BACKGROUND: Pain is prevalent and poorly managed in older adults. Although pain self-management strategies are helpful, adoption and access are limited; thus, technology provides an opportunity for intervention delivery. Mobile health (mHealth) is feasible to use in older adults; however, we have yet to understand the effect of mHealth pain self-management interventions on pain outcomes in older adults.

A female, age 78, with generalized anxiety disorder (GAD), history of major depression, and hypothyroidism presents for mood and anxiety concerns. Her mood is depressed, and sleep, appetite, energy, and concentration are poor. She recently saw a geriatric psychiatrist and described no past manic episodes, psychotic symptoms, or family history for bipolar disorder. No one has concerns about her cognition. She began levothyroxine 150 mcg 3 weeks ago for newly diagnosed hypothyroidism. On exam, she is talkative, overinclusive, has almost pressured speech, labile mood, and lid lag. Background: Passive remote monitoring technologies (RMT) are an option that could keep frail older adults home longer while reducing care burdens on family/friend caregivers. In contrast to active RMT which requires an individual to engage with the technology (i.e., push a button), passive RMT does not require any action to function (i.e., sensors or cameras). Objective: This qualitative study explored the challenges and facilitators of implementing passive RMT in home care settings by applying an implementation science lens. Family law is for young people. To facilitate child rearing and help spouses pool resources over a lifetime, the law obligates parents to minor children and spouses to each other. Family law’s presumption of young, financially interdependent, conjugal couples raising children privileges one family form—marriage—and centers the dependency needs of children.

This age myopia fundamentally fails older adults. Families are essential to flourishing in the last third of life, but the legal system offers neither the family forms many older adults want nor the support of family care older adults need. There is an increasing demand for speech language pathologists (SLPs) willing to work with older adults. Demographic trends warn that the number of people over 65 will double by 2050 (Vincent, 2010). Studies reveal that only 37.3% of SLP students prefer to work with older adults after graduation (Leonard et al., 2016). Ageism is affecting speech-language pathology and related health care professions. Posttraumatic stress disorder (PTSD) is a mental health diagnosis that is a reaction to very stressful experiences. PTSD may hamper the quality of life for individuals suffering from it. The nursing home industry's lack of emotional intelligence (EI) can negatively impact patient satisfaction and performance. Nursing home leaders who lack strategies to apply EI training to increase job satisfaction, reduce employee turnover, and improve interpersonal skills risk financial loss and business failure. Limited health literacy (LHL) has profound implications on measured transplant outcomes as it is associated with higher incidence of graft loss, organ rejection, decreased immunosuppression medication adherence, and increased hospital re-admissions. วัตถุประสงค์: เพื่อศึกษาประสบการณ์การเจ็บป่วยของผู้ป่วยสูงอายุที่ติดเชื้อไวรัสโคโรนา 2019

รูปแบบการวิจัย: เป็นการวิจัยเชิงคุณภาพแบบปรากฏการณ์วิทยา

วิธีการดำเนินการวิจัย: ผู้ให้ข้อมูลเป็นผู้สูงอายุที่เจ็บป่วยด้วยโรคติดเชื้อไวรัสโคโรนา 2019 ทำการเก็บข้อมูลภายหลังได้รับการรักษาและอยู่ในระยะที่ไม่สามารถแพร่กระจายเชื้อจำนวน 16 ราย เก็บรวบรวมข้อมูลโดยใช้เทคนิคการสัมภาษณ์เชิงลึก การสังเกต การบันทึกภาคสนามและการบันทึกเสียงสนทนา ข้อมูลที่ได้นำมาวิเคราะห์ข้อมูลเชิงเนื้อหาตามวิธีการของโคไลซี

