Medication Expenditure And Resource Utilization Among Patients With Musculoskeletal Disorders: Analysis Of 2007 Medical Expenditure Panel Survey Data, Nipun Atreja

Objective: To estimate the national prevalence and direct incremental expenditures of musculoskeletal disorders (MSD's) using the 2007 Medical Expenditure Panel Survey data.
Methods: A retrospective database analysis was conducted and individuals with MSD's (ICD-9-CM codes 274.00; 710.00-738.00) were identified. Dependent variables were total health care and other service category expenditures. The study utilized descriptive and regression analyses.
Results: In 2007, the national prevalence of MSD's was 33 million with incremental costs of $886.49 per person. The inpatient expenditures ($33,461.85) were the highest cost component in MSD's and the predictors of total health care expenditures were age, marital status, and presence …

Chromium has been used extensively in the industry process of metal refinishing and electroplating. It is also a byproduct of the processing of fissionable materials at United States Department of Energy facilities. Chromate (Cr (VI)) is soluble and readily absorbed by cells, while the reduced form of chromium, Cr (III), is insoluble. Thus means of reducing Cr (VI) to Cr (III) in the environment is a potential means of remediation. Desulfovibrio desulfuricans strain 27774 is a sulfate reducing bacterium that can reduce Cr (VI). The toxic dinoflagellate, Karenia brevis, forms nearly annual blooms in the Gulf of Mexico that persist for many months in coastal waters, causing extensive marine animal mortalities and human health impacts. The molecular mechanisms that contribute to cell survival in high density, low growth blooms, and the mechanisms leading to often rapid bloom demise are not well understood. The studies presented in this dissertation investigate the existence and involvement of a programmed cell death-like (PCD-like) pathway in the demise of K. brevis cultures following oxidative stress and chronological aging. Reversed phase liquid chromatography with atmospheric pressure chemical ionization mass spectrometry (RP-HPLC/APCI-MS) was employed for the analysis of natural mixtures of triacylglycerols. An integrated framework for data analysis, including preprocessing, statistical analysis and automated structure identification, was implemented in the R statistical program. Raw data stored as mzXML, mzData, or mzXML files are preprocessed using a series of steps for peak detection, chromatographic alignment, and normalization. Targeted and non-targeted feature selection steps are employed to filter the data for features that are relevant and informative for a particular biological question. This study uses Medicare claims from 1997 and 2004 to examine two important health policy issues using patients with ischemic stroke as an example. We explore the differences in rehabilitation services utilization and expenditures for stroke prior to and after outpatient rehabilitation caps were enacted. We then examine the effect of using a total cost approach versus a marginal cost method to estimate the first year cost of stroke and stroke rehabilitation. We found the cap did constrain outpatient rehabilitation utilization and cost of rehabilitation services if examined only using Medicare Part B provider costs, but not when facility costs …

Cubilin is an endocytic receptor capable of mediating the endocytosis of multiple ligands including High Density Lipoprotein apolipoprotein A-I (apoA-1) and albumin. It is highly expressed on the epithelium of intestinal villi and renal proximal tubules. In the kidney, endocytosis mediated by cubilin and its co-receptor megalin is the principal mechanism for proximal tubule reabsorption of filtered proteins, such as albumin and apoA-1. Yet, the consequences of cubilin deficiency on the blood levels of albumin and apoA-1 have not been studied. Furthermore, cubilin is downregulated in chronic diseases such as diabetes. Granulosa cell apoptosis is associated with follicular atresia; but the cellular mechanisms that drive this process, especially its cell specificity, are relatively unknown. Here, we determined that cultured granulosa cells abundantly express K8/K18 filaments and inhibition of protein synthesis enhances Fas-induced apoptosis. In this context, the roles of cFLIP, ERK1/2 and Akt are minimal but conversely, K8/K18 filaments have a prominent role in granulosa cell resistance to Fas-induced apoptosis. Keratin 8/18 filaments in granulosa cells provide a plausible mechanism to avoid Fas-induced apoptosis and this mechanism potentially involves the synthesis of labile proteins. People participating in family mealtimes ≥ 6 times per week while growing up are known to have a better quality diet, GPA, sleeping habits, and a normal BMI. It is hypothesized that college students with a history of family mealtimes continue to experience these benefits while in college. The purpose of this study is to investigate the impact of the history of family mealtime on diet quality, GPA, sleeping habits, and BMI in a college setting.

