Venous thromboembolism (VTE) with concurrent thrombocytopenia is frequently encountered in patients with cancer. Therapeutic anticoagulation in the setting of thrombocytopenia is associated with a high risk of hemorrhage. Retrospective analyses suggest the utility of modified-dose anticoagulation in this population. To assess the incidence of hemorrhage or thrombosis according to anticoagulation strategy, we performed a prospective, multicenter, observational study. Patients with active malignancy, acute VTE, and concurrent thrombocytopenia (platelet count/µL) were enrolled. The cumulative incidences of hemorrhage or recurrent VTE were determined considering death as a competing risk. There are many toys featuring the illustrations of Vernon Grant as well. For example, there is a xylophone, a record player, pick-up-stix set, and paper dolls. The purpose of this study was to uncover the lived experience of Lebanese oncology patients receiving palliative care. The study design was based on the Utrecht School of phenomenology based on the Dutch school of phenomenology. This study followed purposeful sampling in which ten participants six women and four men with a mean age of 54.4 years were selected. The proposed research was approved by the Institutional Review Boards at the American University of Beirut and Duquesne University. Background: The Accreditation Council for Graduate Medical Education (ACGME) common and specialty program requirements stipulate that residents/fellows participate in scholarly activities. The ACGME provides a reporting template tool for scholarly activity to be utilized by all specialties for evaluation annually. In this study, we aimed to: 1) Conduct a content analysis of the specialty scholarly activity requirements for all ACGME-accredited training programs to identify commonalities and variations, and 2) Assess alignment between the specialty scholarly activity requirements and the reporting template tool provided by the ACGME.

It is estimated that 70% of Americans would prefer to die at home, but less than one-quarter do so. Patients near the end of life often face several barriers that prevent them from successfully transitioning from an acute care setting to their preferred place of death. These barriers include but are not limited to a lack of trust between facilities and community care providers, ineffective communication, delayed discharge planning, difficulty accessing medications and supplies, lack of control, feelings of uncertainty, and feelings of burden. The goal of the dissertation is to undertake an analysis of substance use disorders that focuses on a public health ethics perspective. The ethical argument focuses upon justifying the use of harm reduction and is primarily concerned with the current opioid crisis. While substance abuse/misuse over the course of history has been identified as a public health concern, this dissertation presents substance use disorders over the course of the lifespan, examining various contexts in which it can affect daily living and health outcomes. Problem: Sleep disturbances in long-term care residents are associated with falls, behavioral problems, and increased mortality rates, yet long-term care facilities are traditionally noisy environments at night. Designated quiet hours have been shown to improve patient sleep as well as decrease stress levels in nurses.

Context: In a skilled nursing facility that provides care for military veterans with complex medical and psychosocial needs, signage exists that indicates to be quiet between 10:00 p.m. and 7:00 a.m., but no defined guidelines were available for staff to observe.

Problem: Patients identified on the regional database as specialty palliative care (SPC) patients with congestive heart failure (CHF) have not been consulted annually, as expected by evidence-based practice.

Context: Twenty-three percent of patients with a life-limiting illness on the SPC registry were seen year to date. Patients with CHF are vulnerable populations who benefit from an SPC consult to address quality of life, frequent admissions, and suffering.

In the United States, each year about 22,440 women are diagnosed with ovarian cancer, the deadliest gynecological cancer - with an estimated 14,080 deaths each year. Patients with recurrent ovarian cancer have more options today due to advances in treatments, and specifically, with the advent of immunotherapies. Most prior research on resilience to HIV/AIDS has utilized quantitative tools (e.g., scales and surveys) to examine individual-level assets (e.g., self-efficacy, hope, optimism) that researchers believe represent or approximate resilience to HIV/AIDS with minimal consideration for the perspectives of men who have sex with men (MSM), the population that has remained at greatest risk of, and the most impacted by HIV/AIDS in North America since the 1980s. A lack of comprehension of discharge instructions may cause high readmission and emergency room revisit rates for organizations. At the project site, there was no current evidence-based practice to ensure patient comprehension of discharge instructions. The purpose of this quantitative, quasi-experimental quality improvement project was to determine if the implementation of the Institute for Healthcare Improvement (IHI) Always Use Teach-Back Toolkit would impact emergency room revisit rates among adult medical-surgical patients in a critical access hospital in rural Minnesota over four weeks.
