The purpose of this study is to investigate the impact of the reformation of the In Home Support Services (IHSS) program. This study is significant because the IHSS program keeps many elderly citizens out of assisted living facilities. Surveys were sent to the San Bernardino County IHSS social workers to determine how the program reformation has affected their clients.

Does Medicare's payment rate for a hospital stay influence the quality of care received by a patient? We examine this question, theoretically and empirically. First, a model is developed which generates the key hypothesis -- that Medicare's payment rate per admission should be positively related to care quality. We then test this hypothesis by estimating the relationship between Medicare's DRG payment for pneumonia, heart failure and heart attacks and care quality, using clinically-recognized measures of the quality of pneumonia care, heart failure care and heart attack care. The purpose of this translational research article is to illustrate how general practice occupational therapists have the skills and knowledge to address driving as a valued occupation using an algorithm based on the Occupational Therapy Practice Framework: Domain and Process (2nd ed.; American Occupational Therapy Association, 2008b). Evidence to support the model is offered by a research study. Participants were compared on their performance of complex instrumental activities of daily living (IADLs) and a behind-thewheel driving assessment. Memory problems are a hallmark symptom of dementia. Although memory problems can take various forms, anomia is a common type of cognitive deficit that involves difficulty recalling names of people or objects. The purpose of the current study was to test the effectiveness of two interventions designed to improve the ability to recall the names of objects. Two elderly individuals with memory impairment participated in this study. Baseline involved identifying low and high probability images, with low-probability (low-p) items serving as the target images during the intervention phases. Objective: To determine patient-, treatment-, and facilitylevel characteristics associated with receiving outpatient rehabilitation services after lower extremity amputation within the Veterans Affairs (VA) system.
Design: Observational study.
Setting: All Veterans Affairs Medical Centers (VAMCs).
Participants: Veterans (N=4165) with lower extremity amputation discharged from VAMCs between October 1, 2002, and September 20, 2004. Interventions: Not applicable.
Main Outcome Measures: Receipt of outpatient rehabilitation services up to 1 year postdischarge. A Cox proportional hazards model was used to determine the adjusted hazard ratio and 95% confidence interval of veterans to receive outpatient services.

Owl clutches typically hatch asynchronously, and brood size hierarchies develop. In this study, we describe intra-clutch variation of testosterone, androstenedione, estradiol, and corticosterone in Eastern screech owl egg yolks. In order to assess whether these hormones may have originated in the follicle, we also characterize variation of testosterone, androstenedione, and corticosterone within the exterior, intermediate, and interior regions of the yolk. Concentrations of testosterone and androstenedione were distributed relatively evenly across egg lay order with the exception of first-laid eggs that had significantly lower concentrations of both androgens than eggs later in the laying sequence. Neural Activity, Neural Connectivity, And The Processing Of Emotionally-Valenced Information In Older Adults: Links With Life Satisfaction, Robert J. Waldinger, Elizabeth A. Kensinger, Marc S. Schulz

This study examines whether differences in late-life well-being are linked to how older adults encode emotionally-valenced information. Using fMRI with 39 older adults varying in life satisfaction, we examined how viewing positive and negative images affected activation and connectivity of an emotion-processing network. Participants engaged most regions within this network more robustly for positive than for negative images, but within the PFC this effect was moderated by life satisfaction, with individuals higher in satisfaction showing lower levels of activity during the processing of positive images. Metamemory is an important skill that allows humans to monitor their own memory abilities; however, little research has concerned what perceptual information influences metamemory judgments. A series of experiments assessed the accuracy of metamemory judgments for music as well as determined if metamemory judgments are affected by ease of processing of musical features. A recognition memory task inconjunction with metamemory judgments (Judgments of Learning, or JOLs) were used to determine actual and predicted memory performance. We found that changing the ease of processing of the volume and timbre of unfamiliar tunes affected metamemory judgments, but not memory performance, for unfamiliar …