ผลการวิจัย: อธิบายประสบการณ์ของการเจ็บป่วยเป็น 4 ประเด็นหลัก คือ 1) ประสบการณ์การรับรู้อาการเจ็บป่วย เริ่มแรกไม่ได้คิดว่าตนจะติดเชื้อไวรัสโคโรนา 2019 เมื่อต้องเจ็บป่วยจึงทำให้พบกับความไม่สุขสบายทั้งร่างกายและจิตใจ โดยจะมีอาการเหนื่อยมาก หายใจไม่ออก และเมื่อได้รับการรักษาแล้วพบว่าหลังจากการเจ็บป่วยร่างกายไม่เหมือนเดิม 2) ประสบการณ์ในการเข้ารับการรักษา ผู้สูงอายุรู้สึกว่าตนโชคดีที่เข้าถึงการรักษาแต่รู้สึกไม่ชินกับการรักษาที่ได้รับ ถูกจำกัดให้อยู่แต่ห้อง ทำกิจกรรมบนเตียงและได้รับการดูแลผ่านกล้องวงจรปิดและพูดคุยผ่านลำโพงจึงทำให้ขาดสัมพันธภาพกับผู้ดูแล 3) หลากอารมณ์ หลายความรู้สึกภายในใจ รู้สึกกังวล กลัวว่าจะไม่รอด เมื่อได้รักษาก็รู้สึกว้าเหว่ โดดเดี่ยว เนื่องจากต้องอยู่คนเดียวเพื่อป้องกันการแพร่เชื้อ แต่อย่างไรก็ตามการเจ็บป่วยครั้งนี้ทำให้ผู้สูงอายุรู้สึกปล่อยวาง ยอมรับและเข้าใจสภาพตามอายุที่เพิ่มขึ้น 4)ประสบการณ์ในการกลับเข้าสู่สังคม ภายหลังที่หายจากการเจ็บป่วย พบว่าถูกรังเกียจจากคนรอบข้างมีเพียงคนใกล้ตัวที่ให้กำลังใจ แต่อย่างไรก็ตามการเจ็บป่วยในสถานการณ์ที่มีการแพร่ระบาดของโรคติดเชื้อ ทำให้ผู้สูงอายุต้องปรับตนเองในการดำเนินชีวิต เพื่อป้องกันการติดเชื้อ

สรุป: ผลการวิจัยครั้งนี้ทำให้เข้าใจประสบการณ์การเจ็บป่วยของผู้ป่วยสูงอายุที่ติดเชื้อไวรัสโคโรนา 2019ซึ่งสามารถนำผลการศึกษาไปใช้เป็นแนวทางในการพัฒนาการพยาบาลแบบเป็นองค์รวมที่สามารถตอบสนองความต้องการของผู้ป่วยได้อย่างมีประสิทธิภาพ

