Journal of Community Hospital Internal Medicine Perspectives

Prosthetic valve endocarditis is a serious complication following valve replacement surgery, with the development of aortic root abscess being a catastrophic manifestation. We present a case of a 71-year-old female with a bioprosthetic aortic valve who presented with shortness of breath. Echocardiographic findings revealed vegetation on the prosthetic aortic valve with a periaortic, annular abscess. Given the patient’s comorbidities and high surgical risk, conservative management with antibiotics was chosen. The patient responded well to antibiotic therapy and was discharged with long-term antibiotic treatment. Prosthetic valve endocarditis, particularly with periaortic abscess formation, is associated with significant morbidity and mortality. Early diagnosis …

INTRODUCTION: According to the World Cancer Research Fund International (WCRFI), breast cancer (12.5%) has now topped lung cancer (12.2%) as the most prevalent form of cancer in the world (WCRFI, 2022). While therapeutic advances continue to be made, the current treatment of cancer is widely associated with severe side effects from classic therapies such as radiation and chemotherapy. Over the last two decades, studies have examined breast cancer vaccines in pre-clinical and clinical settings. Here, we present an in-depth review of current mRNA-based cancer vaccine literature to assess the prevalence of this technology in this field of medicine. This includes …

Diagnostic Ambiguity Of Distal Dvt (Ddvt) Misdiagnosed As Nonpurulent Lower Extremity Cellulitis (Nlec): A Case Study, Jessica M. Ruiz-Myara, Hunter Alberto Vásquez, Joy Zarandy May 2024

Diagnostic Ambiguity Of Distal Dvt (Ddvt) Misdiagnosed As Nonpurulent Lower Extremity Cellulitis (Nlec): A Case Study, Jessica M. Ruiz-Myara, Hunter Alberto Vásquez, Joy Zarandy

Research Day

Introduction

Given the lack of a definitive diagnostic test for cellulitis, about 39% of cases are misdiagnosed. The repercussions of misdiagnosing nonpurulent lower extremity cellulitis (NLEC) for deep vein thrombosis (DVT) can be grave, considering the potential risk of pulmonary embolism (PE). While distal DVT (dDVT) is associated with <5% of PE formation, about 25% of untreated dDVT can ascend to the proximal leg veins; whereas 50% of proximal DVTs (pDVT) progress to PE development within a 3-month timeframe.

Case Report

A 50-year-old female presents to her PCP with a right medial ankle rash for 2 days. She denies inciting events, fever, chest pain, dyspnea, weeping lesions, or swelling. Past medical history includes obesity and controlled diabetes mellitus. Vitals are normal. Examination reveals a 10 x 6 cm blanchable, erythematous …

Chronic Stress-Induced Neuroinflammation: Relevance Of Rodent Models To Human Disease., Abigail G White, Elias Elias, Andrea Orozco, Shivon A Robinson, Melissa T Manners

College of Science & Mathematics Departmental Research

The brain is the central organ of adaptation to stress because it perceives and determines threats that induce behavioral, physiological, and molecular responses. In humans, chronic stress manifests as an enduring consistent feeling of pressure and being overwhelmed for an extended duration. This can result in a persistent proinflammatory response in the peripheral and central nervous system (CNS), resulting in cellular, physiological, and behavioral effects. Compounding stressors may increase the risk of chronic-stress-induced inflammation, which can yield serious health consequences, including mental health disorders. This review summarizes the current knowledge surrounding the neuroinflammatory response in rodent models of chronic stress-a …

Rethinking Shelter: Designing A Home For Atlanta's Homeless Families And Youth, Maddi Dutton May 2024

Rethinking Shelter: Designing A Home For Atlanta's Homeless Families And Youth, Maddi Dutton

Bachelor of Architecture Theses - 5th Year

Homelessness is a devastating experience, and its impacts are suffered by Atlanta’s families and youth who find themselves without stable housing. Family homelessness often results in the separation of families, leading to unaccompanied youth living on the streets or in uncomfortable shelters. Existing shelters do not provide family accommodations. They offer basic services and mostly lack a sense of community and personalized private spaces that are essential for fostering a sense of dignity. As emphasized in House as a Mirror of Self by Clare Cooper Marcus, humans need a ‘home’ not just a dwelling. A physical space becomes a home …

The Relationship Between Ultrarunning Status, Subjective Well-Being, And Life Satisfaction, Raquel Ramirez Hernandez May 2024

The Relationship Between Ultrarunning Status, Subjective Well-Being, And Life Satisfaction, Raquel Ramirez Hernandez

