Background/Purpose: Chronic pain management protocols are necessary in all aspects of health care to reduce negative perceptions and improve knowledge and comfort when caring for patients with chronic pain.
Methods: A pre-implementation Simplified Knowledge and Attitude Survey Regarding Pain (KASRP) and Comfort Survey were administered to rural Midwest home health nursing staff followed by implementation of a chronic pain management protocol. The protocol included an educational session on understanding and managing chronic pain, the use of the OPQRST handout to guide proper documentation, Pain Risk Factors Assessment Form, and a Managing Chronic Pain handout. การวิจัยนี้เป็นการวิจัยแบบพรรณนาเชิงความสัมพันธ์ เพื่อศึกษาความต้องการการดูแลแบบสนับสนุนและศึกษาความสัมพันธ์ระหว่างปัจจัยคัดสรรได้แก่ อายุ สถานภาพสมรส ระดับการศึกษา ชนิดการรักษา ความวิตกกังวล ความทุกข์ทรมานจากอาการ และการสนับสนุนทางสังคมกับความต้องการการดูแลแบบสนับสนุนในผู้ที่เป็นมะเร็งเต้านมรายใหม่ กลุ่มตัวอย่าง คือ ผู้ที่เป็นมะเร็งเต้านมรายใหม่จำนวน 133 คนที่รับการรักษาในโรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย และโรงพยาบาลตำรวจ สุ่มกลุ่มตัวอย่างโดยวิธีเลือกตามสะดวก เครื่องมือที่ใช้ ประกอบด้วย ข้อมูลส่วนบุคคล แบบสอบถามความต้องการการดูแลแบบสนับสนุน แบบสอบถามความวิตกกังวลตามสถานการณ์ แบบสอบถามความทุกข์ทรมานจากอาการ และแบบสอบถามการสนับสนุนทางสังคม มีค่าดัชนีความตรงตามเนื้อหาดังนี้ แบบประเมินความต้องการการดูแลแบบสนับสนุน เท่ากับ 0.88 แบบประเมินความทุกข์ทรมานจากอาการ เท่ากับ 0.8 และแบบสอบถามการสนับสนุนทางสังคมเท่ากับ 0.9 และหาค่าสัมประสิทธิ์แอลฟาครอนบาค มีค่าเท่ากับ.96, .95, .85 และ 82 ตามลำดับ วิเคราะห์ข้อมูลโดยการหาค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และค่าสัมประสิทธิ์สหสัมพันธ์ ผลการวิจัยพบว่า 1. ผู้ป่วยมะเร็งเต้านมรายใหม่มีความต้องการการดูแลแบบสนับสนุนในระดับน้อย (mean = 2.39, SD = 0.78) เมื่อแยกรายด้าน พบว่า ด้านที่มีความต้องการสูงสุดคือระบบสุขภาพและข้อมูล มีความต้องการระดับปานกลาง(mean = 3.09, SD = 1.15) รองลงมาคือด้านการดูแลผู้ป่วยและการสนับสนุน (mean = 2.80 SD = 1.22) ขณะที่ด้านที่มีความต้องการน้อยที่สุดคือเพศสัมพันธ์ (mean = 1.60, SD = 0.74) 2. ความวิตกกังวลและความทุกข์ทรมานจากอาการมีความสัมพันธ์ทางบวกกับความต้องการการดูแลแบบสนับสนุนอย่างมีนัยสำคัญทางสถิติที่ .05 (r = .40, .41 และ .45 ตามลำดับ) การสนับสนุนทางสังคมมีความสัมพันธ์ทางลบกับความต้องการการดูแลแบบสนับสนุนอย่างมีนัยสำคัญทางสถิติที่ .05 (r = -.18) สำหรับปัจจัยที่ไม่มีความสัมพันธ์ได้แก่ อายุ สถานภาพ ระดับการศึกษา และการรักษา