Background: Facility-based long-term care provide a variety of health care services, including medication management. However, medication errors are common and such errors contribute to disability and mortality. Due to an inadequate number of nurses, Washington State passed the Medication Assistant Endorsement law, allowing certified nursing assistants (CNAs) with specialized training as medication assistants (MAs) to administer certain medications under a register nurse’s supervision in assisted living facilities and nursing homes. According to the Kentucky Nurses Association, 57% of surveyed Kentucky nurses are considering leaving their jobs. Best retention strategies are unclear. Job embeddedness (JE) and Intent to stay (ITS) are factors that influence retention. Background/Purpose: Physical Activity Guidelines for Americans recommend 150-300 minutes of moderate-intensity physical activity each week for adults; however, half of the adults do not meet this goal, which leads to increased chronic health conditions and poor health outcomes. Higher air pollution exposure and shorter leukocyte telomere length (LTL) are both associated with increased risk of coronary heart disease (CHD), and share plausible mechanisms, including inflammation. LTL may serve as a biomarker of air pollution exposure and may be intervened with to reduce the risk of CHD. To the best of our knowledge, we are the first to test the mediation effect of LTL in the relationship between air pollution exposure and incident CHD. การวิจัยครั้งนี้เป็นการวิจัยแบบหาความสัมพันธ์ มีวัตถุประสงค์เพื่อ ศึกษาความสัมพันธ์ระหว่างระยะของโรค ภาวะวิตกกังวลและซึมเศร้า กิจกรรมทางกาย ความสามารถในการประกอบกิจวัตรประจำวัน ภาวะโรคร่วม การสนับสนุนทางสังคม กับคุณภาพชีวิตด้านสุขภาพของผู้ป่วยสูงอายุโรคมะเร็งลำไส้และทวารหนัก กลุ่มตัวอย่าง คือ ผู้สูงอายุโรคมะเร็งลำไส้ใหญ่และทวารหนักที่เข้ารับการรักษาในโรงพยาบาลตำรวจ โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย และโรงพยาบาลราชวิถี จำนวน 94 คน เลือกกลุ่มตัวอย่างโดยวิธีการสุ่มแบบหลายขั้นตอน เครื่องมือที่ใช้ในการวิจัย คือ แบบสอบถามข้อมูลส่วนบุคคล แบบประเมินภาวะโรคร่วม แบบสอบถามอาการวิตกกังวลและอาการซึมเศร้าของผู้ป่วยในโรงพยาบาล แบบสอบถามการมีกิจกรรมทางกายสากล แบบประเมินกิจวัตรประจำวัน แบบประเมินการสนับสนุนทางสังคม และแบบประเมินคุณภาพชีวิตสำหรับผู้ป่วยโรคมะเร็งลำไส้ใหญ่และทวารหนัก แบบสอบถามเป็นแบบสอบถามที่ผ่านการตรวจความตรงเชิงเนื้อหา และเคยนำมาใช้อย่างแพร่หลายในการศึกษาในผู้ป่วยกลุ่มโรคมะเร็งในประเทศไทยมากก่อน และเหมาะกับบริบทผู้สูงอายุไทย ตรวจสอบความเที่ยงเท่ากับ 0.84, 0.78, 0.83, และ 0.89 ตามลำดับ วิเคราะห์ข้อมูลโดยหาค่าความถี่ ร้อยละ ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน ค่าสัมประสิทธิ์สหสัมพันธ์ของเพียร์สันและสเปียร์แมน ผลการวิจัย สรุปได้ดังนี้ 1. คุณภาพชีวิตด้านสุขภาพของผู้สูงอายุโรคมะเร็งลำไส้ใหญ่และทวารหนักอยู่ในระดับปานกลาง ( X =2.26; SD= 1.24) 2. ระยะของโรค กิจกรรมทางกาย การสนับสนุนทางสังคม ความสามารถในการประกอบกิจวัตรประจำวัน มีความสัมพันธ์เชิงบวกต่อคุณภาพชีวิตด้านสุขภาพในผู้ป่วยสูงอายุโรคมะเร็งลำไส้ใหญ่และทวารหนัก อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r= 0.757, 0.323, 0.608, และ 0.222 ตามลำดับ) 3. ภาวะโรคร่วม มีความสัมพันธ์เชิงลบต่อคุณภาพชีวิตด้านสุขภาพในผู้ป่วยสูงอายุโรคมะเร็งลำไส้ใหญ่และทวารหนัก อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r= -0.314) 4. ภาวะวิตกกังวลและซึมเศร้า ไม่ความสัมพันธ์กับคุณภาพชีวิตด้านสุขภาพของผู้สูงอายุโรคมะเร็งลำไส้ใหญ่และทวารหนัก