Walden Dissertations and Doctoral Studies

The COVID-19 pandemic resulted in a rise in mental health challenges across the United States. Consequently, two important well-being measures for Healthy People 2030—subjective well-being and life satisfaction—were negatively impacted. A plethora of evidence exists about the benefits of running for physical and mental health. Yet, there is no available evidence that shows the benefits of ultrarunning, subjective well-being, and life satisfaction. This quantitative cross-sectional study examined the relationship between ultrarunning status, subjective well-being, and life satisfaction among adult runners in the United States. The advanced triangle of epidemiology served as the conceptual framework. A convenience sample included 369 adult …

The Trnaval Half: A Strong Endogenous Toll-Like Receptor 7 Ligand With A 5′-Terminal Universal Sequence Signature, Kamlesh Ganesh Pawar, Takuya Kawamura, Yohei Kirino

Computational Medicine Center Faculty Papers

Toll-like receptors (TLRs) are crucial components of the innate immune system. Endosomal TLR7 recognizes single-stranded RNAs, yet its endogenous ssRNA ligands are not fully understood. We previously showed that extracellular (ex-) 5'-half molecules of tRNAHisGUG (the 5'-tRNAHisGUG half) in extracellular vesicles (EVs) of human macrophages activate TLR7 when delivered into endosomes of recipient macrophages. Here, we fully explored immunostimulatory ex-5'-tRNA half molecules and identified the 5'-tRNAValCAC/AAC half, the most abundant tRNA-derived RNA in macrophage EVs, as another 5'-tRNA half molecule with strong TLR7 activation capacity. Levels of the ex-5'-tRNAValCAC/AAC half were highly up-regulated in macrophage EVs …

Improving Design And Physical Boundary In Temporary Post-Disaster Relief Shelters: Analysis Of Existing Literature, Min-Hye Jang May 2024

Improving Design And Physical Boundary In Temporary Post-Disaster Relief Shelters: Analysis Of Existing Literature, Min-Hye Jang

Theses

Natural disasters such as floods, hurricanes, cyclones, storms, earthquakes, wildfires, and landslides displace people. As responses at the community resilience level, the temporary post-relief shelter is critical for the community's rapid recovery. Converting existing public facilities is the fastest way to respond to a crisis. However, the Hurricane Katrina Superdome Shelter alerted society to the prevalence of sexual assault in transitory catastrophe contexts. Not only has sexual abuse occurred, but prior shelters did not equip every user with physical barriers to suit their needs. This study examined academic journals, articles, and books to identify issues with natural disaster shelters created …

Purinergic Signaling Drives Multiple Aspects Of Rotavirus Pathophysiology, Kristen A Engevik, Francesca J Scribano, J Thomas Gebert, Joseph M Hyser

Faculty, Staff and Students Publications

Rotavirus causes life-threatening diarrhea in children, resulting in ∼200,000 deaths/year. The current treatment during infection is Oral Rehydration Solution which successfully replenishes fluids but does not alleviate diarrhea volume or severity. As a result, there is an urgent need to better understand rotavirus pathophysiology and develop more effective pediatric therapeutics. Rotavirus primarily infects the tips of small intestinal villi, yet has far-reaching effects on cell types distant from infected cells. We recently identified that rotavirus infected cells release the purinergic signaling molecule ADP, which activates P2Y1 receptors on nearby uninfected cells in vitro . To elucidate the role of purinergic …

Evaluating the heterogeneity of extracellular vesicles (EVs) is crucial for unraveling their complex actions and biodistribution. Here, we identify consistent architectural heterogeneity of EVs using cryogenic transmission electron microscopy (cryo-TEM), which has an inherent ability to image biological samples without harsh labeling methods while preserving their native conformation. Imaging EVs isolated using different methodologies from distinct sources, such as cancer cells, normal cells, immortalized cells, and body fluids, we identify a structural atlas of their dominantly consistent shapes. We identify EV architectural attributes by utilizing a segmentation neural network model. In total, 7,576 individual EVs were imaged and quantified by …

Using Digital Twins To Protect Biomanufacturing From Cyberattacks, Brenden Fraser-Hevlin, Alec W. Schuler, B. Arda Gozen, Bernard J. Van Wie

Military Cyber Affairs

Understanding of the intersection of cyber vulnerabilities and bioprocess regulation is critical with the rise of artificial intelligence and machine learning in manufacturing. We detail a case study in which we model cyberattacks on network-mediated signals from a novel bioreactor, where it is important to control medium feed rates to maintain cell proliferation. We use a digital twin counterpart reactor to compare glucose and oxygen sensor signals from the bioreactor to predictions from a kinetic growth model, allowing discernment of faulty sensors from hacked signals. Our results demonstrate a successful biomanufacturing cyberattack detection system based on fundamental process control principles.