Music therapy clinical practice occurs at various levels. Wheeler (1983) has classified the treatment of adults with mental disorders into three types: music therapy as an activity therapy; insight music therapy with re-educative goals; and insight music therapy with reconstructive goals.
Activity-based therapy is aimed at helping the client reach observable, measurable goals through various forms of music experiences. In contrast, the two remaining levels focus on facilitation of change through personal insight gained via musical experiences and verbalization about those experiences. Singing has long been credited with a wide variety of physical, mental, and social health benefits (Hunter, 1999). Recent scientific inquiry points to the efficacy of singing toward enhanced cardiovascular and pulmonary performance (Bonilha, Onofre, Vieira, Prado, & Martinex, 2009), verbal communication (Wan, Ruber, Hohmann, & Schlaug, 2010), and immune functioning and attendant affective states (Kreutz, Bongard, Rohrmann, Hodapp, & Grebe, 2004; Kuhn, 2002; Unwin, Kenny, & Davis, 2002).

In Drosophila melanogaster, mutations in the gene drop-dead (drd) result in early adult lethality, with flies dying within 2 weeks of eclosion. Additional phenotypes include neurodegeneration, tracheal defects, starvation, reduced body mass, and female sterility. The cause of early lethality and the function of the drd protein remain unknown. In the current study, the temporal profiles of drd expression required for adult survival and body mass regulation were investigated. Knockdown of drd expression by UAS-RNAi transgenes and rescue of drd expression on a drd mutant background by a UAS-drd transgene were controlled with the Heat Shock Protein …

In South Carolina (SC), children aged 10-17 are ranked 13th in the United States for overweight and obesity (33.7%) according to the National Survey of Children's Health 2007. Fruit and vegetable intake in children and adolescents in the United States is below recommended levels (Lorson, Melgar-Quinonez, & Taylor, 2009). In SC, only 15% of the high school students consume fruits and vegetables five or more times per day. Walnuts contain a number of potentially neuroprotective compounds like vitamin E, folate, melatonin, several antioxidative polyphenols and significant amounts of n-3 α-linolenic fatty acid. The present study sought to determine the effect of walnuts on verbal and non-verbal reasoning, memory and mood. A total of sixty-four college students were randomly assigned to two treatment sequences in a crossover fashion: walnuts-placebo or placebo-walnuts. Baseline data were collected for non-verbal reasoning, verbal reasoning, memory and mood states. Data were collected again after 8 weeks of intervention. The nutrient uptake and allocation of cucumber (Cucumis sativus L.) seedlings at different root-zone temperatures (RZT) and different concentrations of nitrogen (N), phosphorus (P), and potassium (K) nutrients were examined. Plants were grown in a nutrient solution for 30 d at two root-zone temperatures (a diurnally fluctuating ambient 10°C-RZT and a constant 20° C-RZT) with the aerial parts of the plants maintained at ambient temperature (10°C -30°C). Child welfare cases involving mental illness suffered either by a child or his parent can be among the most difficult and perplexing that a child’s lawyerguardian ad litem (L-GAL) will handle. They may present daunting problems of accessing necessary and appropriate services as well as questions about whether and when such mental health problems can be resolved or how best to manage them. In the past decade, the existence of shaken baby syndrome (SBS) has been called into serious question by biomechanical studies, the medical and legal literature, and the media. As a result of these questions, SBS has been renamed abusive head trauma (AHT). This is, however, primarily a terminological shift: like SBS, AHT refers to the two-part hypothesis that one can reliably diagnose shaking or abuse from three internal findings (subdural hemorrhage, retinal hemorrhage, and encephalopathy) and that one can identify the perpetrator based on the onset of symptoms. There seems to be an ongoing controversy among researchers with regard to the existence of LD in foreign language (FLLD). It appears that most researchers favor the analogy of FLLD to obesity rather than to measles as both LD and obesity are conditions which reside on a continuum. The purpose of this paper is to evaluate and review current best practices with regards to a minor’s refusal of life-sustaining medical treatment. The policies of other developed countries and states within the United States serve as a guide for developing evidence-based guidelines for Ohio healthcare professionals. Four specific cases were identified and then analyzed. The results showed that in three of the four cases, the wishes of the minor were followed. While each case contains its own set of complexities, creating a developmental guideline for healthcare organizations will ensure protection of the minor’s capacity to make autonomous decisions.

Introduction: This study aimed to understand the chronic disease burden of anemia, cancer, diabetes mellitus, hypertension, overweight/obesity, stroke, and tobacco use in the domestic as well as a refugee population. It also aimed to examine the current literature on appropriate screening for those conditions.
Methods: 348 refugee screening intake exams from Public Health - Dayton & Montgomery County were analyzed for presence of the conditions stated.
