Conventionally, tracheostomy includes surgical creation of tracheo-cutaneous communication. It is done either to bypass the upper airway obstruction or to facilitate optimal mechanical ventilation and early weaning off the ventilator support in neurologically unstable patients. For many years now, percutaneous tracheostomy has been proposed as an alternative to open surgical tracheostomy in ventilator-dependent patients. This review briefly narrates the principles and procedure of percutaneous tracheostomy. In addition, the article sheds light on a few critical problems that one could encounter during the process and discusses countermeasures to overcome those difficulties. Background: Since 2003 perinatal mortality rate of Malaysia is in range of 7.5 -8.5/1000. But higher deaths rates were observed in Jasin District from 2014-2017, hence this study was conducted to find out the prevalence and causes of perinatal mortality and suggest the steps to prevent it. Material and Methods: Cross-sectional study was conducted in Jasin district health office. Registered cases for delivery and birth records covering the period of 2014-2017 were checked. Post-coarctectomy syndrome is a complication of aortic arch repair that occurs due to a sudden increase in reperfusion of the body’s vasculature following surgery. One complication related to postcoarctectomy syndrome is ileus which can prolong a patient’s ICU stay and delay discharge home. While traditionally supportive care has been used to resolve this condition, this case highlights the use of gabapentin as a relatively novel treatment for post-coarctectomy syndrome gastrointestinal ileus.

A Rare Presentation Of Miller Fisher Syndrome, A Rare Variant Of Guillain Barre Syndrome, Saumya Mittal

Guillain Barre syndrome presents as acute onset of gradually progressive ascending flaccid paralysis with hyporeflexia or areflexia. The variants are acute inflammatory demyelinating polyneuropathy, acute motor axonal neuropathy, acute motor sensory axonal neuropathy, and Miller Fisher syndrome. Miller Fisher syndrome includes symptoms that form a classical triad of ophthalmoplegia, ataxia, and hyporeflexia or areflexia. This case presents a case of Miller Fisher syndrome with atypical features wherein the patient had normal reflexes and his reflexes remained normal in follow-up.

Animism and Interconnectivity is an ethnographic study of Batek Dè’ and Manya’ religion on the periphery of the Malaysian rainforest. In the twenty-first century, the lives of these two small-scale groups of former hunter-gatherers take place on the interconnected frontier between forest and the outside world, a nexus of different ideas, peoples and objects of diverse origins. Through an examination of specific events occurring in particular places on the forest periphery, the thesis highlights changes and continuities in shamanistic practices, myths, cosmologies and relations with other-than-human beings in the transformed physical and social landscape. Effects of electroshock treatment (EST) on the microstructural evolution and mechanical properties of near-β titanium alloy (Ti-55531) formed by directed energy deposition (DED) was studied in this work. With the increase in EST time, the average hardness of specimen decreased from 426 HV to 316 HV, and the fracture strain increased significantly, which was attributed to the uniform dispersion of α phase along grain boundaries and inside the β grains. After EST, the texture intensity decreased in terms of the orientation distribution function (ODF), which was ascribed to the redistribution of α phase. She presents herself as a positive role model for younger generations. Thunberg has many accomplishments as an extremely young woman that leads to her global iconicity. Measuring students’ perceptions of active learning activities may provide valuable insight into their engagement and subsequent performance outcomes. A recently published measure, the Assessing Student Engagement in Class Tool (ASPECT), was developed to assess student perceptions of various active learning environments. As such, we sought to use this measure in our courses to assess the students’ perceptions of different active learning environments. Initial results analyzed with confirmatory factor analysis (CFA) indicated that the ASPECT did not function as expected in our active learning environments. Sexual inequality is well researched in the academic sphere but is absent from the political realm. Previous research has identified that sex is defined for men’s pleasure, that women suffer from an orgasm gap, and that male domination is standard in heterosexual sex. Due to the private nature of sex, sexual inequality is untouched as a personal problem rather than deconstructed as a political injustice. However, the universality of sexual inequality in heterosexual relationships demonstrates that these overlooked personal problems are rooted in widespread cultural misogyny. We compared movement and space use patterns of foraging adult and juvenile Colorado checkered whiptails (Aspidoscelis neotesselatus) in pinyon pine–juniper woodland. Through focal observations, we characterized (1) movement paths as a series of step lengths and turn angles at 30-s intervals, and (2) habitat use reflected by the proportion of time spent in different vegetation types and in the open. Adults and juveniles differed in both movement and habitat use. Nesting Biology And Offspring Development Of The Cavity-Nesting Solitary Wasp Isodontia Elegans (F. Smith) From Trap-Nests In Oregon, Ronald C. Spendal, Kevin M. O'Neill, Casey M. Delphia

We studied the nesting biology and developmental schedules of the cavity-nesting solitary wasp Isodontia elegans in Oregon, USA. Trap-nests contained 1–15 brood cells each, separated by partitions, which adult females made with grass fragments. Before leaving nests to hunt, females made loose, temporary closures of longer fragments of dried grass and, upon returning, briefly deposited prey just inside the nest entrance before transferring them to a cell. Cells were provisioned with katydids (Meconema thalassinum, an invasive species in North America) and tree crickets (Oecanthus fultoni), the latter predominating as summers progressed. The Aransas-Wood Buffalo population of Whooping Cranes (Grus americana) migrates approximately 4000 km through the central Great Plains biannually, between their breeding and wintering grounds. Whooping Cranes depend on stopover sites to provide secure resting locations and the caloric resources necessary to complete their migration, such as the USFWS-designated critical habitat area in the Central Platte River Valley (CPRV) of Nebraska. This area includes braided river habitat characterized by low-elevation and submerged sandbars, which provide important roosting and foraging opportunities for migrating Whooping Cranes. Filamentation in air is a phenomenon that has been extensively investigated for the last two decades. At sufficiently high intensity, even air is a nonlinear medium. These intensities are reached with ultrashort pulses (50 to 100 fs) of more than 1 J energy, which self-focus in air, reach ionizing intensities of oxygen and nitrogen, creating a plasma that defocuses the beam. The air filament is a self-induced waveguide resulting from a balance of focusing and defocusing. Both spatial and temporal variability are key attributes of sedimentary microbial communities, and while spatial effects on beta-diversity appear to dominate at larger distances, the character of spatial variability at finer scales remains poorly understood, especially for headwater stream communities. We investigated patterns of microbial community structure (MCS) in biofilms attached to streambed sediments from two watersheds across spatial scales spanning ,1 m within a single stream to several hundred kilometers between watersheds. Software vulnerabilities have significant costs associated with them. To aid in the prioritization of vulnerabilities, analysts often utilize Common Vulnerability Scoring System’s Base severity scores. However, the Base scores provided from the National Vulnerability Database are subjective and may incorrectly convey the severity of the vulnerability in an organization's network. This thesis proposes a method to statically analyze context-aware network graphs to increase accuracy of CVSS severity scores. As space exploration technology advances and more private actors engage in independent space activities, national governments and international organizations need to reform and modernize their approach to regulating private space activities in order to better capture and distribute the benefits of those activities.
