscape by altering the topologically associating domain (TAD) structure of the locus, creating novel TAD structures (neo-TADs) and ectopic enhancer-gene contacts. Currently, screening for RP17-associated SVs is not included in routine diagnostics given the complexity of the variants and a lack of cost-effective detection methods. The aim of this study was to accurately detect novel RP17-SVs by establishing a systematic and efficient workflow.
METHODS: Genetically unexplained probands diagnosed with adRP …

Regulator of G-protein signaling (RGS) proteins are regulators of signal transduction mediated by G protein-coupled receptors (GPCRs). Current studies have shown that some molecules in the RGS gene family are related to the occurrence, development and poor prognosis of malignant tumors. However, the RGS gene family has been rarely studied in gastric cancer. In this study, we explored the mutation and expression profile of RGS gene family in gastric cancer, and evaluated the prognostic value of RGS expression. Then we established a prognostic model based on RGS gene family and performed functional analysis. Further studies showed that RGS4, as an …

OBJECTIVES: to investigate health-related quality of life in patients with idiopathic inflammatory myopathies (IIMs) compared with those with non-IIM autoimmune rheumatic diseases (AIRDs), non-rheumatic autoimmune diseases (nrAIDs) and without autoimmune diseases (controls) using Patient-Reported Outcome Measurement Information System (PROMIS) instrument data obtained from the second COVID-19 vaccination in autoimmune disease (COVAD-2) e-survey database.
METHODS: Demographics, diagnosis, comorbidities, disease activity, treatments and PROMIS instrument data were analysed. Primary outcomes were PROMIS Global Physical Health (GPH) and Global Mental Health (GMH) scores. Factors affecting GPH and GMH scores in IIMs were identified using multivariable regression analysis.
RESULTS: We analysed responses from 1582 …
