The Changing Face Of Rural General Practice: An Ethnographic Study Of General Practitioners And Their Spouses, Angela Durey

Rural general practice is general practice at its best: a comment by one GP interviewed for this study was echoed by colleagues who viewed their work in a rural setting as challenging, diverse, rewarding and satisfying. Despite reported difficulties associated with rural general practice, many GPs argued that the benefits outweigh the disadvantages. Few wanted to leave. Nonetheless, too few Australian trained GPs are willing to move from cities to work in the country. Consequently, overseas trained doctors have been recruited to fill vacancies or nurses provide health services in communities unable to attract a GP.

This study investigated and described the status and quality of secondary science teaching and learning in Lagos State, Nigeria. Quantitative and qualitative methods were used for gathering research data. Quantitative data were obtained from the surveys of 78 junior secondary science teachers and 500 junior secondary students from three Local Education Districts of Lagos State. Inexpensive and commercially available Global Positioning System (GPS) receivers can be used in novel applications, such as nanosatellites and unmanned aerial vehicles (UAVs), by modifying the receiver's software. Unfortunately, typical GPS software development systems cost tens of thousands of dollars and have restrictive license agreements, such as non-disclosure agreements, for both their hardware and software. This thesis describes the design and construction of an open source GPS receiver development system that addresses these unnecessary restrictions. The ISO14001 and OSHAS 18001 standards ("the Standards") have emerged as effective EHS management tools to serve those needs. Corporate charter competition among U.S. states has been held out as a model of welfare-enhancing regulatory competition. Proponents of this story also rely on it as a basis for promoting regulatory competition in international securities regulation. Issuer choice proponents argue that an issuer of securities should be permitted to choose the securities regulation of any nation to govern its securities offerings and trading worldwide. This Article challenges the notion that the claimed success of corporate charter competition among U.S. states argues in favor of issuer choice for international securities regulation.

Although the goal of arbitration is speedy and efficient resolution of disputes, these goals may be frustrated by judicial interpretation of choice of law provisions in commercial agreements. There is uncertainty as to the law that a court will apply in deciding a motion to stay or otherwise interfere with an arbitration. There is further uncertainty surrounding whether the law governing arbitration is the Federal Arbitration Act (FAA), the arbitration law of the state where the arbitration is taking place, or that of the state mentioned in a choice of law clause. This thesis describes the creation of a lay public educational brochure for Rochester General Hospital, showing asthmatic children how to use a spacer device with a metered dose inhaler. The thesis was created using the author's original artwork and was translated and adapted for a Spanish version. The second part of the thesis describes the revision of the logo for Camp Bronchopower, a children's asthma camp run by Rochester General Hospital.

Federal statutes, like the lawmaking enterprise itself, are seldom models of efficiency. Whether through inevitable laxity or conscious choice, Congress when legislating leaves a fair number of gaps in the meaning of its complex regulatory schemes. In filling those gaps with case-specific interpretive responses, federal courts perform an important policymaking function.
Such policymaking has lately generated increased concerns about the politicization of the judiciary. The complex interfacial phenomena involved in two-phase gas-liquid flow have defied mathematical simplification and modeling. However, the systems are used in heat exchangers, condensers, chemical processing plants, and nuclear reactor systems. The present work considers a 1 mm square minichannel and adiabatic flows corresponding to practical PEM fuel cell conditions. Pressure drop data is collected in experimentation covering mass fluxes of 4.0-33.6 kg/m2s, which correspond to superficial gas and liquid velocities of 3.4-10 m/s and 0.001-0.02 m/s respectively. The experiments are repeated with water of reduced surface tension, caused by the addition of surfactant, in order to quantify the surface …

This essay traces the vein of thought represented by Calabresi's "The Costs of Accidents", both backward in time to examine its sources, and forward to its impact on current scholarship. In two important books, The Republic of Choice and The Horizontal Society, published in 1990 and 1999 respectively, Lawrence M. Friedman presents his theories of a massive social transformation which occurred in the last century. I wish to examine these theories through the prism of two cases: Elk Grove Unified School District v. Newdow3 and Hamdi v. Rumsfeld,4 both decided in the spring of 2004. Forty years after the passage of Title VII, scholars Marianne Bertrand and Sendhil Mullainathan reported the results of their groundbreaking study, Are Emily and Greg More Employable Than Lakisha and Jamal? A Field Experiment on Labor Market Discrimination. Their study revealed that simply having an African American-sounding name significantly decreased one's opportunity to receive a job interview, regardless of occupation or industry. The results of Bertrand and Mullainathan's investigation raise critical questions about the effectiveness of Title VII as a remedy for race discrimination in the hiring market today, especially as employment discrimination has evolved into different forms. While neoclassical economic theory suggests that arbitrage will undermine global differential pricing of pharmaceuticals, the empirical results are more complex. Pharmaceutical regulation, IP laws, global trade agreements, and company policies support differential pricing despite the pressure of arbitrage. For essential access programs in particular, the theoretical threat of pharmaceutical arbitrage is shown to be rarely observed empirically. Counterfeiting is demonstrated to be the more serious threat. These conclusions call for changes in the U.S. PEPFAR program for AIDS and in the implementation of the WTO TRIPS Agreement.
A more fundamental question, however, is whether pharmaceutical differential pricing is appropriate for …