การศึกษาครั้งนี้เป็นการศึกษาหาความสัมพันธ์ มีวัตถุประสงค์ เพื่อศึกษาอาการหลงเหลือภายหลังการติดเชื้อไวรัสโคโรน่า 19 ในผู้สูงอายุ และศึกษาความสัมพันธ์ระหว่าง อายุ เพศ ภาวะอ้วน การสูบบุหรี่ โรคประจำตัว ประวัติการรับวัคซีนป้องกันโควิค-19 ก่อนการติดเชื้อ ความรุนแรงของอาการติดเชื้อโควิด-19 การรักษาที่ได้รับระหว่างพักรักษาตัวในโรงพยาบาล ความเครียด และกิจกรรมทางกายกับอาการหลงเหลือภายหลังการติดเชื้อไวรัสโคโรน่า 19 ในผู้สูงอายุ กลุ่มตัวอย่างคือ ผู้ที่มีอายุเกิน 60 ปีบริบูรณ์ขึ้นไปทั้งเพศชายและหญิงที่มีประวัติติดเชื้อไวรัสโคโรนา 2019 เป็นเวลาอย่างน้อย 4 สัปดาห์ขึ้นไปโดยนับตั้งแต่วันแรกของการติดเชื้อ และเคยเข้ารับการรักษาทั้งเป็นผู้ป่วยนอก ผู้ป่วยใน หรือหอผู้ป่วยเฉพาะกิจโควิด-19 ในโรงพยาบาลราชวิถี โรงพยาบาลตำรวจ และโรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย จำนวน 87 คน เลือกการสุ่มกลุ่มตัวอย่างแบบหลายขั้นตอน เครื่องมือที่ใช้ในการวิจัย คือ แบบบันทึกข้อมูลส่วนบุคคล แบบวัดความรู้สึกเครียด แบบสอบถามกิจกรรมทางกายระดับสากล และแบบสอบถามอาการหลงเหลือภายหลังการติดเชื้อ ไวรัสโคโรน่า 19 ในผู้สูงอายุ โดยเครื่องมือมีคุณภาพทั้งความตรงตามเนื้อหาและความเที่ยง วิเคราะห์ข้อมูลโดยใช้สถิติสัมประสิทธิ์สหสัมพันธ์อีต้า สถิติสหสัมพันธ์สเปียร์แมน และสถิติสหสัมพันธ์เพียร์สัน ผลการศึกษา พบว่า 1. อาการหลงเหลือภายหลังการติดเชื้อไวรัสโคโรน่า 19 ในผู้สูงอายุ พบมากที่สุด 10 อันดับแรก ได้แก่ อ่อนเพลีย/เหนื่อยล้า ไอ นอนไม่หลับ มีเสมหะ เจ็บคอ ปวดกล้ามเนื้อ มีน้ำมูก หายใจลำบาก ปวดตามข้อ และปวดหลัง 2. ภาวะอ้วน เพศ ความเครียด โรคประจำตัว มีความสัมพันธ์ทางบวกกับอาการหลงเหลือภายหลังการติดเชื้อไวรัสโคโรน่า 19 ในผู้สูงอายุ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (rxy=.69, η= .31, rxy=.33, η=.23 ตามลำดับ) 3. อายุ ประวัติการรับวัคซีนป้องกันโควิค-19 ก่อนการติดเชื้อ มีความสัมพันธ์ทางลบกับอาการหลงเหลือภายหลังการติดเชื้อไวรัสโคโรน่า 19 ในผู้สูงอายุ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (rs= -.23, rxy= -.16 …

Background. Identities and dynamics in relationships can change when one spouse develops dementia. This study aimed to understand the various types of goals both spousal care partners and persons living with dementia (PLWD) have for their relationship, and to explore whether approach or avoidance goals were associated with individual wellbeing and relational support – as well as their partners’ wellbeing and support.

To examine the association between self-reported relationship closeness, loneliness, and engagement in shared activities with a person living with dementia (PLWD) in a community sample of family members and friends of PLWD. Methods. Using data from a Qualtrics-administered online survey, we examined involvement in shared activities of 51 respondents who have or had a family, friend, or intimate relationship with a PLWD. Objectives – The objective of this paper was to investigate the associations between childhood parental bonding with current perceived caregiving burden and levels of perceived support from the parent among adult children of a parent living with early-stage dementia.Methods – One hundred and forty-four adult children completed self-report questionnaires that included the following measures: the Parental Bonding Inventory (PBI), Zarit Burden Interview (ZBI), and Perceived Partner Responsiveness Scale (PPRS). Objectives: This study aimed to examine the cross-sectional, longitudinal associations and trajectories between cognitive decline and blood pressure (BP) categories defined by the 2017 American College of Cardiology/American Heart Association guideline and to explore sex differences in cognitive decline trajectories in each BP category.Design, setting, and participants: The Atherosclerosis Risk in Communities study is a longitudinal cohort study (1987-2018). 10,456 of 15,792 participants (female 55.5%) were included in the analysis with a median follow-up time of 22.2 years.