CD4+ T cells are vital for host defense and immune regulation. However, the fundamental role of CD4 itself remains enigmatic. We report seven patients aged 5-61 years from five families of four ancestries with autosomal recessive CD4 deficiency and a range of infections, including recalcitrant warts and Whipple's disease. All patients are homozygous for rare deleterious CD4 variants impacting expression of the canonical CD4 isoform. A shorter expressed isoform that interacts with LCK, but not HLA class II, is affected by only one variant. All patients lack CD4+ T cells and have increased numbers of TCRαβ+CD4-CD8- T cells, which phenotypically …

Therapeutic Advances In The Management Of Type Ii Diabetes, Ashfod Mukula May 2024

Therapeutic Advances In The Management Of Type Ii Diabetes, Ashfod Mukula

Senior Honors Theses

Type 2 diabetes mellitus (T2DM) is a metabolic disorder characterized by persistent hyperglycemia. This condition occurs due to two main factors: β-cell dysfunction and reduced response to insulin by cells and tissues with insulin receptors (insulin resistance). Insulin resistance occurs when the body fails to respond to insulin; therefore, cells and tissues are impaired in transporting glucose, leading to abnormally high glucose levels in circulation. Insulin resistance is a multifactorial condition; thus, the eminent cause is not fully known. Individual health conditions and genetic predisposition have been postulated to cause it. Molecularly, insulin resistance has been associated with complications in …

Measuring The Effects Of Selenium Exposure On Batrachochytrium Dendrobatidis (Bd) Growth In Vivo In Larval American Bullfrogs (Rana Catesbeiana)., Taylor Morrison

Honors Thesis

Most amphibians in today’s world are exposed to a variety of environmental stressors. This project’s main objective was to determine any effects of selenium on Batrachochytrium dendrobatidis (Bd) and infection levels in South Dakota amphibians. I selected American bullfrogs (Rana catesbeiana) as they are susceptible to Bd and are found throughout eastern South Dakota. The secondary objective of this project was to measure any impact of Bd and selenium, combined, on growth measures of the frogs. The third objective was to see if survival rates were reduced when amphibians were exposed to Bd and selenium. Tadpoles were collected (n=225) from …

Serine Deamination By Human Serine Racemase Synergizes With Antibiotics To Curtail The Replication Of Chlamydia Trachomatis, Patricia D. Mott, Arnold H. Zea, Jamiya Lewis, Oygul Mirzalieva, Ashok A. Aiyar

School of Medicine Faculty Publications

The obligate intracellular bacterium, Chlamydia trachomatis, has evolved to depend on its human host for many metabolites, including most amino acids and three of the four nucleotides. Given this, it is not surprising that depletion of a single amino acid in the host cell growth medium blocks chlamydial replication. Paradoxically, supra-normal levels of some amino acids also block productive replication of Chlamydia. Here, we have determined how elevated serine levels, generated by exogenous supplementation, impede chlamydial inclusion development and reduce the generation of infectious progeny. Our findings reveal that human serine racemase, which is broadly expressed in multiple tissues, potentiates …

Implementation Of An Algorithm To Reduce Wait Times For College-Aged Women Presenting With Uti Symptoms, Mallory Beck, Miranda Neu

Doctor of Nursing Practice Projects: College of Nursing

Urinary tract infections, UTIs, are among the most common reasons college-aged women seek medical care. The selected University clinic has a difficult time triaging walk-in UTI patients promptly. Long patient wait times have a negative effect on patient and employee satisfaction scores. An algorithm was created to assist the University clinic in triaging walk-in UTI patients using guidelines from the American Academy of Family Physicians updated with the most current clinical practice recommendations. The algorithm includes the utilization of a point-of-care urine analyzer which provides urinalysis results in 120 seconds. With the algorithm and point-of-care urine analyzer, the student health …

Chronic inflammation, when left untreated, may result in significant morbidity and in severe cases, mortality. While glucocorticoids (GCs) are still being extensively used clinically as a class of very potent anti-inflammatory agents, they are also notorious for diverse adverse side effects. To mitigate these side effects, we have developed a polyethylene glycol (PEG)-based prodrug (ZSJ-0228) of dexamethasone (Dex). When tested on rodent models of Lupus Nephritis and Focal Segmental Glomerulosclerosis (FSGS), ZSJ-0228 was found to provide potent and sustained therapeutic efficacy without any detectable typical GC-associated side effects. Given ZSJ-0228’s transient renal exposure after systemic administration, it is yet to …

The global outbreak of COVID-19, triggered by the novel coronavirus, Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2), has spurred an urgent need for a deeper comprehension of the molecular mechanisms involved in the host-virus interactions. Despite advancements in transcriptomic technology and computational resources, limited attention has been given to the holistic integration of molecular and clinical data to characterize the genotype/phenotype aspects of the disease.

