Computed Tomography Perfusion (CTP) is an imaging modality that generates parametric maps of cerebral hemodynamics which are useful in the assessment of suspected acute ischemic stoke. However, the technology underlying CTP is complex and serious controversy surrounds the safety of CTP tests and the reproducibility and validity of CTP results. This report briefly outlines the history of CTP, its current clinical applications for stroke management, the main controversies surrounding CTP, and future directions for this technology.

Dural arteriovenous malformations (DAVMs), also known as dural arteriovenous fistulas, are arteriovenous shunts from a dural arterial supply to a dural venous channel, typically supplied by pachymeningeal arteries and located near a major venous sinus.1 The etiology of these lesions is not fully understood. DAVMs in the pediatric population are associated with structural venous abnormalities ,2 but the majority of DAVMs are thought to be acquired. Different etiologies have been implicated in this phenomenon, namely: sinus thrombosis, trauma or surgery.2–6

A 59 year-old female presented with visual problems in 2006, and was initially diagnosed with a pituitary macroadenoma on MRI (Figure 1). Preoperatively, the pituitary lesion decreased in size after the patient was started on steroids, raising the possibility of lymphoma and inflammatory conditions. Ultimately, endoscopic trans-nasal biopsy of the pituitary lesion revealed lymphocytic hypophysitis. She was treated with steroids for two years and developed several side effects from chronic steroid use, including Cushingoid features, diabetes, cataracts, weight-gain and osteopenia. As a result, she was subsequently treated with methotrexate and gradual weaning of the steroids.

Infection of the spine may occur spontaneously, by direct hematogenous or contiguous spread from an area of infection, or secondarily to an inciting event such as trauma or spine operation. The overall rate of spine surgery has increased steadily over the last decade. Medicare spending on inpatient spine surgery has doubled over the same period with lumbar fusion representing the greatest percentage of spending.1 Surgical site infection (SSI) after spinal surgery is not uncommon, leading to increased morbidity and mortality. Fever is an adaptive response to a variety of infectious, inflammatory, and foreign stimuli. The “febrile response” confers an immunological advantage to the host over invading microorganisms – bacterial, fungal and viral. Fever results from a cytokine-mediated reaction that results in the generation of acute phase reactants and controlled elevation of core body temperature. The anterior hypothalamus coordinates the “febrile response” in reaction to the release of endogenous pyrogenes and subsequent up-regulation of prostaglandin synthesis. An ensuing change in the hypothalamic set point for temperature regulation advances a synchronized physiologic response from CNS to periphery, on a microscopic and macroscopic …

Introduction:

The advent of evidence-based medicine has resulted in higher quality journal manuscripts in numerous medical disciplines. However, the impact in the neurosurgical literature has not been reported.

Objective:

To quantify the impact of evidence-based medicine on the quality of articles published in the Neurosurgery literature.

Methods:

Articles published in the journal Neurosurgery (founded in 1977) were reviewed for 1978, 1988, 1998, and 2008. Each decade’s sample was classified as therapeutic, diagnostic and prognostic based on a published system for determining level of evidence.

Results:

438 articles were reviewed. Articles not considered included any published under the heading “Case Report” …

Optimizing queries in a distributed database is quite difficult. This work proposes defining new generalized quantifiers which operate on sets rather than tuples. These quantifiers would allow for easier optimization in a horizontally distributed database. These operators are scalable with respect to both the number of hosts in the environment and the size of the data used.

Aortic Valve Bypass surgery has found renewed interest to researchers due to its application in high-risk patients with severe aortic stenosis. This therapy consists of construction of a conduit containing a bioprosthetic or mechanical valve from the apex of the left ventricle to the descending thoracic aorta. Blood is ejected from the left ventricle via both the native stenotic aortic valve and the conduit. Many dedicated-use computer systems sold as complete products require a turn-key design delivered to the customer. This requires a system which is stable, secure, and serviceable. Adaptability of the system to existing software applications is a key consideration for many vendors. This thesis attempts to establish and gather best practices for designing, configuring, and building a Microsoft Windows Embedded Standard 2009 system. An existing real-world system will be used as a case study and example implementation. The end result will be a relatively compact, secure, and efficient Microsoft Windows Operating System image to support the target software application.

Career and Technical Education (CTE) has evolved from industry-specific training to a more broad-based education that incorporates core academic learning objectives and prepares the student for work (Rothwell & Gerity, 2008; Uloa, 2006; Reese, 2002; Rojewski, 2002; Stone, 2002; Schmidli, 2001). Little is known about how well individual programs align with the industry for which graduates are prepared, (Zinser, 2003) or what CTE has meant to its graduates.