This Article looks at the coming-of-age stories in two recent films, Bend It Like Beckham and Real Women Have Curves, as an avenue to explore the question of constructing identity. Both films, arising out of the filmmakers' experiences, aim to offer representations of particular individuals in minority groups that challenge dominant representations. They also offer aspirational visions of how such individuals might find a way to construct a hybrid identity that allows them to negotiate their place within the various groups that claim them and within the broader society. Arbitration unfolds within an enclosure created by the contract terms and the applicable arbitration law. Some measure of judicial scrutiny must be imposed to ensure that an award does not fall beyond an arbitrator’s authority. But how should one identify excess of authority? The House of Lords decision in Lesotho Highlands v. Impreglio serves as a prism through which to separate several themes that inhere in the nature of arbitral authority. In rejecting arguments that an error about the currency of an award represented an excess of jurisdiction, their Lordships confirmed a healthy appreciation that arbitrators do not exceed their …

Penicillin and other antibiotics were the original wonder drugs and laid the foundation of the modern pharmaceutical industry. Human health significantly improved with the introduction of antibiotics. By 1967, the US Surgeon General declared victory over infectious diseases in the US. But pride goes before a fall. The evolutionary pressure of antibiotic use selects for resistant strains with the least fitness cost. Effective drugs should be used. But when they are used, no matter how carefully, evolutionary pressure for resistance is created. The problem is not limited to antibiotics. Variants of the human immunodeficiency (AIDS) virus develop resistance to anti-retroviral …

This article considers a number of different ways of conceptualizing the relationship between South and North in the environmental context, focusing on international responses to climate change and, particular, the Kyoto Protocol to the United Nations Framework Convention on Climate Change. It explores three stories about international cooperation. One derives from the concept of "ecological debt," the second comes from the concept of "environmental space," and the third, which might be said to underlie the U.S. approach to the Kyoto Protocol at the present time, is labelled "leading towards a level playing field." Metallic Cu and Au shells were fabricated around cobalt nanoparticles. A new technique to coat nanoparticles with carbon coatings and poly(methyl methacrylate) (PMMA) was developed. The copper shell formation is a self-limiting process. A thin copper shell (0.82 nm) around the cobalt nanoparticle (1.56 nm) enhanced the magnetic property by increasing the blocking temperature from 124 K to 235 K for nanoparticles with a copper shell. The formed gold shell (0.67 nm) enhanced the cobalt nanoparticle magnetic property by increasing the blocking temperature above room temperature. The magnetic moment in the Co-Cu and Co-Au core-shell nanoparticle is much higher than …

This study describes the policymaking process and policy solutions enacted in the Kentucky Postsecondary Improvement Act of 1997 (or House Bill 1). The study employs both an historical recounting of the story of House Bill 1 and a narrative analysis of opinion-editorials and policymaker interviews to reveal and explain how political power comprised both the perennial problem of Kentuckys higher education policymaking and the tool with which conflicts over power distribution were resolved. Inspired by philosopher Richard Rorty’s assertion that we need poetic imagination to move beyond modernity, this pragmatist inquiry seeks to understand the poetic, not as it is rationally defined, but rather as a historically situated discursive practice. In western cultures modernist discursive practices are characterized by rationalist reasoning. Traced to Aristotelian re- interpretations of mimesis (this is that), which structure modernist forms of representation, mimesis operates on a principle of equivalency between the specific and the universal, based on Euclidean, geometric reasoning. Likewise, mimesis underlies poetic representation as an imaginative expression of an objective reality, ideally, a form that moves …

The primary focus of the current study was to examine the relations between children’s peer relationships and academic performance. This study involved a secondary analysis of data provided by children, parents, and teachers. In all, 585 families participated in the study. The sample is predominately middle class, generally representative of the geographical regions, and demographically diverse; 52% of the children were male, 19% were of a minority or ethnic background, and 26% lived with single parents. The current study analyzed data collected over a four year period beginning when the children were enrolled in kindergarten. Data were obtained using classroom-based …
