Microfinance institutions (MFIs) established to provide the poor with access to capital have typically operated outside of their countries' regulated banking environments. Many have relied on donor grants and low-interest funds to support loan portfolios and social programs. As MFIs mature they aspire to become more efficient and attain economic sustainability because hey understand that greater numbers of the poor can be serviced by economically sustainable institutions. Many MFIs collect savings deposits but are often barred from using them for loans by their countries' laws. Acetaminophen-induced hepatotoxicity is the most frequent cause of fulminant liver failure, and can have a mortality rate 90%. The present work aimed at the investiga­tion of the possible effect of trimetazi-dine in preventing acetaminophen -induced hepatotoxicity and comparing it with the traditionally used N- acetyl cysteine for the same purpose. Zinc deficiency in children is an important public health problem in the developing world {Manary et al, 2002). Several lines of evidence sug­gest that zinc status of our infants and children are marginal or low. First, animal products, the main source of zinc, represent only a small percentage of the usual diet. Second, high consumption of rice and vegeta­bles may preclude adequate zinc ab­sorption because of their high phytate and fiber contents. 為測試以「問題導向學習」(PBL)教學法，教授醫學倫理之可行性，我們在醫學院四年級之同學，連續兩年間，以前半段合堂上課，後半段PBL個案討論之方式，實施倫理教學。此課程為一學分，兩週上課一次，一次兩小時，以利PBL之推行。前半的內容在於哲學思維及尊重自主、行善、不傷害及社會正義四大原則之教學，後半段則採用了安寧療護，器官移植，和SARS的個案作為討論之對象。學生基本上可以接受這樣的教學方式，他們感謝這樣的教法可以提高思考能力，對將來和病人溝通、協調的重要性也更能體會。另一方面，對PBL個案的使用上，同學希望增加討論之個案數，另外，也期待在選擇上，讓個案的情況更適合於倫理原則之運用，如有牽涉太多法條的個案較不適用等。我們覺得，即使同一個案，時代不同，需要注意隨醫學進步而有不同的倫理考量。而師資之培養，亦是遂行倫理學PBL教學極為重要之因素。

中國醫藥大學醫學系自2001年起進行大規模的課程改革，設置教改執行委員會統籌，將傳統依學科作大堂授課的課程，改為依器官模組作混合式教學的「新整合課程」，並增加問題導向學習(problem-based learning, PBL)及臨床技能與溝通(clinical skills and communication, CSC)的課程。從2001至2005年間共有三屆377名醫學生完成臨床十二個模組，學生們對模組課程評論之問卷回收率為71.5%(54.1%~97.0%)，三屆學生總平均滿意度有逐年提昇之趨向，分別為3.76、3.81及3.98（5分很滿意；1分很不滿意）。總共2146人次之敘述性評論回饋中，正面贊同性回饋佔26.3%，批評建議性回饋佔73.7%；依課程安排、教師教學、教學資源、學業評量、其他等五項分類之敘述性建議，對新整合課程之推動有極大的助益。如文獻所述，領導團隊、合作風氣、團體成員參與、評論檢討、人力資源發展、行政配合等是成功推展教改必備的要素，本校教改的經驗，可供其他醫學院參考。

營造無菸校園：一個健康促進學校計畫, 蘭 李, 涵文 晏, 松景 高

本研究在雅動一個以營造無菸校園為目的之健康促進學校計畫。為評價介入效果，採用類實驗研究設計，立意選取同一行政區內的一所中學為實驗學校，另一所中學為對照學校。實驗學校的學生代表受邀參加一系列的增能活動後，自發性地組成以促進學校健康為順景之學生社團。社員們完成自我成長課程後，於校園內推出各種以「拒菸」為訴求的活動。實驗學校和對照學校的國一至國三各有一半的班級被隨機抽出，在這些被抽中的班級中，同時完成介入前（前測）和介入後（後測）之問卷調查的學生共有597人。在介入活動結束後，實驗學校的學生在菸害知識、拒菸態度、覺知不吸菸對健康的重要、菸害防制資訊與服務的可近性等，都顯著優於對照學校的學生。因此，建議可應用社區營造策略於健康促進學校計畫中。

臨床醫學教育與Mini-Cex, 偉德 陳, 長海 蔡, 崑巖 黃

師徒制是臨床醫學教育所特有的，但國內外的調查卻皆顯示相當多數的醫學生或住院醫師，從未在主治醫師的直接觀察下從事醫療技能的操作。美國內科醫學會所發展的「迷你臨床演練評量(mini-CEX)」係由一位主治醫師直接觀察住院醫師對病人做重點式的臨床診療工作，並於結構式表單上根據醫療面談、身體檢查、人道專業、臨床判斷、諮商衛教、組織效能、及整體臨床稱職等項目給予評分，並給予即時的回饋；其特點是可在例行的門、急診或住院工作中同步進行，僅需費時20-30分鐘，可行性較高，而且係經由不同的主治醫師評量，其信效度亦較優良。希望藉著引進mini-CEX，能使台灣的臨床醫學教育達成醫學教育白皮書中所要求的目標。

日本岐阜大學醫學教育開發研究中心是負責日本國內醫學教育發展的國立機構，每年舉辦至少四場醫學教育研習會，第十五回醫學教育研習會於2005年1月29-30日在東京慈惠醫科大學西新橋校區舉行，筆者應岐阜大學醫學部高橋優三教授與鈴木康之教授之邀請前往參加。本次研習會的主題有四：(l)住院醫師訓練結束時之客觀結構式臨床能力測驗(Objective Structured Clinical Examination, OSCE)；(2)基礎醫學教育之實驗課程整合；(3)筆試測驗之設計與評量；(4)醫療溝通技巧之訓練。本心得報告將介紹由近畿大學松尾理教授所主持的主題二：基礎醫學教育之實驗課程整合。

We present the results of HST/WFPC2 medium and narrow band imaging and VLA and MERLIN2 radio imaging of three powerful radio galaxies: 3C 171, 3C 277.3, and PKS 2250-41. We obtained images of the rest frame [OIII] 5007 and [OII] 3727 line emission using the Linear Ramp Filters on WFPC2. The correlations between the emission line morphology and the [OIII]/[OII] line ratios with the radio emission seen in ground based observations are clarified by the HST imaging. We confirm that the radio lobes and hot-spots are preferentially associated with lower ionization gas. 3C 171 exhibits high surface brightness emission line …

In 1992 my wife, Claudia, published a book titled America Discovers Columbus: How an Italian Explorer Became an American Hero. The book argued that until the American Revolution, Columbus was almost completely neglected in histories of the British colonies. Not until three centuries after the fact did North Americans honor him as the discoverer of America.
