Pathology, East Africa

Background: Artemether-lumefantrine (AL) is a major and highly effective artemisinin-based combination therapy that is becoming increasingly important as a new first-line therapy against Plasmodium falciparum malaria. However, recrudescences occurring after AL treatment have been reported. Identification of drug-specific parasite determinants that contribute to treatment failures will provide important tools for the detection and surveillance of AL resistance.

Methods: The findings from a 42-day follow-up efficacy trial in Tanzania that compared AL with sulfadoxinepyrimethamine (SP) were analyzed to identify candidate markers for lumefantrine tolerance/resistance in the chloroquine resistance transporter gene (pfcrt) and multidrug resistance gene 1 (pfmdr1). The findings were corroborated …

Individual Accountability For Human Rights Abuses: Historical And Legal Underpinnings, Steven R. Ratner, Jason S. Abrams, James L. Bischoff Jan 2009

Individual Accountability For Human Rights Abuses: Historical And Legal Underpinnings, Steven R. Ratner, Jason S. Abrams, James L. Bischoff

Book Chapters

The international legal community is beset today with talk of accountability. Governments, international organizations, non-governmental organizations, and scholars speak of the need to hold individuals responsible for official acts that violate the most cherished of international human rights. Some study the nature of various infractions with an eye toward codification; others seek to create or engage mechanisms for trying or otherwise punishing individuals. Their common mission is based on a shared understanding that international law has a role to play not only in setting standards for governments, non-state actors, and their agents, but in prescribing the consequences of a failure …

Privilege Or Punish: Criminal Justice And The Challenge Of Family Ties, Dan Markel, Jennifer M. Collins, Ethan J. Leib Jan 2009

Privilege Or Punish: Criminal Justice And The Challenge Of Family Ties, Dan Markel, Jennifer M. Collins, Ethan J. Leib

Faculty Journal Articles and Book Chapters

This book answers two basic but under-appreciated questions: first, how does the American criminal justice system address a defendant's family status? And, second, how should a defendant's family status be recognized, if at all, in a criminal justice system situated within a liberal democracy committed to egalitarian principles of non-discrimination? After surveying the variety of "family ties benefits" and "family ties burdens" in our criminal justice system, we explain why policymakers and courts should view with caution and indeed skepticism any attempt to distribute these benefits or burdens based on one's family status. This is a controversial stance, but we …

Background: Intermittent preventive treatment (IPT) is a promising strategy for malaria control in infants. We undertook a pooled analysis of the safety and efficacy of IPT in infants (IPTi) with sulfadoxine-pyrimethamine in Africa. Methods: We pooled data from six double-blind, randomised, placebo-controlled trials (undertaken one each in Tanzania, Mozambique, and Gabon, and three in Ghana) that assessed the efficacy of IPTi with sulfadoxine-pyrimethamine. In all trials, IPTi or placebo was given to infants at the time of routine vaccinations delivered by WHO's Expanded Program on Immunization. Data from the trials for incidence of clinical malaria, risk of anaemia (packed-cell volume <25% or haemoglobin <80 g/L), and incidence of hospital admissions and adverse events in infants up to 12 months of age were reanalysed by use of standard outcome definitions and time periods. Analysis was by modified intention to treat, including all infants who received at least one dose of IPTi or placebo. Findings: The six trials provided data for 7930 infants (IPTi, n=3958; placebo, n=3972). IPTi had a protective efficacy of 30·3% (95% CI 19·8-39·4, p<0·0001) against clinical malaria, 21·3% (8·2-32·5, p=0·002) against the risk of anaemia, 38·1% (12·5-56·2, p=0·007) against hospital admissions associated with malaria parasitaemia, and 22·9% (10·0-34·0, p=0·001) against all-cause hospital admissions. There were 56 deaths in the IPTi group compared with 53 in the placebo group (rate ratio 1·05, 95% CI 0·72-1·54, p=0·79). One death, judged as possibly related to IPTi because it occurred 19 days after a treatment dose, was subsequently attributed to probable sepsis. Four of 676 non-fatal hospital admissions in the IPTi group were deemed related to study treatment compared with five of 860 in the placebo group. None of three serious dermatological adverse events in the IPTi group were judged related to study treatment compared with one of 13 in the placebo group. Interpretation: IPTi with sulfadoxine-pyrimethamine was safe and efficacious across a range of malaria transmission settings, suggesting that this intervention is a useful contribution to malaria control. Background: Chlorproguanil-dapsone-artesunate (CDA) was developed as an affordable, simple, fixed-dose artemisinin-based combination therapy for use in Africa. This trial was a randomized parallel-group, double-blind, double-dummy study to compare CDA and artemether-lumefantrine (AL) efficacy in uncomplicated Plasmodium falciparum malaria and further define the CDA safety profile, particularly its hematological safety in glucose-6-phosphate dehydrogenase (G6PD) -deficient patients. Methods and Findings: The trial was conducted at medical centers at 11 sites in five African countries between June 2006 and August 2007. 1372 patients (≥1 to <15 years>old, median age 3 years) with acute uncomplicated P. falciparum malaria were randomized (2:1) to receive CDA 2/2.5/4 …