การวิจัยครั้งนี้เป็นการศึกษาความสัมพันธ์เชิงทำนายเพื่อศึกษาปัจจัยทำนายความรู้สึกกลัวความก้าวหน้าของความเจ็บป่วยในบุคคลภายหลังเกิดภาวะกล้ามเนื้อหัวใจตายเฉียบพลัน กลุ่มตัวอย่าง คือ บุคคลที่ได้รับการวินิจฉัยจากแพทย์ว่ามีภาวะกล้ามเนื้อหัวใจตายเฉียบพลัน จำนวน 214 คน ที่มารับบริการที่แผนกผู้ป่วยนอกอายุรกรรมโรคหัวใจและหลอดเลือด โรงพยาบาลพระมงกุฎเกล้าและโรงพยาบาลนพรัตนราชธานี คัดเลือกกลุ่มตัวอย่างด้วยวิธีการสุ่มแบบเจาะจง เก็บข้อมูลโดยใช้แบบสอบถามทั้งหมด 9 ส่วน คือ 1) แบบสอบถามข้อมูลส่วนบุคคล 2) แบบสอบถามความรู้เกี่ยวกับโรคหลอดเลือดหัวใจ 3) แบบสอบถามการรับรู้อาการเจ็บหน้าอก 4) แบบสอบถามการรับรู้ความเจ็บป่วยฉบับย่อ 5) แบบสอบถามความรู้สึกกลัวความก้าวหน้าของโรคฉบับย่อ 6) แบบสอบถามความวิตกกังวลในผู้ป่วยโรคหัวใจ 7) แบบประเมินความรู้สึกไม่แน่นอนในความเจ็บป่วย 8) แบบสอบถามการรับรู้สมรรถนะแห่งตน และ 9) แบบสอบถามการสนับสนุนทางสังคม ผ่านการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิ 5 ท่าน ได้ค่าดัชนีความตรงตามเนื้อหาเท่ากับ .70, 1, .77, .83, .94, .91, .92 และ 1 ตามลำดับ และผ่านการตรวจสอบความเที่ยงของเครื่องมือเท่ากับ .70, .71, .84, .80, .81, .84 และ .89 ตามลำดับ วิเคราะห์ข้อมูลโดยคำนวณสัมประสิทธิ์สหสัมพันธ์พหุคูณ ผลการศึกษาพบว่า 1. บุคคลภายหลังเกิดภาวะกล้ามเนื้อหัวใจตายเฉียบพลันมีความรู้สึกกลัวความก้าวหน้าของความเจ็บป่วยในระดับน้อยคิดเป็นร้อยละ 23.8 มีความรู้สึกกลัวความก้าวหน้าของความเจ็บป่วยในระดับปานกลางคิดเป็นร้อยละ 57.9 มีความรู้สึกกลัวความก้าวหน้าของความเจ็บป่วยในระดับมากคิดเป็นร้อยละ 18.2 และบุคคลภายหลังเกิดภาวะกล้ามเนื้อหัวใจตายเฉียบพลันมีความรู้สึกกลัวความก้าวหน้าของความเจ็บป่วยโดยรวมอยู่ในระดับปานกลาง (Mean = 36.92, SD = 9.89) 2. ปัจจัยทำนายความรู้สึกกลัวความก้าวหน้าของความเจ็บป่วยในบุคคลภายหลังเกิดภาวะกล้ามเนื้อหัวใจตายเฉียบพลัน ประกอบด้วย 4 ตัวแปร ได้แก่ ความวิตกกังวล (Beta = .369) ความรู้สึกไม่แน่นอนในความเจ็บป่วย (Beta = .281) การรับรู้ความเจ็บป่วย (Beta = .119) และการรับรู้สมรรถนะแห่งตน (Beta = - .309) สามารถทำนายความรู้สึกกลัวความก้าวหน้าของความเจ็บป่วยในบุคคลภายหลังเกิดภาวะกล้ามเนื้อหัวใจตายเฉียบพลันคิดเป็นร้อยละ 69.6 (Adjusted R2 …

วิจัยนี้เป็นการวิจัยกึ่งทดลองแบบสองกลุ่มวัดแบบอนุกรมเวลามีการให้สิ่งทดลองซ้ำ มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการนวดกดจุดสะท้อนฝ่าเท้าต่ออาการปวดหลังส่วนล่างในผู้ป่วยภายหลังได้รับการตรวจสวนหัวใจ กลุ่มตัวอย่าง คือ ผู้ป่วยโรคหลอดเลือดหัวใจทั้งเพศชายและเพศหญิง อายุ 40 ปีขึ้นไป ที่ได้รับการตรวจสวนหัวใจผ่านหลอดเลือดแดงบริเวณขาหนีบ ณ โรงพยาบาลธรรมศาสตร์เฉลิมพระเกียรติ จังหวัดปทุมธานี เลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจงตามคุณสมบัติที่กำหนด แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลองกลุ่มละ 19 คน โดยจับคู่กลุ่มตัวอย่างให้มีคุณสมบัติคล้ายคลึงในด้านเพศ อายุ ชนิดและปริมาณยาลดปวดที่ได้รับ กลุ่มควบคุมได้รับการพยาบาลตามปกติ และกลุ่มทดลองได้รับโปรแกรมการนวดกดจุดสะท้อนฝ่าเท้า พัฒนาขึ้นโดยใช้กรอบแนวคิดทางสรีระวิทยาการเกิดอาการปวดหลังส่วนล่างในผู้ป่วยภายหลังได้รับการตรวจสวนหัวใจร่วมกับการทบทวนวรรณกรรม เครื่องมือที่ใช้ในงานวิจัย ได้แก่ แบบบันทึกข้อมูลส่วนบุคคล และแบบประเมินอาการปวดหลังส่วนล่างโดยใช้มาตรวัดความปวดด้วยสายตา (Visual analog scale) ผ่านการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิ 5 คน และได้ค่าความเที่ยงด้วยวิธีการวัดซ้ำเท่ากับ 0.83 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา สถิติการวิเคราะห์ความแปรปรวนแบบวัดซ้ำ และสถิติทดสอบค่าที ผลการวิจัยสรุปได้ดังนี้ 1. ผู้ป่วยภายหลังได้รับการตรวจสวนหัวใจในกลุ่มที่ได้รับโปรแกรมการนวดกดจุดสะท้อนฝ่าเท้า หลังได้รับโปรแกรมในชั่วโมงที่ 2, 4 และ 6 มีคะแนนอาการปวดหลังส่วนล่างน้อยกว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 2. ผู้ป่วยภายหลังได้รับการตรวจสวนหัวใจในกลุ่มที่ได้รับโปรแกรมการนวดกดจุดสะท้อนฝ่าเท้ามีคะแนนอาการปวดหลังส่วนล่างน้อยกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ ในชั่วโมงที่ 2, 4 และ 6 อย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05

