Introduction: Functional appliances are intended to correct by incremental advancement of the mandible for correction of skeletal Class 11 relationships. Similar in design to functional appliances, mandibular advancing devices (MAD) are used for obstructive sleep apnea (OSA) patients as they increase the size of the airway by drawing the tongue and soft palate forwards, and thus maintain its patency during sleep. It has been shown that significant increases in airway dimensions and decreases in airway resistance were gained in OSA patients while evaluated with a MAD in place. Despite the increase in children born prenatally exposed to methamphetamine, little is known about the cognitive and neuropsychological outcomes of these children. Research specific to prenatal-methamphetamine exposure is extremely limited and has been primarily restricted to rat studies. This research combined with the few studies examining children prenatally exposed to methamphetamine suggests that methamphetamine-exposure is associated with various cognitive and neuropsychological delays and is impacted by both biological and environmental factors. Pediatric overweight and obesity is a growing national epidemic with 15 percent of children ages 6-19 considered obese and at least 22 percent considered overweight. Although there are many suggested causes, treatments, and preventions for this epidemic, a construct that is showing promise in the current literature as a preventive factor against obesity is breastfeeding in infancy. However, the mechanisms by which breastfeeding may impact later childhood weight are unknown. When a doctoral student plans to conduct qualitative education research, the aspect of the dissertation that often becomes problematic is determining which theoretical paradigm(s) might frame the study. In this article, the author discusses how he resolved the quandary through eclecticism. The author begins by describing briefly the purpose of his dissertation study, providing a justification for eclecticism in the selection of theories. The thermal reaction of [Os3(CO)10(μ-H)(μ-SN2C4H5)] (1) at 110 °C afforded the new compound [Os3(CO)9(μ-H)(μ 3-η 2-SN2C4H5)] (2) in 84% yield. Compound 2 exists as two isomers, which differ in the disposition of the bridging hydride ligand. Both of the isomers of 2 have been characterized by a combination of elemental analysis, infrared and 1H NMR spectroscopic data together with single crystal X-ray crystallography. The isomers …

Case Report: A 61-year-old-woman with a past medical history significant for hypertension, bipolar disorder, obstructive sleep apnea and chronic obstructive pulmonary disease presented with diffusive lymphadenopathy suspicious for lymphoma. A lymph node in her right groin was biopsied by fine needle aspiration and showed atypical cells, but was not diagnostic. An excisional biopsy of the 2 x 2 cm lymph node demonstrated metastatic Merkel cell carcinoma (MCC). No skin lesions were detected. A CT scan revealed the presence of a large nasopharyngeal mass that following surgical removal was diagnosed as MCC. Treatment with etoposide and cisplatin and radiation were instituted. …

Secondary bronchiolitis obliterans-organizing pneumonia (BOOP) is a reaction to injury, forming granulation tissue with airspaces. Idiopathic cryptogenic organizing pneumonia (COP) and secondary BOOP are histologically similar. COP has been reported to show increased collagen in the granulation plugs and fewer myofibroblasts and capillaries. Using CD34 for capillaries and alpha-smooth muscle actin for myofibroblasts, we assessed these statins to differentiate COP from secondary BOOP.

Metabolic engineering of cellular systems to maximize reaction fluxes or metabolite concentrations still presents a significant challenge by encountering unpredictable instabilities that can be caused by simultaneous or consecutive enhancements of many reaction steps. It can therefore be important to select carefully small subsets of key enzymes for their subsequent stable modification compatible with cell physiology. An individual’s age at first reproduction and investment in successive reproductive attempts are involved in mechanisms that can impede somatic repair, resulting in a decline in reproductive abilities with age (reproductive senescence). We used long-term data from the Black-legged Kittiwake, a long-lived seabird, to address the relationship between recruitment age, age-specific breeding success (BS), and reproductive senescence, while accounting for breeding experience and temporal variation in BS. We first detected late-life improvement in BS across all recruitment groups, which we recognized as ‘‘within-generation selection’’ or the selective disappearance of ‘‘frail’’ phenotypes. Performance Flight Testing Of Small Electric Powered Unmanned Aerial Vehicles, Jon N. Ostler, W. Jerry Bowman, Deryl O. Snyder, Timothy W. Mclain

