Avoidable hospitalizations of hospice patients cost Medicare $3 billion a year. When hospice nurses are able to identify early signs and symptoms of acute illness and provide appropriate interventions to prevent such admissions, 20-60% of the hospitalizations are preventable. The practice problem addressed in this quality improvement doctor of nursing project was the 30% hospital admission rate of hospice patients as evidenced by chart review, admission data, and revocation data. Problem: In January 2019, a medium-sized acute care hospital in Northern California reported new safety concerns related to patients using illicit substances in the hospital.

Context: Leaving the hospital AMA is an increasing problem in acute care hospitals among patients who use illicit substances, with a rate of 25% to 30% (Ti & Ti, 2015). Grewal et al. (2015) conducted a study of over 1,000 illicit substance users who had experienced a hospitalization, where 43.9% reported use of illicit substances while in the hospital.

The setting for this DNP project was a 326-bed, suburban hospital in Northern California that was facing challenges with preventing falls with injury and rising sitter costs. Between May 2018 and June 2019, there were four sentinel events related to falls with significant injuries. This hospital was financially impacted by the growing cost of the expanding sitter program caused by the rising elderly patient population, with demographics requiring one-to-one, in-person sitters to observe them visually. Mediation analyses study the effect of exposures on outcomes thorugh mediators. With the develoment of high throughput technologies, high-dimenisonal data are increasinbl produced. To better control fro both Type I and II errors in high dimensional data, a screening method, emscreening, was developed using cross-validation. Through simulation studies, we examined the sensitivity and specificity of emscreening in different scenarios and compared them to two commonly used methods: the false-discovery rate (FDR) and Bonferroni methods. The propsed method showed higher sensitivity and comparable specificyt with the number of mediators tested was large (5,000 or 500,000). Overall, emscreening can identify significant mediators …

Globally consistent measurements of airborne metal concentrations in fine particulate matter (PM2.5) are important for understanding potential health impacts, prioritizing air pollution mitigation strategies, and enabling global chemical transport model development. PM2.5 filter samples (N ~ 800 from 19 locations) collected from a globally distributed surface particulate matter sampling network (SPARTAN) between January 2013 and April 2019 were analyzed for particulate mass and trace metals content. Metal concentrations exhibited pronounced spatial variation, primarily driven by anthropogenic activities. PM2.5 levels of lead, arsenic, chromium, and zinc were significantly enriched at some locations by factors of 100–3000 compared …

Hospitals with strong and consistently activated rapid response teams (RRTs) have significantly fewer cardiac arrests. Early recognition of clinical deterioration supports the timely activation of RRTs, which increases earlier assessment and intervention. Current early warning tools are not sufficient and reliable for recognizing patient deterioration, and they are evolving, incorporating artificial intelligence (AI) to identify clinical decline much earlier. The project organization had previously implemented the medical early warning score tool into the RRT nurses’ practice to prioritize patient assessments, but this was not sustained due to its unreliability in identifying patients at risk.

Sandusky Bay is largely populated by cyanobacterial algal blooms, mainly formed by Planktothrix. Fertilizers containing nitrogen and phosphorus run from agricultural lands into the bay, building up excess nutrients forming eutrophic waters. The Planktothrix feed off these nutrients and grow into algal blooms. To determine a potential solution to the growth of these blooms, I implemented a hydroponics system involving Lactuca Sativa for analysis of Planktothrix growth and productivity. Many college students who experience sexual assault experience subsequent (i.e., repeat) sexual assault incidents. There is also an established relationship between sexual assault and binge drinking. The “once bitten, twice shy” (OBTS) hypothesis suggests that those who experience alcohol- or drugrelated (AOD) sexual assault would reduce how frequently they binge drink in an effort to avoid repeat victimization. We test this hypothesis by analyzing two years of survey data collected from a panel of three cohorts of freshmen women. Supportive of OBTS, our analyses reveal that students who experienced an AOD-related sexual assault at time 1 only reduced the number …

Individuals with higher body dissatisfaction have higher disordered eating, excessive exercise, mental health concerns, and lower self-esteem. Gratitude interventions have been used to decrease body shame (objectified body consciousness), body dissatisfaction and increase positive body image. Gratitude interventions have primarily focused on changing state gratitude, leading to quick short changes. Individuals with long term, trait gratitude may have lower levels of excessive exercise, disordered eating, and a more positive body image. The current research is focused on better understanding the relationship between trait gratitude, body image, excessive exercise and disordered eating. The minority student population in higher education has been affected by food insecurity at a disproportionate rate. Several studies have captured some of the issues associated with the material deficit but fail to identify more in-depth contributing factors. Using the theoretical framework of intersectionality, the researcher examines the experience, interpretation, and navigation of food insecurity in a medium-sized university located in a major city on the west coast. While previous studies have looked at the effects of social media dependency on disordered eating symptoms, body image concerns, and anxiety and depression symptoms, few have looked at these effects on exclusively college students. The current study utilized a survey to examine the associations between social media dependency, disordered eating symptoms, body image concerns, and anxiety and depression symptoms. BACKGROUND: Necrotizing enterocolitis (NEC) remains one of the overall leading causes of death in premature infants, and the pathogenesis is unpredictable and not well characterized. The aim of our study was to determine the molecular phenotype of NEC via transcriptomic and epithelial cell-specific epigenomic analysis, with a specific focus on DNA methylation.

METHODS: Using laser capture microdissection, epithelial cell-specific methylation signatures were characterized by whole-genome bisulfite sequencing of ileal and colonic samples at the time of surgery for NEC and after NEC had healed at reanastomosis (n = 40).
