The blood–brain barrier is a restrictive interface between the brain parenchyma and the intravascular compartment. Tight junctions contribute to the integrity of the blood–brain barrier. Hypoxic–ischemic damage to the blood–brain barrier could be an important component of fetal brain injury. We hypothesized that increases in blood–brain barrier permeability after ischemia depend upon the duration of reperfusion and that decreases in tight junction proteins are associated with the ischemia-related impairment in blood–brain barrier function in the fetus. Purpose: Response Elaboration Training (RET) is a treatment program for increasing the amount of information contained in the speech of individuals with aphasia. This study employed a sentence production task (SPT) to examine whether RET will (1) generalize to a task not worked on in treatment and (2) affect syntactic performance.
Methods and Procedures: Three adults with chronic non-fluent Broca’s aphasia received RET in this prospective, repeated case study. The dependent variable was a SPT requiring the participant to put a verb in a sentence. Changes in the syntactic performance of the participant’s sentence productions were classified across …

Nursing aides at long-term care institutions need to stand and walk all day. As a result, the average burden of lower limbs for nursing aides is greater than nurses was. Feet have to bear the bodies weight as well as for support, shock absorption, and wear and tear from walking. This study investigated major factors of foot pain as caused by functional activities or the abnormal foot structures from prolonged standing and walking due to work. Hemophilia is an Ⅹ-linked congenital bleeding disorder, with deficiency of coagulation factor Ⅷ (hemophilia A) or factor Ⅸ (hemophilia B). Hemophilia patients often have hemarthrosis with severe joint pain, which results in joint immobility and subsequent muscle atrophy, joint contracture, and gait disturbance. This study investigated the symptoms and severity, joint involvement, and imaging findings of hemophilia patients in Taiwan.A total of 51 male participants, including 40 with hemophilia A, aged from 6 to 69 years (mean age: 27.2±15.6 years), were recruited. Previous studies have shown beneficial effects of modified constraint-induced movement therapy (mCIMT) in improving motor functions of the upper extremities in children with spastic cerebral palsy (CP). However, there are few studies which have used kinematic analysis to validate the effectiveness and mechanisms underlying motor improvement after mCIMT. Few victims can survive high-voltage electric burns in which the electricity passes through the scalp. Notably, 0.7% to 8.0% of these survivors will progressively develop cataracts caused by the electric injury. This report presents a unique case manifested with rapid visual deterioration as a result of posterior subcapsular cataract. A 36-year-old male was incidentally injured by an electric burn of 11,000 volts during work. Immediately following the injury, his vision remained sufficient in daily activities. Unfortunately, progressive non-painful visual loss developed in his right eye within a month during rehabilitation training and then later in the left eye. Ophthalmologist confirmed …

Peripheral neuropathies after cardiac surgery are usually attributed to manipulation during the surgical procedure itself. A 54-year-old man developed left ulnar, median, and radial neuropathies after off-pump coronary artery bypass surgery. An electrodiagnostic study indicated a lesion located in the distal part of the upper arm. In view of the anatomy, this patient most likely developed neuropathy associated with intraoperative blood pressure monitoring. Iatrogenic peripheral neuropathy after cardiac surgery should be carefully assessed to find the exact cause.

Critical illness polyneuropathy and myopathy (CIPNM) is a common complication seen in patients with critical illness. However, long-lasting oropharyngeal dysphagia in patients with CIPNM has not been reported previously. Besides, severe dysphagia is also a rare condition in patients with diabetes. Here, we present the case of a woman with diabetes, who developed neuromuscular weakness after recovery from severe pneumonia and respiratory failure. Brain lesion was excluded by magnetic resonance imaging of brain. CIPNM was confirmed by nerve conduction and electromyographic studies. Videofluoroscopic swallow study showed severe oropharyngeal dysphagia. The general weakness and functional status improved gradually after rehabilitation. However, …

Other than craniofacial anomaly, children with cleft lip and palate usually have problems of speech and swallowing. Therefore, in addition to multiple sessions of operations for repair and reconstruction, active rehabilitation is also required to improve their function of speech and swallowing. This report disclosed a ten-year-old boy who depended on nasogastric tube feeding and respired via tracheotomy after birth because of bilateral cleft lip and palate as well as subglottic stenosis. We studied rural-urban differences in medical spending trends over eleven years for VA as well as non-VA care received by male veterans who used any VA services, and compared those trends to trends for other healthcare-using men. Using inflation-adjusted annual medical expenditures for non-veterans, VA users, and other veterans who participated in Medical Expenditure Panel Surveys from 1996 through 2006, we examined trends in spending on inpatient, hospital-based outpatient, office-based, pharmacy, and other care, by major payers (self/family, private insurance, Medicare, other sources, and VA), to assess changes in expenditures for the care of rural veterans, younger or older than …