Coartem°: The Journey To The Clinic, Zul Premji Jan 2009

Coartem°: The Journey To The Clinic, Zul Premji

Pathology, East Africa

Artemisinin, from which the artemether component of Coartem° (artemether/lumefantrine, AL) is derived, is obtained from the plant sweet wormwood (Artemisia annua) which has been used for over 2,000 years as a Chinese herbal remedy. Artemisinin was first identified by Chinese researchers as the active anti-malarial constituent of A. annua and its derivatives were found to be the most potent of all anti-malarial drugs. Artemether acts rapidly, reducing the infecting parasite biomass by approximately 10,000-fold per asexual life cycle. Lumefantrine, the other active constituent of AL, acts over a longer period to eliminate the residual 100-100,000 parasites that remain after artemether …

The Game Of Conservation: International Treaties To Protect The World’S Migratory Animals, Mark Cioc Jan 2009

The Game Of Conservation: International Treaties To Protect The World’S Migratory Animals, Mark Cioc

Ohio University Press Open Access Books

The Game of Conservation is a brilliantly crafted and highly readable examination of nature protection around the world.

Twentieth-century nature conservation treaties often originated as attempts to regulate the pace of killing rather than as attempts to protect animal habitat. Some were prompted by major breakthroughs in firearm techniques, such as the invention of the elephant gun and grenade harpoons, but agricultural development was at least as important as hunting regulations in determining the fate of migratory species. The treaties had many defects, yet they also served the goal of conservation to good effect, often saving key species from complete …

Explaining Ethnic Peace: The Importance Of Institutions, Rebecca Tong, '09 Jan 2009

Explaining Ethnic Peace: The Importance Of Institutions, Rebecca Tong, '09

Honors Projects

This paper examines the cause of ethnic peace, and subsequently, the cause of ethnic violence. Varying theories explain ethnic violence: primordialism, instrumentalism, and constructivism. The question central to this study is how master narratives, scarce resources, and democratic institutions have influenced the occurrence of ethnic violence. Small n comparison is used to analyze two pairs of sub-Saharan African nations in order to control for other explanatory variables: (1) Cote d'Ivoire and Ghana, and (2) Kenya and Tanzania. In each pair, one nation is relatively peaceful and the other relatively violent. Drawing from ethnographic research papers, and news sources this paper …

Temporal And Spatial Stability Of Anopheles Gambiae Larval Habitat Distribution In Western Kenya Highlands, Li Li, Ling Bian, Laith Yakob, Guofa Zhou, Guiyun Yan

Political Science & Geography Faculty Publications

Background: Localized mosquito larval habitat management and the use of larvicides have been proposed as important control tools in integrated malaria vector management programs. In order to optimize the utility of these tools, detailed knowledge of the spatial distribution patterns of mosquito larval habitats is crucial. However, the spatial and temporal changes of habitat distribution patterns under different climatic conditions are rarely quantified and their implications to larval control are unknown.