การวิจัยแบบกึ่งทดลองครั้งนี้มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมพัฒนาความรอบรู้ด้านสุขภาพต่อพฤติกรรมป้องกันภาวะน้ำเกินในผู้ที่มีภาวะหัวใจล้มเหลว กลุ่มตัวอย่างเป็นผู้ที่มีภาวะหัวใจล้มเหลว เพศชายและหญิง อายุตั้งแต่ 18 ปีขึ้นไป คัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจงตามคุณสมบัติที่กำหนด โดยจับคู่ให้มีลักษณะคล้ายคลึงกันในเรื่องเพศ อายุ ระดับการศึกษา แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 30 คน โดยกลุ่มควบคุมได้รับการพยาบาลตามปกติและกลุ่มทดลองได้รับโปรแกรมพัฒนาความรอบรู้ด้านสุขภาพร่วมกับการพยาบาลตามปกติ เครื่องมือที่ใช้ในการวิจัย ได้แก่ แบบสอบถามพฤติกรรมป้องกันภาวะน้ำเกิน และแบบสอบถามประเมินความรอบรู้ด้านสุขภาพเกี่ยวกับหัวใจล้มเหลวและภาวะน้ำเกิน ผ่านการตรวจสอบจากผู้ทรงคุณวุฒิ 5 คน ได้ค่าดัชนีความตรงตามเนื้อหาเท่ากับ .95 และ .80 และมีค่าความเชื่อมั่นเท่ากับ .91 และ .93 ตามลำดับ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและเปรียบเทียบความแตกต่างด้วยสถิติทดสอบที ผลการวิจัยสรุปได้ดังนี้ 1. คะแนนเฉลี่ยพฤติกรรมป้องกันภาวะน้ำเกินในผู้ที่มีภาวะหัวใจล้มเหลวในกลุ่มทดลองหลังได้รับโปรแกรมพัฒนาความรอบรู้ด้านสุขภาพสูงกว่าก่อนได้รับโปรแกรมพัฒนาความรอบรู้ด้านสุขภาพอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. คะแนนเฉลี่ยพฤติกรรมป้องกันภาวะน้ำเกินในผู้ที่มีภาวะหัวใจล้มเหลวที่ได้รับโปรแกรมพัฒนาความรอบรู้ด้านสุขภาพสูงกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

This study was designed to determine whether the need for social support moderates the relationship between social support (both perceived and enacted) and depression, anxiety, and stress. The study tested the role of Need for Support in the context of both buffering and direct effect models of the effect of social support on mental health and well-being. If perceived availability of social support (i.e., PASS) had a direct effect on well-being, then a simple moderation was predicted. We think of scams primarily as a problem for older adults. Indeed, in the past few years, states and the federal government have undertaken a range of legal actions designed to prevent seniors, as a distinct class, from scams-- from more harshly punishing perpetrators of scams directed towards older adults to authorizing financial institutions to closely monitor and rapidly freeze the accounts of their older clients. But this successful, popular, and bipartisan law reform movement has taken place without a thorough empirical understanding of whether, in fact, seniors fall victim to scams more frequently than other age groups.

