BACKGROUND: Value-based health care aims to optimize the balance of patient outcomes and health care costs. To improve value in perinatal care using this strategy, standard outcomes must first be defined. The objective of this work was to define a minimum, internationally appropriate set of outcome measures for evaluating and improving perinatal care with a focus on outcomes that matter to women and their families.
METHODS: An interdisciplinary and international Working Group was assembled. Existing literature and current measurement initiatives were reviewed. Serial guided discussions and validation surveys provided consumer input. A series of nine teleconferences, incorporating a modified Delphi …

Antibiotic is a type of medication that helps in fighting bacterial infection. Treating bacterial infections in clinical setting become more complicated and costly due to drug resistance. This study was conducted to determine the antibacterial potential of Psidium guajava Linn leaf extracts against Gram-positive and Gram-negative bacteria. P. guajava Linn leaf was obtained from Research Orchards at University Putra Malaysia (UPM). Leaves were extracted using three types of extracts; hot, cold and methanol extract. The objective of this research is to examine the role that digital media usage plays in elections, specifically the Illinois State Senate Elections during the 2018 midterms. This research argues that the use of multiple digital mediums can affect election percentages at the end of the campaign. Taking into account the fact that the target base of each campaign varies across Illinois, the greater usage of multiple digital mediums to reach voters will correlate with a positive rise in election percentage regardless of target base. The U. S. Gulf of Mexico is experiencing a dramatic increase in tidal marsh restoration actions, which involves planting coastal areas with smooth cordgrass (Spartina alterniflora) and black needlerush (Juncus roemerianus) for erosion control and to provide habitat for fish and wildlife. It can take decades for sedimentary cycles in restored marshes to approach reference conditions, and the contribution of the sediment microbial communities to these processes is poorly elucidated. Student Growth Percentiles have recently become popular as a way to report student progress over time. Because of the perception that they are relatively simple to understand, they have also become a popular tool to use when evaluating teachers and school-based administrators, even though this is not the purpose of their design. Interindividual variation in the composition of the human gut microbiome was examined in relation to demographic and anthropometric traits, and to changes in dietary saturated fat intake and protein source. One hundred nine healthy men and women aged 21 to 65, with BMIs of 18 to 36, were randomized, after a two-week baseline diet, to high (15% total energy [E])- or low (7%E)-saturated-fat groups and randomly received three diets (four weeks each) in which the protein source (25%E) was mainly red meat (beef, pork) (12%E), white meat (chicken, turkey) (12%E), and nonmeat sources (nuts, beans, soy) (16%E). A method of treating and preventing a neurological disorder, such as Alzheimer's disease, in a subject in need thereof by positioning an electromagnetic field emitting source proximal to the subject and exposing the subject to an electromagnetic field having a predetermined frequency (preferably â300-3,000 MHz) for a predetermined absorption period (preferably greater than â3 days). Each individual treatment (comprising exposure to the predetermined frequency for the predetermined absorption period) is continued at a predetermined schedule for a predetermined treatment period. While many developing countries have reported gender gaps in education, Bangladesh has made remarkable progress and terminated the gender gap in secondary school enrollment through its national Female Stipend Program (FSP) in the 1990s. Conditional Cash Transfer programs (CCTs) like the FSP have become a popular development policy prescription, but the literature on CCTs in South Asia is surprisingly limited. A similar program to the FSP, the Female Secondary School Stipend, was implemented in the Punjab province of Pakistan in 2004 and had modest success, increasing secondary school enrollment for girls by 10%. Accessibility to health care, especially complex surgical care, represents one of the major health care disparities in developing countries. While surgical teams may be willing to travel to these areas to help address these needs, there are many logistical and ethical dilemmas inherent in this pursuit. We reviewed our approach to the establishment of the team-based surgical outreach program, wherein we perform head and neck free tissue transfer surgery in Haiti. We describe the challenges encountered in the delivery of surgical care as well as ethical dilemmas relevant to surgical outreach trips, highlighting an approach reliant on strong local cooperation. …

In this brief, authors Mark Ducey, Kenneth Johnson, Ethan Belair, and Barbara Cook combine demographic, land-cover, and other spatial data to estimate the incidence and extent of conversion from greenspace (forestland, shrublands, and grasslands) to development in the Great Lakes states. They report that greenspace conversions to developed land are most common in areas where greenspace is already limited. Population density strongly influences the conversion of greenspace to development. Conversions are most likely to occur on the urban periphery and in high-amenity rural areas. For several hundred years, the scholarly discussion of temperance was dominated by the legacy of classical models. This dissertation, titled A Poetics of Temperance: Gender, Genre, and Meaning-Making in Early Modern England, develops said discussion in a new way by forging a language about ethics and aesthetics into a poetics of temperance. The term poiesis derives from Greek and signifies “to make, create, produce” (OED). Accordingly, Aristotle uses the Greek poietes, “from which our word ‘poet’ derives,” to denote a “maker” (Murray xxxii). In brief, this project understands the role of women in emergent genres as …

One of the most consistently observed phenomena in autobiographical memory research is the reminiscence bump: a tendency for middle-aged and elderly people to access more personal memories from approximately 10–30 years of age. This systematic review (PROSPERO 2017:CRD42017076695) aimed to synthesize peer-reviewed literature pertaining to the reminiscence bump. The researchers conducted searches in nine databases for studies published between the date of inception of each database and the year 2017. Keywords used included: reminiscence, bump, peak, surge, blip, reminiscence effect, and reminiscence component. Sixty-eight quantitative studies, out of 523, met the inclusion criteria. Glycosaminoglycans (GAGs) are a class of linear polysaccharides that are ubiquitous in the extracellular matrix (ECM) and on cell surfaces. Due to their key role in development, homeostasis, pathogenesis, and regeneration, GAGs are increasingly used in the design of ECM-mimicking hydrogels to stimulate tissue formation and regenerative processes via specifically orchestrated cell-instructive signals. These applications first and foremost build on the ability of GAGs to effectively bind, protect, and release morphogens. The specificity and strength of morphogen-GAG interactions are largely governed by the number and spatial distribution of negatively charged sulfate groups carried by GAGs. Alphaviruses are enveloped pathogens that cause arthritis and encephalitis. Here, we report a 4.4-Å cryoelectron microscopy (cryo-EM) structure of eastern equine encephalitis virus (EEEV), an alphavirus that causes fatal encephalitis in humans. Our analysis provides insights into viral entry into host cells. The envelope protein E2 showed a binding site for the cellular attachment factor heparan sulfate. The presence of a cryptic E2 glycan suggests how EEEV escapes surveillance by lectin-expressing myeloid lineage cells, which are sentinels of the immune system. A mechanism for nucleocapsid core release and disassembly upon viral entry was inferred based on pH changes and capsid …

Operating room (OR) turnover cleaning is an important part of perioperative workflow for surgical teams, particularly to prevent cross-contamination of infectious pathogens from the surgical environment to staff and patients. Incomplete OR turnover cleaning ultimately leaves vulnerable surgical patients at risk for surgical-site infections (SSIs) and other types of hospital acquired infections (HAIs), which leads to delays in overall operations of the OR. A microsystem assessment at a large metropolitan hospital revealed an issue with incomplete OR turnover cleaning evident by the presence of dirty canisters, filters, and unclean anesthesia machines/work-areas after sterile surgical supplies had already been opened in …

This session will share lessons learned from the four-year implementation of a pain-care pathway across 45 patient-centered medical home (PCMH) clinics in Oregon. Leveraging the PCMH team, communication, and data to improve pain management and reduce risk, Providence Medical Group was able to realize a 68% reduction of patients on high-dose opioids.
