Ethical Considerations In Access To Experimental Drugs For Treatment Use, Sonja Rakowski Sep 2010

Ethical Considerations In Access To Experimental Drugs For Treatment Use, Sonja Rakowski

Yale Medicine Thesis Digital Library

Do dying patients have a moral claim to access experimental drugs when all else has failed? This question has been the focus of an active and evolving debate concerning the rights of terminally ill patients, the nature of the drug development process, and the scope of federal regulation, with supporters arguing that seriously ill patients should be able to decide for themselves whether and when to attempt experimental therapies and opponents arguing that the resulting state of affairs would be disastrous for patient safety and for the integrity of the drug development process. This thesis concerns the ethical considerations surrounding …

Focusing On Optic Tectum Circuitry Through The Lens Of Genetics, Linda M. Nevin, Estuardo Robles, Herwig Baier, Ethan K. Scott

Neuroscience: Faculty Publications

The visual pathway is tasked with processing incoming signals from the retina and converting this information into adaptive behavior. Recent studies of the larval zebrafish tectum have begun to clarify how the 'micro-circuitry' of this highly organized midbrain structure filters visual input, which arrives in the superficial layers and directs motor output through efferent projections from its deep layers. The new emphasis has been on the specific function of neuronal cell types, which can now be reproducibly labeled, imaged and manipulated using genetic and optical techniques. Here, we discuss recent advances and emerging experimental approaches for studying tectal circuits as …

B-Vitamins And Fatty Acids In The Prevention And Treatment Of Alzheimer's Disease And Dementia: A Systematic Review, Peter J. Whitehouse Sep 2010

B-Vitamins And Fatty Acids In The Prevention And Treatment Of Alzheimer's Disease And Dementia: A Systematic Review, Peter J. Whitehouse

Faculty Scholarship

The increasing worldwide prevalence of dementia is a major public health concern. Findings from some epidemiological studies suggest that diet and nutrition may be important modifiable risk factors for development of dementia. In order to evaluate the strength of the available evidence of an association of dietary factors with dementia including Alzheimer's disease (AD), we systematically searched relevant publication databases and hand-searched bibliographies up to end July 2007. We included prospective cohort studies which evaluated the association of nutrient levels with the risk of developing dementia and randomized intervention studies examining the treatment effect of nutrient supplementation on cognitive function. …

Child, Family, State, And Gender Equality In Religious Stances And Human Rights Instruments: A Preliminary Comparison, Linda C. Mcclain Sep 2010

Child, Family, State, And Gender Equality In Religious Stances And Human Rights Instruments: A Preliminary Comparison, Linda C. Mcclain

Faculty Scholarship

The UN Convention on the Rights of the Child (CRC) recently began its third decade. Why has the United States still not ratified the CRC, celebrated as the most widely ratified international human rights treaty in history? Once again, this question is on the table: Congressional resolutions that President Obama should not transmit the CRC to the Senate for advice and consent rapidly followed intimations that the Obama Administration had some qualms about the U.S. keeping company only with Somalia in not ratifying it. Some scholars contend that enlisting the unique resources of religions would help to ground a culture …

Prenatal Undernutrition And Cognitive Function In Late Adulthood, Susanne R. De Rooij, Hans Wouters, Julie E. Yonker, Rebecca C. Painter

University Faculty Publications and Creative Works

At the end of World War II, a severe 5-mo famine struck the cities in the western part of The Netherlands. At its peak, the rations dropped to as low as 400 calories per day. In 1972, cognitive performance in 19-y-old male conscripts was reported not to have been affected by exposure to the famine before birth. In the present study, we show that cognitive function in later life does seem affected by prenatal undernutrition. We found that at age 56 to 59, men and women exposed to famine during the early stage of gestation performed worse on a selective …

Estimation Of Quality Adjusted Lifetime (Qal) Distribution., Biswabrata Pradhan Dr. Sep 2010

Estimation Of Quality Adjusted Lifetime (Qal) Distribution., Biswabrata Pradhan Dr.

