Extension educators across the United States are being asked to expand their direct education efforts to include policy, systems, and environmental (PSE) changes. However, professional development opportunities and tools are needed to familiarize Extension professionals with PSE change approaches, build their capacity to implement PSE change, and make the process relevant to their work. We describe a planning tool developed for a unique multistate PSE change intervention training and designed to facilitate the process of PSE change implementation at the local level. An example of the tool and recommendations for others wishing to use it are included.

Policy, systems, and environmental (PSE) change interventions are a key part of comprehensive nutrition and health education. Although Extension educators find value in PSE approaches and report being involved in PSE work, many still indicate a lack of understanding regarding PSE approaches. We describe a unique multistate training designed to increase Extension educators' understanding, skills, and capacity related to implementing PSE change interventions. Data demonstrating success of the training are presented. Additionally, best practices for others wishing to create similar multistate programs conclude the article.

Through a community-focused needs assessment conducted in rural Appalachia, we gauged perceptions of facilitators and barriers related to healthful eating and physical activity for young children and identified suggestions for improvement. Thirty-seven key informant interviews and three caregiver focus group sessions were coded and analyzed for key themes. Limited community resources emerged as a barrier to both healthful eating and physical activity. Suboptimal communication about existing opportunities was also identified. Community members reviewed the needs assessment data and implemented initiatives to address identified needs. South Africa has not only been identified with high records of gender-based violence (GBV), but that, it takes a complex toll on young people than does transport accidents. Therefore, two methods: Pao’s Least Squared (LS) and Sen’s methods were used to determine the validity of GBV research output vis-a-vis Lotka’s law of scientific productivity over a ten-year window 2009-2018. Data on GBV scientific publications in South Africa was harvested from the EBSCO Discovery Service Database. In this paper we contribute an alternative conceptualization of creativity by highlighting movement as creating spatial and temporal dimensions that are important to make sense of creativity in children mathematical thinking. Using data from an international collaboration between two teaching-research teams from the United States and Chile, we trace how children mobilized their social bodies, materials, tools, images, metaphors, languages, and improvisations in order to make sense of the concept of fractions. The short gut syndrome can lead to multiple metabolic disorders along with chronic malnutrition. Such patients can develop changes in mental status due to metabolic alterations. We present a case of eleven years old male who presented to the hospital with an altered mental state. There were repeated admissions to the hospital with a similar complain with non-conclusive cerebrospinal fluid analysis and normal neuroimaging findings. The patient suffered from chronic malnutrition and multiple hospitalizations with altered mental status, encephalopathy. However, he responded well to bicarbonate therapy and prophylactic metronidazole treatment.

Marijuana use during pregnancy has dramatically increased in recent years, driven in part by the increasing legalization of marijuana for medicinal and recreational use. However, the impact of prenatal exposure to marijuana on a developing fetus is not clear. Previous studies examining this effect have been limited by polysubstance use during pregnancy and confounding factors that make data interpretation challenging. In this study, we propose a prospective cohort study that will address the limitations of prior studies and measure the association between prenatal marijuana exposure and adverse neonatal outcomes. Gestational diabetes is defined as impaired glucose tolerance with onset during pregnancy and affects approximately 8.2% of pregnancies. Recently, it has been shown that insulin resistance is a predictor of poor milk supply in women attempting to breastfeed. Some mothers with gestational diabetes who used metformin for optimal glycemic control during pregnancy have found that metformin use has improved their ability to produce milk after delivery; however, the effects of continuing metformin postpartum have not yet been studied. It is theorized that continued metformin use in the postpartum period in women with gestational diabetes may improve postpartum milk production. Using …