Results: Using larval habitat data collected in western Kenya highlands during both dry and rainy seasons of 2003-2005, this study analyzed the seasonal and inter-annual changes in the spatial …

Parametric Study Of Stimulus-Response Behavior Incorporating Vehicle Heterogeneity In Car-Following Models, Saidi Siuhi Jan 2009

Parametric Study Of Stimulus-Response Behavior Incorporating Vehicle Heterogeneity In Car-Following Models, Saidi Siuhi

UNLV Theses, Dissertations, Professional Papers, and Capstones

The objective of this study was to develop a family of car-following models that address the shortcomings of car-following models developed by General Motors (GM) in the 1950s. The developed models consist of separate models for acceleration, deceleration, and steady-state responses for congested freeway traffic conditions. The study calibrated the models using individual vehicle trajectory data collected on a segment of Interstate 101 in Los Angeles, California. Furthermore, the study validated the models using individual vehicle trajectory data collected on a segment of Interstate 80 in Emeryville, California. The study used nonlinear regression with robust standard errors to estimate the …

A New Record Of Procynosuchus Delaharpeae (Therapsida: Cynodontia) From The Upper Permian Usili Formation, Tanzania, D. Marie Weide, Christian A. Sidor, Kenneth D. Angielczyk, Roger M. H. Smith Jan 2009

A New Record Of Procynosuchus Delaharpeae (Therapsida: Cynodontia) From The Upper Permian Usili Formation, Tanzania, D. Marie Weide, Christian A. Sidor, Kenneth D. Angielczyk, Roger M. H. Smith

Department of Earth and Atmospheric Sciences: Faculty Publications

Procynosuchus, the best-known Permian cynodont, has a remarkably broad geographic range, with records stretching from southern Africa to Europe. Fossils' of Procynosuchus are most common in the Upper Permian Dicynodon Assemblage Zone of South Africa, but also occur in coeval East African rocks. Currently, there is one documented occurrence from the Madumabisa Mudstone Formation of Zambia, and two specimens from the Usili (=Kawinga) Formation of Tanzania. The Tanzanian specimens include a poorly preserved, incomplete skull and a partial cranium originally attributed to Parathrinaxodon proops. The latter is now considered a subjective junior synonym of Procynosuchus delaharpeae. Here we …

The Lived Experience Of Hiv-Positive Tanzanian Orphans, Carol A. Parker

Walden Dissertations and Doctoral Studies

The purpose of this investigation was to generate a context- specific model of the lived experience of HIV positive Tanzanian orphans. With the advent of anti-retroviral drugs, Sub-Saharan African populations with high burdens of human immunodeficiency virus (HIV) face annually accumulating increases in the number of HIV positive children, and ultimately adults. Perinatally infected, these children often become orphans while still young. Extended interviews were performed with a purposive sample of 12 HIV infected orphaned children between 9 and 12 years of age. Half of the research participants lived in an orphanage and half were children living with guardians in …

Using Decision Analysis To Improve Malaria Control Policy Making, Jonathan B. Wiener, Randall A. Kramer, Katherine L. Dickinson, Richard M. Anderson, Vance G. Fowler, Marie Lynn Miranda, Clifford M. Mutero, Kathryn A. Saterson

Faculty Scholarship

Malaria and other vector-borne diseases represent a significant and growing burden in many tropical countries. Successfully addressing these threats will require policies that expand access to and use of existing control methods, such as insecticide-treated bed nets (ITNs) and artemesinin combination therapies (ACTs) for malaria, while weighing the costs and benefits of alternative approaches over time. This paper argues that decision analysis provides a valuable framework for formulating such policies and combating the emergence and re-emergence of malaria and other diseases. We outline five challenges that policy makers and practitioners face in the struggle against malaria, and demonstrate how decision …