Doctoral Theses

Quality Adjusted Lifetime (QAL)Normally, overall survival time is considered as the end point for many clinical trials to study the effectiveness of different treatments. If the survival time passes through different health states, which differ in their quality of life, then other endpoints are also considered for treatment comparison, which incorporates both quality and duration of life. It is, therefore, necessary to provide a composite measure for comparison of different treatment choices, specially in the context of clinical trials, after taking into account both quality and duration of life. This issue has been first addressed by Gelber and coauthors in …

High Hopes: Public Health Approaches To Reducing The Need For Health Care, Michele J. Orza Sep 2010

High Hopes: Public Health Approaches To Reducing The Need For Health Care, Michele J. Orza

National Health Policy Forum

Driven in part by a desire to contain health care costs, policymakers are looking beyond medical care for opportunities to reduce the need for expensive services. This paper briefly reviews current public health concepts and strategies for improving health that emphasize nonmedical factors such as behavior, socioeconomic status, and environment. It also provides examples of how these concepts and strategies undergird many of the public health provisions of the Patient Protection and Affordable Care Act, other legislation, and several programs and initiatives. These concepts include prevention, health in all policies, global health, the One Health Initiative, and climate change and …

Questions And Answers: Regarding The Gunnison Sage-Grouse Candidate Species Status Sep 2010

Questions And Answers: Regarding The Gunnison Sage-Grouse Candidate Species Status

United States Fish and Wildlife Service: Publications

What is the Service’s determination regarding the status of the Gunnison sage-grouse?

After evaluating all the available scientific and commercial information regarding the Gunnison sage-grouse, including an analysis of the threats to the species and sagebrush habitat, the U.S. Fish and Wildlife Service has determined that protection under the Endangered Species Act (ESA) is warranted. However, listing the Gunnison sage-grouse at this time is precluded by the need to address other listings of higher priority.

The Gunnison sage-grouse will be added to the list of candidate species under the ESA and will be proposed for listing when funding and workload …

Macrophage Integrin-Mediated, Hur-Dependent Angiogenic Factor Gene Expression, Yasha Modi Sep 2010

Macrophage Integrin-Mediated, Hur-Dependent Angiogenic Factor Gene Expression, Yasha Modi

Yale Medicine Thesis Digital Library

Activated macrophages, via the secretion of pro-angiogenic factors, modulate the angiogenic response to inflammation. Several of these factors, including vascular endothelial growth factor (VEGF), are encoded by inherently unstable mRNA transcripts containing AU-rich elements (AREs) in their 3' untranslated region. During stress events, the half-lives of these mRNAs must be prolonged to allow for significant protein production and an adequate angiogenic response. This laboratory has demonstrated that engagement of the β-integrin receptor, LFA-1, leads to stabilization of ARE-bearing mRNAs in T lymphocytes through a nuclear-to-cytoplasmic translocation of the RNA-binding protein, HuR. Here, we address whether β-integrin engagement stabilizes VEGF mRNA …

Incidence Of Leukoencephalopathies With Restricted Diffusion On Magnetic Resonance Imaging, Michael William Destefano Sep 2010

Incidence Of Leukoencephalopathies With Restricted Diffusion On Magnetic Resonance Imaging, Michael William Destefano

Yale Medicine Thesis Digital Library

The purpose of our study was to determine the incidence, causes, and reversibility of leukoencephalopathies demonstrating confluent areas of restricted diffusion on magnetic resonant imaging (DWI+LE). We hypothesized DWI+LE would have a low incidence, and be primarily caused by toxic exposures. We performed a logic sentence based search of the Yale-New Haven MRI database to select for reports indicating restricted diffusion within the cerebral white matter. We examined patients neuroimaging studies and medical record. We identified a total of 35 cases of DWI+LE, which resulted in an overall incidence of 0.2% over the five-year period queried. The medical conditions associated …