Tulisan ini memperkenalkan metode fuzzy c-means dalam mengklasifikasi konstruk tes. Untuk memverifikasi sifat unidimensional suatu tes biasanya menggunakan analisis faktor sebagai bagian dari statistik parametrik dengan beberapa persyaratan yang ketat sedangkan metode fuzzy c-means termasuk metode heuristik yang tidak memerlukan persyaratan yang ketat. Studi simulasi penelitian ini menggunakan dua metode yakni analisis faktor menggunakan program SPSS dan fuzzy c-means menggunakan program Matlab. Latino combat soldiers report both higher prevalence and greater overall severity of post-traumatic stress disorder (PTSD) symptoms than non-Hispanic Caucasians. However, these veterans face unique social and cultural barriers to accessing treatment for PTSD that distinguish them from their non-Hispanic white counterparts. Latino veterans who reside in rural settings face additional socio-cultural and structural impediments, in that they are likely to reside far from VA (Veterans Administration) medical facilities, have limited access to public transportation, and hold more conservative views toward mental health treatment than those residing in urban locales. Native Veterans comprise unique populations within the VeteransAdministration (VA) system of care and represent a proud tradition of military service. Limited healthcare data available on rural Native veterans indicate significant disparities in access to care and health status compared with other populations. This article provides an assessment of current challenges, barriers, and issues related to addressing the healthcare needs of rural Native veterans and offers recommendations to improve healthcare for this special population. Women veterans seeking employment in rural areas often face several challenges, such as geographical barriers, limited employment opportunities, and a lack of childcare resources within their respective communities. This exploratory study builds on the 2001 report by the Women’s Research & Education Institute (WREI), which outlined the effects of military service on women veterans’ civilian employment prospects. In addition, it explores the specific challenges through a review of current literature, assessment of Bureau of Labor Statistics datasets, and the conduct and analysis of qualitative interviews. Little information exists on the health-related quality of life (HRQL) of U.S. veterans based on rural (versus urban) status, especially those in younger age groups, and whether deployment influences this outcome. We addressed these questions in the Millennium Cohort Study, a prospective investigation of U.S. military personnel assessed first in 2001 and then subsequently every three years via self-administered questionnaires. Participants separated from the military at the time of the most recent survey were eligible (n = 10,738). HRQL was assessed using the SF-36V Physical Component Summary (PCS) and Mental Component Summary (MCS) scores. Rural status was assigned from zip …

National Guard and Reserve (RC) troops (N=617) primarily from the Appalachian Region in Southwestern Pennsylvania who recently returned from deployment in support of current military conflicts responded to a survey that assessed their demographics, mental health symptoms, help-seeking behaviors, barriers for not seeking treatment, deployment history, and stressors. Veterans were classified as rural (N = 334) or non-rural (N = 283). Rural participants reported a significantly greater number of issues with transportation/access in seeking mental health treatment, were more likely to perceive others as worse off as a reason not to seek treatment, had a more negative attitude …

Findings are presented from two focus group discussions with rural non-VA (Veterans Administration) primary care providers to better understand their experience with treating dual care veterans, those who receive care from both VA and non-VA providers. Participants reported challenges related to a lack of communication and coordination between the VA and non-VA providers. Participants agreed that improvements must be made to the current healthcare delivery model for rural dual care veterans to support seamless care. Penelitian ini bertujuan untuk mengetahui kondisi dan fenomena sosial budaya secara empirik. Metode yang digunakan adalah survai data dikumpulkan melalui observasi, wawancara, dan Fokus Group Discussion (FGD) sumber data adalah pejabat pemerintah, tokoh adat, tokoh agama, tokoh masyarakat, dan para pemuda di Provinsi NTT. Hasil penelitian menunjukkan bahwa gatra sosial budaya dapat memperkuat ketahanan nasional masyarakat NTT untuk berpacu bersama daerah lainnya di Indonesia.