The effect of a distraction-based intervention on positive affect and aggressive and distress-related behaviors during morning activities of daily living in an elderly dementia patient was tested. Concurrently, the effectiveness of distractors chosen by staff and family report as compared to those chosen through a stimulus preference assessment (SPA) was tested. An alternating treatment design was used to implement identified distractors and a direct observation system was used to measure outcomes. Paramount to multicultural social work practice is acquiring a comprehensive understanding of oppressed and vulnerable populations who often lack access to healthcare and economic resources. A practitioner’s self-awareness of social attitudes can provide a foundation for developing cultural competency skills. Using a pretest posttest design, social attitudes of Master of Social Work (MSW) students enrolled in a required social justice course were examined (N=85). A repeated-measure MANOVA indicated significant main effects on: 1) within subject factor of time on students’ cognitive attitudes towards racial diversity, affective attitudes toward racial diversity, and women equity following students’ exposure to the social justice …

Community-campus engagement has evolved significantly over the past quarter century, shaped by a number of factors. One has been the effort to reclaim the civic mission of American higher education. Frank Newman, while at the Carnegie Foundation for the Advancement of Teaching in the early 1980s, asserted that "the most critical demand is to restore to higher education its original purpose of preparing graduates for a life of involved and committed citizenship,” and concluded that “the advancement of civic learning, therefore, must become higher education's most central goal" (1985, xiv). The purpose of this study was to contribute to women’s healthy aging across the adult lifespan by empirically examining potential protective factors (e.g., empowerment and feminist attitudes) in maintaining positive body image, healthy eating behavior, and aging satisfaction. Objectification Theory (Fredrickson & Roberts, 1997) provided a theoretical framework for understanding the connections between sexual-objectification experiences, media influences, and self-objectification, and the resulting negative psychological consequences for women in Western society. This study was the first to examine empowerment in relation to Objectification Theory. Inquiry learning is a pedagogical approach that focuses on the processes and skills required to conduct research. It is a pedagogical approach that has been demonstrated to have positive learning outcomes. McMaster University has been committed to this form of learning for more than ten years in three of the faculties on campus (i.e., Humanities, Science and Social Sciences). This commitment has been in the creation of stand-alone, small class size first year inquiry classes. Recently developed models of health care provision have promoted the relationship between providers and patient for improving care continuity and coordination. This may be especially important for the growing population of elders, who often have fragmented care because of multiple chronic illnesses. Previous research shows that elders who have a usual source of care (USC) have better health and health care experiences than other elders. However, research has rarely considered whether the benefits of USCs may be affected by the elder's socioeconomic status (SES), their levels of chronic illness, or various provider characteristics.

Acute Impact Of Moderate-Intensity And Vigorous-Intensity Exercise Bouts On Daily Physical Activity Energy Expenditure In Postmenopausal Women, Xuewen Wang, Barbara J. Nicklas

This study determined whether performing a single moderate- or vigorous-intensity exercise bout impacts daily physical activity energy expenditure (PAEE, by accelerometer). Overweight/obese postmenopausal women underwent a 5-month caloric restriction and moderate- (n = 18) or vigorous-intensity (n = 18) center-based aerobic exercise intervention. During the last month of intervention, in women performing moderate-intensity exercise, PAEE on days with exercise (577.7 ± 219.7 kcal·d−1) was higher (P = .011) than on days without exercise (450.7 ± 140.5 kcal·d−1); however, the difference (127.0 ± 188.1 kcal·d−1) was much lower than the energy expended …

Purpose: The aim of this study was to examine whether older people are prepared to engage in appropriate falls prevention strategies after discharge from hospital.
Design and Methods: We used a semi-structured interview to survey older patients about to be discharged from hospital and examined their knowledge regarding falls prevention strategies to utilize in the post-discharge period. The study was part of a prospective cohort study, nested within a larger, randomized controlled trial.