Resident care assistants (e.g., direct care workers, caregivers, personal care aides) provide the majority of services to assisted living, residential care and memory care-endorsed (ALF/RCF) residents. They faced significant burdens during the COVID-19 pandemic, which amplified existing challenges in some ALF/RCF communities, yet little is known about their work experiences. This qualitative study collected data through focus group and individual interviews with 21 resident care assistants and 12 external colleagues who have experience hiring, training or managing resident care assistants and other ALF/RCF staff, including registered nurse consultants, staffing agency operators, state policy staff, ALF/RCF community operations managers, and advocates. …

Introduction: Educational attainment, widely used in epidemiologic studies as a surrogate for socioeconomic status, is a predictor of cardiovascular health outcomes. Methods: A two-stage genome-wide meta-analysis of low-density lipoprotein cholesterol (LDL), high-density lipoprotein cholesterol (HDL), and triglyceride (TG) levels was performed while accounting for gene-educational attainment interactions in up to 226,315 individuals from five population groups. We considered two educational attainment variables: “Some College” (yes/no, for any education beyond high school) and “Graduated College” (yes/no, for completing a 4-year college degree). Genome-wide significant (p < 5 × 10−8) and suggestive (p < 1 × 10−6) variants were identified in Stage 1 (in up to 108,784 individuals) through genome-wide analysis, and those variants were followed up in Stage 2 studies (in up to 117,531 individuals). Results: In combined analysis of Stages 1 and 2, we identified 18 novel lipid loci (nine for LDL, seven for HDL, and two for TG) by two degree-of-freedom (2 DF) joint tests of main and interaction effects. Four loci showed significant interaction with educational attainment. Two loci were significant only in cross-population analyses. Several loci include genes with known or suggested roles in adipose (FOXP1, MBOAT4, SKP2, STIM1, STX4), brain (BRI3, FILIP1, FOXP1, LINC00290, LMTK2, MBOAT4, MYO6, SENP6, SRGAP3, STIM1, TMEM167A, TMEM30A), and liver (BRI3, FOXP1) biology, highlighting the potential importance of brain-adipose-liver communication in the regulation of lipid metabolism. An investigation of the potential druggability of genes in identified loci resulted in five gene targets shown to interact with drugs approved by the Food and Drug Administration, including genes with roles in adipose and brain tissue. Discussion: Genome-wide interaction analysis of educational attainment identified novel lipid loci not previously detected by analyses limited to main genetic effects.

In the present investigation, we investigated the role of three intensities of aerobic exercise (Low Intensity [LI], Moderate Intensity [MI], High Intensity [HI]) on IPC using a unique method to fractionate RPT into three components: (1) RT, which is the interval between the onset of the stimulus signal and the onset of muscle activity in the responding muscle (cognitive-decision making component), (2) movement time (MT), which is the time interval between the onset of muscle activation and completion of the required motor response, and (3) RPT, which encompasses both RT and MT. AbstractObesity is a chronic disease that plagues about 34% of the world population and rising (CDC,2023). Obesity develops over time due to lifestyle factors, biological factors, and the environment. Life course epidemiology approaches understanding the etiology of obesity by pinpointing how particular events in a lifetime can facilitate later life obesity. One emerging important early life factor that may influence adult obesity risk is adverse childhood experiences (ACEs). Rates of ACEs are higher in sexual minorities, women, and nonwhite individuals (Hughes et al., 2022). Furthermore, the rate of obesity is higher in women and nonwhite communities (CDC,2023). Student-run free clinics (SRFC) offer students the opportunity to learn and work with students from other disciplines; however, there is very little research on the benefits of SRFC to social work students. The purpose of this article is to describe the qualitative student outcomes of a service learning (SL) and interprofessional education (IPE) experience in a SRFC designed to enhance the interprofessional competencies of BSW social work students. Students (n=38) enrolled in a service-learning course and participated in a semester-long SL and IPE experience at a university-affiliated interdisciplinary SRFC. The concept of the American Dream and its promises of prosperity and social mobility, primarily through hard work, has been prevalent throughout modern U.S. history. However, what defines the American Dream has been arbitrary and varied, and research has shown that attitudes and beliefs surrounding the Dream have changed over time with shifts in the economic and political environments. Using data from 14 waves (2003–2016) of the Korean Labor and Income Panel Study (KLIPS) (N = 1,627 individuals aged 45–64; 22778 observations), in this study, we conducted sequence analysis and a multi-categorical variable mediation analysis (1) to examine to what extent long-term work histories exhibit varying degrees of de-standardization and precariousness using sequence analysis (2) to explore the potential mediating effects of work, material, and social environments in the association between multiple work sequences and self-rated health. This Case Report provides a detailed overview of the CoGenerate Innovation Fellowship, a 9-month fellowship that supports leaders of intergenerational initiatives developed by the national nonprofit organization, CoGenerate. It describes the Fellowship’s recruitment and cohort development strategies, monthly meetings and community-building efforts, end-of-Fellowship celebration, and post-Fellowship support. It summarizes findings from the two-year pilot’s process and outcomes evaluation and provides recommendations to individuals and organizations seeking to support social sector innovators who are leading intergenerational initiatives.

The population of older adults has been rising exponentially in recent years (Duffin, 2022), leading to an influx in people moving into retirement or independent living communities. This transition may cause many changes in the new residents’ occupational participation and overall well-being.
