Percutaneous gastrostomy (PEG) tube placement in patients with continuous flow left ventricular assist device. (LVAD). Introduction: Inadequate nutritional support after LVAD placement is known to increase postoperative infections and to decrease survival. The LVAD patients with complicated postoperative recovery requiring prolonged mechanical ventilation may require long-term tube feedings. Application of the miniaturized ImaCor Trans-Esophageal Echocardiogram (TEE) probe in Heart Transplant/Mechanical Cardiac Support Patients In the surgical cardiac care unit (SCCU), therapeutic interventions often need to be done at the bedside, necessitating the need for a rapidly employable diagnostic tool for the cardiac intensivist. We report the clinical utility of the miniature ImaCor TEE-probe in guiding management of post heart transplant (H-Txp) and mechanical cardiac support patients (MCS) and describe the economic benefit of such a device. An old problem with a new therapy: GI Bleeding in VAD patients and deep bowel enteroscopy (Spiral and Double Balloon Enteroscopy) Purpose: Evidence suggests that patients treated with non-pulsatile ventricular assist devices (VAD) are at an increased risk for gastrointestinal bleeding (GIB) beyond what is expected from routine anticoagulation. Diagnostic and treatment algorithms are currently undefined. We reviewed our experience of GIB in VAD patients and propose a new algorithm utilizing deep bowel enteroscopy (DBE) aimed to speed diagnosis and limit transfusions. In 1985, a number of goals and research questions were proposed in relation to the archaeology of' pre-1664 sites in the Dutch colony of New Netherland. Significant Dutch sites were subsequently ~xcavated in Albany, Kingston, and other places from 1986 through 1988, while a series of useful publications continued to be produced after 1988. Excavations at historic period Indian sites also continued after 1988 . . The Castello Plan-Evidence Of Horticulture In New Netherland Or Cartographer's Whimsy?, Richard Schaefer, Meta Fayden Janowitz

Few descriptions or depictions of horticulture in New Netherland have come down to us, although 17th-century observers' accounts of gardens and orchards present lengthy lists of fruits, vegetables, 'and fiowers transplanted from Europe, as well as those discovered in North America. Perhaps the most evocative source is the mid-century Castello Plan, a view of the settlement af New Amsterdam, which shows elaborate · parterres on most of the unoccupied lots. The Van Lidth De Jeude Family And The Waste From Their Privy: Material Culture Of A Wealthy Family In 18th-Century Tiel, The Netherlands, Michiel H. Bartels

This study uses the comprehensive material culture of the 18th-century privy of the regent family Van Lidth de Jeude in Tiel (the Netherlands). The analysis takes advantage of the secondary sources that are common in the Netherlands to evaluate the privy contents. The archaeological finds are interpreted with the aid of historical documents, contemporary literature and probate inventories, and the content of the privy is set against the background of an 18th-century provincial town in the heart of the Netherlands. “What’Ll Thou Have”: Quakers And The Characterization Of Tavern Sites In Colonial Philadelphia, John M. Chenoweth

Northeast Historical Archaeology

In 1766, Ebenezer Robinson, an active Quaker and middle-class tradesman, was one of the first to develop the land just north of Independence Hall, then at the edge of urban Philadelphia. Recent work on Independence Mall sponsored jointly by the National Park Service and the National Constitution Center has uncovered several features on this property, but analysis and historical documentation has suggested that a low-class tavern occupied the spot while Robinson owned it. Just as the firm has long served as the foundational molecule of the U.S. capitalist economy, theories of the firm have for more than a century dominated legal and economic discourse. Ever since Ronald Coase published The Nature of the Firm in 1937 and asked why firms should exist in an efficient market, classicists and neoclassicists have competed to develop theories — predominantly managerialist and contractual — that best explain the structure and behavior of business organizations.

A companion piece to Rulemaking vs. Democracy: Judging and Nudging Public Participation that Counts, this Essay continues to examine the nature and value of broader public participation in rulemaking. Here, we argue that rulemaking is a “community of practice,” with distinctive forms of argumentation and methods of reasoning that both reflect and embody craft knowledge. Rulemaking newcomers are outside this community of practice: Even when they are reasonably informed about the legal and policy aspects of the agency’s proposal, their participation differs in kind and form from that of sophisticated commenters. Background: Although cigarette smoking is a leading cause of death and disability in the United States (US), over 40 million adults in the US currently smoke. Quitting smoking is particularly difficult for smokers with certain types of psychological vulnerability. Researchers have frequently called attention to the relation between smoking and anxiety-related states and disorders, and evidence suggests that panic and related anxiety vulnerability factors, specifically anxiety sensitivity (AS or fear of somatic arousal), negatively impact cessation. Accordingly, there is merit to targeting AS among smokers to improve cessation outcome. Aerobic exercise has emerged as a promising aid for smoking …

Background—Several lines of evidence support a pathophysiological role of immunity in atherosclerosis. Tyrosine-nitrated proteins, a footprint of oxygen- and nitrogen-derived oxidants generated by cells of the immune system, are enriched in atheromatous lesions and in circulation of patients with coronary artery disease (CAD). However, the consequences of possible immune reactions triggered by the presence of nitrated proteins in subjects with clinically documented atherosclerosis have not been explored. Methods and Results—Specific immunoglobulins that recognize 3-nitrotyrosine epitopes were identified in human lesions, as well as in circulation of patients with CAD.