One-quarter of all emergency department visits in the United States results in peripheral intravenous line (PIV) placement for parenteral fluid administration (Fields, Piela, Au and Ku, 2014). When PIV access is delayed, critical care measures are also delayed. Meyer et al. (2014), reports the first attempt PIV access failure rate is approximately 25%. In the critically ill patient, timely PIV access may be the difference between survival and death. Intensification of forage legume strata system technology (FLSST) through dairy cattle production was implemented in collaboration with the Department of Agriculture and the local government unit of San Jorge, Samar, Republic of the Philippines. The beneficiaries of the program were selected based on specific criteria and qualifications. Farmers were given training on dairy cattle production, and 10 beneficiaries in five communities received one ready-to-breed cow. Each farmer-beneficiary established a 500 sqm pasture area using FLSST. Abundant grasses and leguminous forages helped improve the health of the cattle. Background
Soil-transmitted helminth (STH) infections have many negative health outcomes (e.g., diarrhea, nutritional deficiencies) that can also exacerbate poverty. These infections are generally highest among low-income populations, many of which are also undergoing market integration (MI; increased participation in a market-based economy). Yet the direct impact of MI-related social and environmental changes on STH infection patterns is poorly understood, making it unclear which lifestyle factors should be targeted to better control disease spread. This cross-sectional study examines if household infrastructure associated with greater MI is associated with lower STH burdens among Indigenous Ecuadorian Shuar.

The prevalence of chronic renal failure in Indonesia tends to increase in the lower age group (45–54 years).Chronic renal failure may lead to impaired sexual function. A descriptive phenomenology study with in-depth interviews was carried out with 12 participants, and thematic content analysis was applied. Six themes were revealed, as follows: 1) adaptation process to sexual dysfunction experienced, 2) sexual dysfunction experience,3)importance of fulfilling sexuality needs,4) behavior in dealing with sexual dysfunction, 5) perception of the cause of sexual dysfunction, and 6) participants’ expectation of health service related to sexual function. The turbulence in the diffuse intergalactic medium (IGM) plays an important role in various astrophysical processes across cosmic time, but it is very challenging to constrain its statistical properties both observationally and numerically. Via the statistical analysis of turbulence along different sight lines toward a population of fast radio bursts (FRBs), we demonstrate that FRBs provide a unique tool to probe the intergalactic turbulence. We measure the structure function (SF) of dispersion measures (DMs) of FRBs to study the multiscale electron density fluctuations induced by the intergalactic turbulence. Heart failure and hypertension are non-communicable diseases that are responsible for 70% of deaths world wide and cause anxiety and impaired body image. Nursing interventions(therapy in general) and acceptance and commitment therapy increase patients’ acceptance of the disease and commitment to alleviate anxiety and improve impaired body image. Meanwhile, family psychoeducation improves the family’s ability to care for the patient. This case report presents two patients with heart failure and hypertension. An integrated agriculture-based agrotourism model needs to be implemented in the Carik Injeman area of Cibodas village. To date, Carik Land, with an area of 30 hectares to date, has not been optimally used. Cibodas villagers mostly work as farmers and ranchers. More than 50% of the workforce does not have permanent employment. Some previous researchers have said that with integrated agriculture, independent communities could increase their land resources and farmers would prosper in a sustainable manner. In developing countries, on average, one out of five children die from pneumonia. Death from pneumonia is most prevalent in Southeast Asia, particularly in Indonesia. Many factors can cause pneumonia. In Jakarta, the prevalence period of pneumonia in toddlers reached 19.6‰, which is higher than the national prevalence period of only 18.5‰. This study used a cross-sectional research design to analyze the relationship between disease prevalence and risk factors simultaneously. Results showed that, out of 721 children under 5 years old investigated in this study, 31 toddlers suffered from pneumonia. Furthermore, bivariate analysis showed that two variables were related to …

In this body of research, reproductive and adrenal hormones and behavior were used to evaluate individual and group responses to physiological, social, and environmental changes in zoo-housed Asian elephants, considering factors of sex, age, and life stage. Animals experience physiological, social, and environmental changes as part of their natural history and individual life experience. Measures of both positive and negative states are needed to assess the impact of these changes at the individual and group level. RATIONALE: The elderly experience profound systemic responses after stroke, which contribute to higher mortality and more severe long-term disability. Recent studies have revealed that stroke outcomes can be influenced by the composition of gut microbiome. However, the potential benefits of manipulating the gut microbiome after injury is unknown.
OBJECTIVE: To determine if restoring youthful gut microbiota after stroke aids in recovery in aged subjects, we altered the gut microbiome through young fecal transplant gavage in aged mice after experimental stroke.
