Purpose: This study aimed to examine whether long-term fish oil (FO) supplementation is associated with a lower risk of mobility disability and enhances benefits of physical activity (PA). Methods: A total of 1635 sedentary adults age 70 to 89 yr from the Lifestyle Interventions and Independence for Elders single-blinded randomized,multicenter clinical trial, which compared a structured PA program to a health education program. Primary outcome was incident major mobility disability (MMD), defined by loss of ability to walk 400 m, measured every 6 months for an average of 2.6 yr. Cacrep Accreditation Simulation: Transformative Learning In Counselor Education, Molly M. Strear, Jennifer L. Murdock Bishop, Heather M. Helm
Journal of Counselor Preparation and Supervision
A CACREP accreditation simulation activity is provided as a framework for counselor educators to facilitate experiential learning for doctoral students in counselor education. This article includes instructional strategies for self-directed learning and reflective journaling to introduce program development and accreditation processes throughout a semester length assignment. Participating students embody the roles of fictional faculty members embarking on accreditation to promote student collaboration and increased knowledge of CACREP standards through transformative learning.

Background: This study aims to assess the decline in telomere length (TL) with age and evaluate effect modification by gender, chronic stress, and comorbidity in a representative sample of the US population. Methods: Cross-sectional data on 7826 adults with a TL measurement, were included from the National Health and Nutrition Examination Survey, years 1999–2002. The population rate of decline in TL across 10-year age categories was estimated using crude and adjusted regression. Results: In an adjusted model, the population rate of decline in TL with age was consistent and linear for only three age categories: 20–29 (β = -0.0172, 95% …

Communicating The Significance Of Home: Stroke Survivors And Caregivers Share Their Stories, Donna Pawlowski

The goal of this qualitative study was to examine how stroke survivors and caregivers communicated the significance of home in the short-term transition from hospital discharge to home care. Narratives from 12 stroke survivors and their caregivers produced five themes: marker events as motivation, comfort of home and family, emotional privacy of home, excited anticipation of going home, and nervous preparation for home. While much of the communication focused on the positive and hopeful recovery of stroke survivors upon going home, caregiver preparedness and the modifications to home impacted physical and emotional recovery. This article brings forth the experiential knowledge and views of sixteen Finnish older adults, (mean of age 84,9), who tried out home technology in their daily life, with the objective of discussing media literacy education in relation to this specific age group and context. The data were analyzed from the perspective of sensemaking as theorized by Karl Weick, which works as a heuristic enabling investigation of older adults’ experiences in relation to home technology. BACKGROUND: DNA methylation is implicated in coronary heart disease (CHD), but current evidence is based on small, cross-sectional studies. We examined blood DNA methylation in relation to incident CHD across multiple prospective cohorts.
METHODS: Nine population-based cohorts from the United States and Europe profiled epigenome-wide blood leukocyte DNA methylation using the Illumina Infinium 450k microarray, and prospectively ascertained CHD events including coronary insufficiency/unstable angina, recognized myocardial infarction, coronary revascularization, and coronary death. Cohorts conducted race-specific analyses adjusted for age, sex, smoking, education, body mass index, blood cell type proportions, and technical variables. We conducted fixed-effect meta-analyses across cohorts.

Previous research suggests a relationship between life satisfaction and healthcare utilization or HCU (Habibov & Afandi, 2016; Kim, Park, Sun, Smith, & Peterson, 2014). However, no study was found that examined this relationship among elderly American immigrants. The purpose of this study was to determine whether life satisfaction is linked to HCU for this population. A variety of caregivers, including grandparents, help raise children. Among grandparents, most Western samples evidence a matrilateral (i.e., mother’s kin) bias in caregiving, and many studies show more positive impacts and stronger relationships with grandmothers than grandfathers. The aim of the present study is to test competing hypotheses about a potential laterality bias and explore contrasts between grandmothers and grandfathers in a sample of urban young adult university students in Bangalore, India. A sample of 377 (252 women) relatively mobile and high socioeconomic status individuals 17 to 25 years of age completed a survey consisting of sociodemographic and grandparenting questions. …

This article inserts Aldous Huxley's Brave New World (1932) into a bioethical conversation about the value of old age and old people. Exploring literary treatments of bioethical questions can supplement conversations within bioethics proper, helping to reveal our existing assumptions and clear the way for more considered views; indeed, as Peter Swirski has argued, literary texts can serve as thought experiments that illuminate the ramifications of philosophical ideas. Modern humans have more fragile skeletons than other hominins, which may result from physical inactivity. Here, we test whether reproductive effort also compromises bone strength, by measuring using computed tomography thoracic vertebral bone mineral density (BMD) and fracture prevalence among physically active Tsimane forager-horticulturalists. Earlier onset of reproduction and shorter interbirth intervals are associated with reduced BMD for women. Tsimane BMD is lower versus Americans, but only for women, contrary to simple predictions relying on inactivity to explain skeletal fragility. Minimal BMD differences exist between Tsimane and American men, suggesting that systemic factors other than fertility (e.g. diet) do not …

Medical aid in dying (MAID) is a process by which individuals with terminal illness can voluntarily ingest a lethal dose of medication provided to them by their physician to intentionally end their life. MAID is currently legal in eight U.S. states and several other countries. Licensed psychologists and other mental health professionals are implicated in MAID laws in the form of psychological evaluation that is required for select patients. Background: Delirium is a common complication in the ICU setting and has serious long-term consequences. Increased mortality and long-term cognitive issues are associated with under-recognition and lack of treatment for delirium. Nursing staff must identify delirium and manage the event, in addition to providing care to other assigned patients.

Introduction: Dog guides were found to be effective in helping adults with vision loss navigate in the community and improve overall well-being. In spite of the vast amount of literature on pet therapy and dog companionship, limited study exists on older adult with vision loss experience of owning a dog guide. The purpose of this study was to understand how rural-dwelling elderly people describe their health and to gain an understanding of rural community processes as they relate to the health of older residents. Although community health nursing practice supports community-level interventions, the influence of communities upon the health of older community members is not well known. Ethnographic design and methods were employed to study the health of older residents in a Western Plains farming community of about 500 people.

“I Have A Job... But You Can’T Make A Living”: How County Economic Context Shapes Residents’ Livelihood Strategies, Marybeth J. Mattingly, Jessica A. Carson

This study explores how rural residents’ livelihood strategies are shaped by community economic and population characteristics. We use qualitative data from interviews and focus groups with low-income residents and social service providers (N=85 participants) in two rural New England counties to understand livelihood strategies within rural places. We then employ quantitative data to understand how these strategies are shaped by local historical labor markets and demographic characteristics. Although one county attracts wealthy retirees, with corresponding work opportunities in the service sector, and the other is remote and losing population, low-income workers in both places are struggling to make ends meet. …

Problem: Hospital falls are a growing national patient safety concern that cause anxiety, pain, distress, serious injuries, and increased health care utilization. Despite the presence of a well-developed falls prevention protocol since 2017. Internal data from an inpatient medical-surgical telemetry (MST) unit indicate the largest number of fall-related events among the hospital’s departments.
Context: Practice improvement project was initiated in a 217-bed community hospital to determine barriers and potential success factors.
