Corticosteroid Weaning In Stable Heart Transplant Patients: Guidance By Serum Cortisol Level, David A. Baran, Cheryl Rosenfeld, Mark J. Zucker

Background: Despite earlier studies describing the feasibility of steroid weaning in heart transplant patients, the majority of patients are maintained on steroid therapy for life. We examined a strategy based on a single morning serum cortisol measurement.

Methods: We assigned stable posttransplant patients, who were maintained on tacrolimus, mycophenolate mofetil, and corticosteroids, into one of two groups based on a screening morning cortisol level. Patients with a cortisol < 8 micrograms/deciliter were assigned to a "maintenance" group and the others were assigned to the weaning group and steroids were tapered off over 4-6 weeks. Patients were monitored on subsequent office visits for adrenal insufficiency and allograft rejection.

Results: Thirty-one patients were enrolled (6 patients in the maintenance group and 25 in the steroid-weaning group). Mean follow-up was 10.2 ± 4 years for the weaning group and 9.0 ± 4.9 …

This paper studies a high frequency inverter for low power applications at a short distance. Class 𝐸𝐹2 inverter is selected and designed as a transmitting driver. A printed spiral coil is designed as a transmitting coil and is connected to the high frequency inverter. Experimental results are obtained to compare with simulation results. The experiment and simulation are in good agreement and the circuit design is verified. Operating frequency is 2.75 MHz. Maximum flux density is generated by transmitting coil is measured 149uT.

This chapter studies an inductive-based wireless power transfer system for low-power applications at short distances. The transferring power system has been modeled, simulated and analyzed via finite element method. A wireless power transfer system includes important parts such as coil, core and driver. In this chapter, the important parts of an inductive power transfer system have been analyzed. Receiving and transmitting printed spiral coils are designed in an optimized procedure. This paper explores the question of the unity of Transcendental Idealism at the end of Eighteenth Century German philosophy, given that it circulated in different versions, Kant’s Critique [of humans’ rational powers] and Fichte’ System of Science [Wissenschaftslehre]. Both thinkers take the transcendental turn. They base conceptual investigations not on facts or empirical evidence, but on the possibility of a situation; they are idealists since they look inward to the spontaneity of the agent/knower for explanation, not the environment, stimulus, or sensory given. Reason can fathom only what it has constructed.

Colleen Murphy’s impressive book presents a unified theory of transitional justice as a single, novel, distinct kind of justice, intended to guide normative evaluation of the choices transitional societies make in dealing with the past. I raise three central challenges to Murphy’s theory. First, how do we know that transitional justice is fundamentally a single special kind of justice that permits a grand unified theory? Second, is it plausible to hold, as Murphy claims, that societal transformation is the overarching aim or objective of transitional justice? Climate change represents a global commons problem, where individuals, businesses, and nation-states all lack sufficient incentives to reduce their greenhouse gas emissions to levels consistent with meeting their collectively agreed upon mitigation goals. This column describes a process for integrating information literacy (IL) and evidence-based medicine (EBM) content within a new school of medicine curriculum. The project was a collaborative effort among health sciences librarians, curriculum deans, directors, and faculty. The health sciences librarians became members of the curriculum committees, developed a successful proposal for IL and EBM content within the curriculum, and were invited to become course instructors for Analytics in Medicine. Orthorexia nervosa has been suggested as a new psychological phenomenon in which an individual restricts their diet to consume only biologically pure and healthy foods. Some have suggested that orthorexia nervosa should be considered a new eating disorder. Inconsistencies have been identified regarding the executing functioning (EF) profile of children with ADHD and comorbid internalizing symptoms. Some research has found that internalizing symptomology may buffer against EF impairment among children with ADHD whereas other findings suggest a cumulative effect resulting in greater EF impairment. Similarly, symptoms indicative of emotional dysregulation are often identified in those with ADHD, both with and without internalizing symptoms. However, the impact of emotional dysregulation on EF is unclear. The purpose of this quantitative study was to determine whether factors of social comparison theory influence public school choice in Indiana. There were two arms in the study. One arm used three district-level variables (parent Education level, socioeconomic status, and homogeneity of race) associated with social comparison theory (SCT) and five district-level variables (prevalence of advanced courses, rate of perceived safety, graduation rate, passing rate on the state achievement test, and athletic success) associated with traditional measures of school quality (SQ) to address three research questions. In this Article, Professor Stewart Chang, through the examination of three recent mass shooting, proposes that mass shootings driven by hegemonic masculinity should be classified and addressed as acts of terrorism. Professor Chang defines hegemonic masculinity as patterns or practices that promote the dominant social position of men and the subordinate social position of women and other gender identities. In this Article, he examines how hegemonic masculinity is allowed to become mainstream and flourish unchecked based on our characterization, classification and reaction to mass shootings and their perpetrators.

One of the most positive responses to heightened federal enforcement of immigration laws has been increasing local and philanthropic interest in supporting immigrant legal defense. These measures are tentative and may be fleeting, and for the time being are not a substitute for federal support for an immigration public defender system. This Article proposes a methodology for interpreting the Supreme Court's long-standing inconsistency in the application of the Chevron doctrine. Developing such an approach is important because this central, canonical doctrine in administrative law is entering a period of uncertainty after long seeming to enjoy consensus support on the Court. In retrospect, it makes sense to view the many cases in which the Court failed to apply Chevron consistently as signals of underlying doctrinal doubt. Eve Hanan addresses how implicit cognitive biases may affect judges when they decide whether to credit defendants' displays of remorse and how we can lessen the effects of that bias. Recent disputes in Ohio and Nevada about whether lawyers should be allowed to wear “Black Lives Matter” pins in open court expose a fault line in First Amendment law. Results of a centrifuge test on a pile-supported wharf were used to investigate the time-, depth-, and row-dependent nature of kinematic and inertial loading on wharf piles in sloping rockfill. P-y models were calibrated against recorded bending moments in different piles and different depths. It was found that full kinematic demands and full superstructure inertia should be combined to estimate bending moments at pile head and shallow depths (less than 10 diameters below the ground surface). Effective-stress nonlinear dynamic analyses (NDA) were performed for a large-diameter reinforced concrete (RC) pile in multi-layered liquefiable sloped ground. The objective was to assess the effects of earthquake duration on the combination of inertia and liquefaction-induced lateral spreading. A parametric study was performed using input motions from subduction and crustal earthquakes covering a wide range of motion durations. The NDA results showed that the pile head displacements increased under liquefied conditions, compared to nonliquefied conditions, due to liquefaction-induced lateral spreading. The NDA results were used to develop a displacement-based equivalent static analysis (ESA) method that combines inertial and lateral spreading …
