Objective: To evaluate the clinical characteristics and predictors of in-hospital mortality in patients with infective endocarditis at a tertiary care centre in Pakistan.
Methods: The cross-sectional study was conducted at the Aga Khan University Hospital, Karachi, from January 1, 2002, to December 31, 2006, and comprised 84 consecutive patients hospitalised with infective endocarditis,. All patients underwent verification of the diagnosis according to the Modified Duke criterion. Data was collected through a questionnaire and analysed using Student's t test, Chi square and Fisher's exact test. Univariate and multivariate logistic regression analysis was performed to assess predictors of mortality.

Introduction: Ectopic thyroid tissue is the most common form of thyroid dysgenesis. But primary malignant transformation in ectopic thyroid tissue is quite a rare entity, with follicular malignancy being the dominant form at ectopic sites. Very infrequently, malignant ectopic thyroid tissue can present with metastasis to lymph nodes. But we report a case of malignant ectopic thyroid tissue over manubrium sterni with distant metastasis.

Hemorrhagic manifestations are fairly common in Dengue hemorrhagic fever and are associated with increased mortality. During last few decades there have been increasing reports of Dengue infection with unusual manifestations. Here we present a case of dengue hemorrhagic fever complicated by spontaneous rupture of an intercostal artery leading to a large hematoma which was treated successfully with angio-embolization. To the authors' knowledge this is a first case of dengue hemorrhagic fever complicated by spontaneous intercostal artery hemorrhage.

Background: Stroke is a leading cause of morbidity and mortality worldwide. There is a paucity of data from South Asia where stroke is highly prevalent. Validated tools administrable by Community Health Workers (CHWs) are required to identify stroke in the community in a resource strapped region such as this.
Methods: The study was conducted in a transitional slum in Karachi, Pakistan. Questionnaire for Verifying Stroke-Free Status (QVSFS) was adapted and translated into Urdu. Research has shown that forgiveness is a complex process and is vital to one’s well-being (Coyle & Enright, 1997; Enright, 1991; Freedman, 2007). There have been several research studies conducted with various populations (Freedman & Enright, 1996; Coyle & Enright, 1997; Gambaro, Enright, Baskin, & Klatt, 2008; Waltman et al., 2009) who have experienced deep, personal, and unfair hurts. Forgiveness has been found to be healing and beneficial to one’s overall psychological well-being in these studies. The 1928 language revolution in Turkey involved the change of the Turkish writing script from the Arabic to the Latin alphabet, as well as the elimination of Arabic and Persian loan words and associated imported grammar. In this study, we assessed the impact this language revolution had on Turkish identity by examining it through the lens of Ottoman and Turkish literature and poetry. Background: Prolonged, severe, unalleviated fatigue may be disabling whether it occurs on its own or in conjunction with medical or psychiatric conditions. This paper compares the prevalence and correlates of prolonged fatigue in general population samples in Ukraine versus the US. Methods: Population surveys were conducted in 2002 in both Ukraine (Ukraine World Mental Health [WMH] Survey) and the US (National Comorbidity Survey-Replication; NCS-R). Both surveys administered the Composite International Diagnostic Interview (CIDI 3.0), which contained modules assessing: neurasthenia (prolonged fatigue); mood, anxiety, and alcohol/drug use disorders; chronic medical conditions; and demographic characteristics. In this essay, I explore the place of a genealogy of morals within the context of a history of political economy. More specifically, I investigate the types of moralization – of criminals and delinquents, of the disorderly, but also of political economic systems, of workers and managers, of rules and rule-breaking – that are necessary and integral to making a population accept new styles of political and economic governance, especially the punitive institutions that accompany modern political economies in the contemporary period.

Humans are remarkably complex and unpredictable; however, while predicting human behavior can be problematic, there are methods such as modeling and simulation that can be used to predict probable futures of human decisions. The present study analyzes the possibility of replacing human subjects with data resulting from pure models. Decisions made by college students in a multi-level mystery-solving game under 3 different gaming conditions are compared with the data collected from a predictive sequential Markov-Decision Process model. This case study outlines an ethical dilemma faced by a young female student who is planning to buy airline tickets. Her purchase decision is influenced by cost and advertising strategies. The case discusses advertising ethics, ethical moral philosophies, including teleology and deontology, and sexist advertising with examples from the airline industry. This case will be beneficial for marketing students to discuss the topic of advertising ethics, and for business students to discuss the topic of organizational ethics. The accident rate for general aviation remains high. While most general aviation accident studies have been pilot-focused, there is little research on the involvement of aircraft maintenance errors. We undertook a study to answer this question.
The Microsoft Access database was queried for accidents occurring between 1989 and 2013 involving single engine piston airplanes operating under 14CFR Part 91. Pearson Chi-Square, Fisher’s Exact Test, and Poisson probability were used in statistical analyses.
The rate of maintenance-related general aviation accidents was 4.3 per million flight hours for the 1989–1993 period and remained unchanged for the most recent period (2009–2013). Maintenance errors …

"After deregulation in the United States in 1978, airlines faced intense competition on previously regulated routes. The proponents of deregulation stated that equilibrium in the industry would be achieved by providing lower fares and improved service (Daraban and Fournier, 2008). While this became true to some extent, the airline network in the U.S. was dominated by the hub-and-spoke system and concentrated in the hands of few large airlines. Strategies for improving student success are particularly important in high-demand fields like science, technology, engineering, and math (STEM). As the U.S. experiences a decline in skilled STEM workers and a growing number of racial/ethnic minorities, it is critical that more attention be paid to the college success of underrepresented populations majoring in STEM. Non-traditional students, particularly military and veteran students, pose challenges for universities to serve efficiently and well. For the purpose of this discussion, the non-traditional student is defined as one with a delayed entry to college after high school completion, who may bring time-management conflicts due to work and family commitments, and who may have a variety of transcripted and non-transcripted credits to consider for transfer or advanced standing.

Thermal imaging is a well-established technique for the non-destructive evaluation of FRP composites applied to reinforced concrete. Defect characterization using IR thermography, however, remains a topic of on-going research, and there are currently no universally accepted standards for data collection or interpretation. This research involved large scale thermography inspection of two FRP strengthened bridge girders that were removed from service after approximately 10 years of service in a potentially corrosive marine environment. Trial inspections were performed on test areas where defects could be identified using sounding methods. The environmental friendliness of short sea shipping has been justified in Europe by the ensuing lower congestion at hinterlands and unneeded large-scale infrastructure investments on roads and railways. However, the attractiveness of short sea shipping is about to change. By using meteor radar, radiosonde and satellite observations over 20° N and NCEP/NCAR reanalysis data during 81 days from 22 December 2004 to 12 March 2005, a quasi-27-day oscillation propagating from the troposphere to the mesosphere is reported. A pronounced 27-day periodicity is observed in the raw zonal wind from meteor radar. Spectral analysis shows that the oscillation also occurs in the meridional wind and temperature and propagates westward with wavenumber s = 1; thus the oscillation is of Rossby wave type.