This study analyzes gene expression patterns in various tissues, including the lung, nasal, blood, and placenta, in patients with COVID-19 to identify differentially regulated genes and pathways. We also evaluated organ-specific gene co-expression patterns that …

Development Of Long-Acting Antiviral And Antimicrobial Prodrug Nanoformulations, Srijanee Das

Theses & Dissertations

Over the past decade, the global burden of infectious diseases has increased substantively, underscoring the need for more effective treatment strategies. However, the discovery and development of new drugs with desirable safety, pharmacokinetics, and efficacy is a time consuming and costly process with great uncertainty around success rate. As an alternative to new drug development, prodrugs and novel drug delivery systems have increasingly been explored. Both serve to overcome the drug development challenges. Such drug formulations that permit extended dosing intervals of potent native drugs serve multiple purposes by promoting patient adherence to in improving treatment outcomes. This is especially …

A Tale Of Two Proteins: An Investigation Of Recruitment Of Eukaryotic Proteins Fli1 And Lrrf1 To The Chlamydial Inclusion Membrane, Natalie Sturd May 2024

A Tale Of Two Proteins: An Investigation Of Recruitment Of Eukaryotic Proteins Fli1 And Lrrf1 To The Chlamydial Inclusion Membrane, Natalie Sturd

Theses & Dissertations

Chlamydia trachomatis is the pathogen responsible for the majority of infectious blindness and sexually transmitted bacterial infections worldwide. As an obligate intracellular pathogen, it establishes a niche within a pathogen-specific vacuole, termed the chlamydial inclusion. C. trachomatis modifies the inclusion membrane by type III secretion and insertion of effectors known as inclusion membrane proteins, or Incs. These Inc proteins are critical for chlamydial pathogenesis via their interactions with host proteins and their capacity to stabilize the inclusion membrane. Previous studies from our lab demonstrated the localization of two eukaryotic proteins, Flightless 1 (FLI1) and its binding partner leucine-rich repeat interacting …

Mucins: Drivers Of Cancer Cell And Microenvironment Crosstalk In Pancreatic Cancer, Xiaoqi Li May 2024

Mucins: Drivers Of Cancer Cell And Microenvironment Crosstalk In Pancreatic Cancer, Xiaoqi Li

Theses & Dissertations

Mucins facilitate the pancreatic cancer (PC) initiation, progression, and metastasis. Among mucins, MUC4 has been reported to inhibit lymphokine-activated cell killing and induce the apoptosis of cytotoxic T-cells. Counterintuitively, MUC4 expression is upregulated by multiple T-cell-secreted cytokines, such as IFN-γ, IL-17, and stroma-secreted factors like retinoic acid. Previously, we have identified that nuclear receptor coactivator 3 (NCOA3) regulates the MUC1 and MUC4 expression by increasing chromatin accessibility and maintaining protein stability. However, the comprehensive crosstalk mediated by MUC4 in cancer cells and T-cells and how its upstream regulator, NCOA3, modifies the cancer cell-intrinsic behavior is still elusive. Here, we show …

The Physiological Significance Of Nitric Oxide Synthase In Staphylococci, Ryan M. Singh May 2024

The Physiological Significance Of Nitric Oxide Synthase In Staphylococci, Ryan M. Singh

Theses & Dissertations

Staphylococci, particularly Staphylococcus epidermidis, are common causes of implant-associated infections that may result in notable morbidity and mortality. One of the ways the innate immune system attempts to combat these invading pathogens is via the respiratory burst, in which free radical species such as nitric oxide (NO•), toxic to bacterial respiration, are released in large quantities. Despite the toxicity of NO•, some bacteria, including staphylococci, encode an endogenous bacterial nitric oxide synthase (bNOS), whose physiological role has been unclear until now. In this dissertation, we demonstrate that S. epidermidis is able to overcome the toxicity …