Flight testing methods are developed to find the drag polar for small unmanned aerial vehicles powered by electric motors with fixed-pitch propellers. Wind tunnel testing was used to characterize the propeller-motor efficiency. The drag polar was constructed using data from flight tests. This drag polar was then used to find the following performance parameters: maximum velocity, minimum velocity, velocity for maximum range, velocity for maximum endurance, maximum rate of climb, maximum climb angle, minimum turn radius, maximum turn rate, and maximum bank angle. The developed flight testing methods are used to characterize the performance of a small UAV.

Chagas disease, caused by the protozoan parasite, Trypanosoma cruzi, is the most important vector-borne disease in Latin America. It is estimated that 16 to 18 million people are infected with Chagas disease and some 50,000 people die each year. In endemic countries, people living in rural areas are at greatest risk for acquiring infection. Ecuador is one of these countries, and it is estimated that between 2.4-3.8 million people in Ecuador are exposed to Chagas disease, primarily in the Eastern endemic coastal region. Attention is a complex construct involving extensive interactions with working memory and executive control systems to process and extract meaning out of large amounts of multi-sensory information. Disruption of attention is a characteristic symptom in schizophrenia, and further studies on selective attention are crucial for understanding the disease. The current study looks at how subjects with schizophrenia selectively attend to either a visual or auditory metronome in the presence of asynchronous cross-modal distractors of 3 different frequencies (0.5, 1, and 2 Hz). Highly dense, repetitive antigens such as virus particles induce strong immune responses. Correspondingly, Virus Like Particles (VLPs), which consist of the viral structural proteins, can be used as molecular scaffolds to increase the antigenicity of normally poorly immunogenic antigens. This ability to elicit strong antibody responses is not limited to foreign antigens, but also to self-antigens, which are normally subject to B-cell tolerance. The intention of this proposal is to develop a system for rapid identification of asthma vaccine candidates based on potent immunogenicity of antigens (IgE) displayed in dense repetitive arrays on virus-like particles. Growth of individual carbon nanotube (CNT) intra-connects (bridges) between two prefabricated electrodes is a great challenge and a pre-requisite to the development of single electron devices. In this thesis, CNT intra-connects were fabricated and studied. Later on, the intra-connects were deposited with electrically conductive polymer (ECP) in the realization of CNT/ECP hybrids.

The process started by realizing two electrodes with sharp end on a silicon wafer using e-beam lithography. The intra-connects were then grown by use of chemical vapor deposition (CVD) technique. In microfluidic devices, the fluid can be manipulated either as continuous streams or droplets. The latter is particularly attractive as individual droplets can not only move but also split and fuse, thus offer a greater flexibility in applications such as a laboratory-on- a-chip. In this thesis, a technique is developed that uses an externally applied electric field for manipulating and removing particles trapped on the surface of a drop. The drop is assumed to be immersed in another liquid with which it is immiscible, and the electric field is generated by placing electrodes on the sides of the microdevice. Both …

This paper provides a concise introduction to targeted maximum likelihood estimation (TMLE) of causal effect parameters. The interested analyst should gain sufficient understanding of TMLE from this introductory tutorial to be able to apply the method in practice. A program written in R is provided. This program implements a basic version of TMLE that can be used to estimate the effect of a binary point treatment on a continuous or binary outcome.

As part of the diverse discussions around health care reform, many have looked to refining Medicare payment systems as a way to give health care practitioners and providers greater incentives to deliver care more efficiently, and thus slow health care spending growth. Understanding how Medicare currently pays for Part B services, including drugs covered under Part B, is essential to understanding the potential impact of these types of reforms.
