The Association Between Physiological Sources Of Pain And Sleep Quality In Older Adults With And Without Dementia, Crystal-Rae Waltons, Christine R. Kovach

The association of musculoskeletal pain, respiratory distress, gastrointestinal discomfort, and genitourinary pain to sleep quality in 89 persons with and without dementia was examined in the current exploratory, cross-sectional, observational quantitative design. Sources of pain were recorded, with 37.1% of the sample reporting urinary retention, 30.3% reporting genitourinary discomfort, and 53.9% reporting gastrointestinal discomfort. Mild scores of musculoskeletal pain and respiratory discomfort were elicited in >75% of the sample. An actigraph was used to measure sleep variables. Findings from this research illustrated dementia, gender, pillow use, respiratory distress, and urinary retention were statistically significantly associated with nighttime sleep quality. Results …

The pandemic of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) had overwhelmed the healthcare system worldwide with multiple ethical dilemmas. Several tools have been used to assess risk factors in these patients. One of them, the Clinical Frailty scale, has shown good correlation between the patient functional status and hospital stay with overall mortality. We present a case were the Clinical Frailty Scale was used to assess patient management and goals of care.

Older adult students are at risk academically. The senior citizen population and average life expectancy are increasing globally, which results in an increased number of adults returning to school later in life. Sense of community is related to academic engagement, achievement, interactivity, and retention. Self-efficacy is related to motivation, learning, and academic performance. Adipose tissue (AT) has diverse and important functions in body insulation, mechanical protection, energy metabolism and the endocrine system. Despite its relative abundance in the human body, the clinical significance of AT in musculoskeletal (MSK) medicine, particularly its role in painful MSK conditions, is under-recognized. Pain associated with AT can be divided into intrinsic (AT as a primary pain generator), extrinsic (AT as a secondary pain generator) or mixed origin. This study captured the various mental health impacts among youth, younger adults, and older adults in the LGBTQI community. The LGBTQI community embodies a diverse set of subgroups that includes youth, younger adults, and older adults. Many in the LGBTQI community face prejudice, discrimination, harassment, and even rejection from family members. Youth and younger adults in LGBTQI communities are often victims of school violence, as well as intimate partner violence, and typically show signs of psychosocial adjustment disorder and psychological distress. LGBTQI older adults, although have a high incidence of social isolation and depression, are shown to be more resilient …

Federal Sentencing Disparities And Marginalized Offenders: Revisiting Cumulative Disadvantage Theory Through Individual-Level Variables, April Terry, Ashley Lockwood

Over the past several decades, sentencing reforms have claimed to establish guidelines to reduce sentencing disparity; yet, recent studies continue to find discrepancies in sentencing outcomes. The current study explored individual factors using data from the U.S. Sentencing Commission (FY 2010) to further analyze these variables through the lens of cumulative disadvantage theory. The factors included the influence of age, race, sex (gender), offense type, instant offense score, and overall criminal history score on sentencing length (in months). Hierarchical regression revealed being identified as Black, committing fraud/white collar crime or a property offense, and overall criminal history were able to …

Background Diet and inflammation have both been studied in relation to type 2 diabetes mellitus (T2DM). The aim of this cross-sectional study was to examine the association between the Dietary Inflammatory Index (DII®) and T2DM. Methods Subjects were adults enrolled in the baseline study of the Xinjiang multi-ethnic natural population cohort and health follow-up study from January to May 2019. The study involved 5,105 subjects (58.7% men) between 35 and 74 years of age. The DII score was calculated from a data obtained via a food frequency questionnaire consisting of 127 food items. Background and Objectives: In the absence of effective pharmacotherapy, there is an urgent need to test evidence-based dementia care interventions using pragmatic trial approaches. We present results from a study in which an evidence-based, nonpharmacologic intervention for persons living with Alzheimer's disease and related dementia (ADRD) and their informal caregivers, Care of Persons with Dementia in their Environments (COPE), was tested in a Medicaid and state revenue-funded home and community-based service (HCBS) program.

We have previously demonstrated hippocampal hyperglutamatergic signaling occurs prior to plaque accumulation in AβPP/PS1 mice. Here, we evaluate 2-Amino-6-(trifluoromethoxy) benzothiazole (riluzole) as an early intervention strategy for Alzheimer's disease (AD), aimed at restoring glutamate neurotransmission prior to substantial Beta amyloid (Aβ) plaque accumulation and cognitive decline. Male AβPP/PS1 mice, a model of progressive cerebral amyloidosis, were treated with riluzole from 2-6 months of age. Morris water maze, in vivo electrochemistry, and immunofluorescence were performed to assess cognition, glutamatergic neurotransmission, and pathology, respectively, at 12 months. Four months of prodromal riluzole treatment in AβPP/PS1 mice resulted in long-lasting procognitive effects and …

Researchers request a variety of levels of engagement from their participants in a research study. This can range from merely serving as a data generation opportunity to being actively involved in each stage of the project. The latter is a co-researcher. In this paper, I explore how nine participants in a qualitative case study became co-researchers by the study’s conclusion. The increase in their active participation required methodological alterations to the project while I conducted the study. This article focuses on the responsive management of municipal leadership by identifying organizational and community values that affect age friendly policy making. The data comes from a sample of 1050 cities extracted from a national list of cities identified as place geography on the U.S. census list of geographies. The web-based questionnaire explored policy choices of 331 respondents in the areas of mobility, housing, the built environment, and public service delivery administered between May and August 2016. Palliative care is specialized medical care for people living with a serious illness. This type of care is focused on providing relief from the symptoms and stress of the illness. The goal is to improve the quality of life for both the patient and the family. When incorporated early, end of life care is improved. Additionally, there is a cost of care improvement, that palliative care has the capacity to substantially reduce healthcare expenditures among advanced cancer patients. Furthermore, the cost reduction depends on the timing of the palliative care consult.

Human gait is essential for long-term health monitoring as it reflects physical and neurological aspects of a person's health status. In this thesis, we proposed a non-invasive video-based gait analysis system to detect abnormal gait, and record gait and postural parameters on a day-to-day basis. The system does not require subjects to wear any sensors or attach markers on their body, therefore it is convenient to use in daily life. It takes videos captured from a single camera as input. Background: Current health care policy supports occupational therapy services with individuals experiencing homelessness in primary care settings. Research on the impact of interventions is needed to support this emerging practice area.
Method: A retrospective, descriptive study was completed following the initiation of full-time occupational therapy services in an integrated primary setting and Federally Qualified Health Center (FQHC) for clients experiencing homelessness. Typically, gait rehabilitation uses an invariant stimulus paradigm to improve gait related deficiencies. However, this approach may not be optimal as it does not incorporate gait complexity, or in more precise words, the variable fractal-like nature found in the gait fluctuations commonly observed in healthy populations. Aging which also affects gait complexity, resulting in a loss of adaptability to the surrounding environment, could benefit from gait rehabilitation that incorporates a variable fractal-like stimulus paradigm. SIGNIFICANCE: Macular pigment (MP) confers potent antioxidant and anti-inflammatory effects at the macula; however, its optical density in the eye is not routinely measured in clinical practice.
PURPOSE: This study explored a range of surrogate biomarkers including anthropometric, clinical, and plasma measures that may be associated with lower MP optical density (MPOD).
METHODS: Two thousand five hundred ninety-four subjects completed a full MP assessment as part of wave 1 of The Irish Longitudinal Study of Aging.
