OBJECTIVE: To evaluate our experience of management of necrotizing fasciitis (NF) particularly the role of early and aggressive surgical intervention. METHODS: This is a retrospective review of the cases affected by this disease and managed at Riyadh Medical Complex, Riyadh, Kingdom of Saudi Arabia, during 5 calendar years from 1996 through to 2000 (1416-1420 A.H). The preoperative and operative data was collected, and postoperative morbidity and mortality were analyzed. RESULTS: A total of 25 cases of NF were treated during the 5-year period, 18 were males and 7 female. The majority of patients had perianal suppuration as the primary lesion, …

Research on the western gray whale (Eschrichtius robustus) population summering off northeastern Sakhalin Island, Russia, has been ongoing since 1995. This collaborative Russia-U.S. research program has produced important new information on the present day conservation status of this critically endangered population. This report reviews recent findings from 2003 research activities and combines such with data from previous years, in some cases ranging back to 1994. Photo-identification research conducted off Sakhalin Island in 2003 resulted in the identification of 75 whales, including 11 calves and two previously unidentified non-calves. Restocking has conventionally been viewed as a short-term relief or recovery intervention initiated by development agencies for the benefit of destitute pastoralists. In contrast, this case study illustrates how restocking became a community initiated and organized strategy to alleviate poverty in specific communities with support from a development agency. The case study outlines the process that occurred in Samburu district of northern Kenya through which a number of communities designed restocking programs to meet their perceived needs. It presents results of the program, constraints and contrasts with more conventional restocking approaches. This study shows the potential for asset building approaches …

Activity patterns, spatial use and reproductive hormones of nine adult-female captive manatees at Homosassa Springs Wildlife State Park (HSWSP) were studied from January 6 to August 10, 2003. This study probed two main topics: 1) activity pattern and spatial use of the facility by manatees over three times of day and three seasons (winter, spring and summer), and 2) correlation between manatees behavior and reproductive hormone concentrations collected via fecal samples. Activity patterns and spatial use of the manatees were affected by provisioned food availability over the day and natural vegetation over the study period. Five manatees had estrous cycle …

Although there is not a published opinion addressing same-sex couple adoption in Georgia, it is currently a hot issue in many state courts and, most recently, in the Eleventh Circuit. On January 28, 2004, a unanimous three-judge panel of the Eleventh Circuit Court of Appeals declined to extend the reasoning of Lawrence v. Texas to adoptions by homosexual couples. Specifically, in Lofton v. Secretary of the Department of Children & Family Services, the Eleventh Circuit upheld a Florida statute prohibiting homosexuals from adopting children.

The objective of this study is to determine the effects of elevated UV-B (320 - 290 run) light on Xanthomonas campestris. Xanthomonas campestris is a bacterium that causes a disease commonly known as Black Rot. This disease affects crucifers a large variety of vegetable crops. The disease is easily recognized by the presence of yellow, V-shaped lesions extending inward from the margin of the leaf. The disease progresses, the yellow lesions tum brown and the tissue dies. Looking At The Impact Of Parental Involvement On Student Learning, Carly H. Oliver

This qualitative study focuses on parental involvement in schools, in particular, the over involvement of some parents. Major themes covered include parent-teacher communication, appropriate involvement in and out of the classroom, and parental interference. Research was conducted by surveying two hundred fifty parents and sixty teachers in one elementary school in conjunction with follow-up interviews and the researcher's field notes. The response rates were low and respondents may share many characteristics, skewing the results. Questions on the surveys were categorized into themes to look for trends. The results find that some parents do interfere with the educational process and children …

The Arctic National Wildlife Refuge in Alaska contains an area commonly referred to as the 1002 area. This area contains millions barrels of oil but is currently protected from oil development by Congress. Many groups support opening the 1002 area for development, including the state of Alaska and the Bush administration. Those opposing any potential oil development include the Canadian government and the Gwich'in Indians. Were Congress to open the 1002 area for development, Canada and the Gwich'in would probably take legal action to prevent potential harms associated with the drilling. OBJECTIVE: The aim of the present study was to determine the prevalence of hypertension and cardiovascular disease (CVD) risk factors and its association with the sociodemographic, behavioral, and lifestyle characteristics of the adult qatari population. METHODS: This is a cross-sectional study, which was carried out in primary health care clinics (PHCs). The objective of this study was to determine the effect of Escherichia coli 0157:H7 on feedlot profits. Fecal samples from 711 feedlot pens in 73 feedlots in Nebraska, Kansas, Oklahoma, and Texas were tested for E. coli 0157:H7. Average daily gain and feed-to-gain ratios were computed for each feedlot pen, and managers from each feedlot provided information on various feedlot management practices. Cattle performance and E. coli 0157:H7 prevalence are both affected by feedlot management practices. The indirect effect of E. coli 0157:H7 on potential feedlot profits was determined by measuring the effects of management practices on E. coli 0157:H7 …

Numerous biological and psychological factors associated with impaired neurological functioning have been identified as common among the homeless, but there has been relatively little systematic examination of the cognitive functioning of homeless people. This study explored the neuropsychological functioning of 90 homeless men. There was great variability in their test scores, but the presence of possible cognitive impairment was detected in 80% of the sample. Average general intellectual functioning and reading abilities were found to be relatively low, and the incidence of impairments in reading, new verbal learning, memory, and attention and concentration was high. Objective: To study the trends of cancer esophagus in Karachi South during 1995-2002 and identify risk factors.
Methods: Incident esophageal cancer cases recorded at the Karachi Cancer Registry for Karachi South, during 1st January 1995 to 31st December 2003 were reviewed. For maximum completion of data, incident cases registered from 1st January 1995 to 31st December 2002 were included for final analyses.
Results: The Age Standardized Incidence Rates (ASIRs) of cancer esophagus in Karachi South for males were 6.5/100,000 (1995-1997) and 6.4/100,000 (1998-2002). In females the observed rates were 7.0/100,000 (1995-1997) and 8.6/100,000 (1998-2002).

Gaining Compliance And Losing Weight: The Role Of The Service Provider In Health Care Services, Stephanie Dellande, Mary C. Gilly, John L. Graham

This research provides and empirically tests a conceptualization of health care services in which customer compliance outside of the service organization is necessary for successful health outcomes. Using data from service providers and customers in a weight-loss clinic, the authors examine the provider's role in gaining customer compliance. They find that provider expertise and attitudinal homophily play a role in bringing about customer role clarity, ability, and motivation. This study demonstrates that compliance leads to goal attainment, which results in satisfaction. More important, compliance also leads to satisfaction directly; consumers who comply with program requirements have greater satisfaction with the …

Objective: To look at the clinical presentations, spectrum and site of isolation of the organisms, sensitivity patterns of the organisms and the antibiotic prescribing practices for the treatment of febrile neutropenic patients at our hospital.Methods: The data were collected retrospectively from the records of all neutropenic patients with an absolute neutrophil count (ANC) of less than 500/ml admitted during the period of 3 years from August 1999 to July 2002 at AKUH.Results: Out of the total of 404 patients, 65% had hematological malignancies and around half of them had leukaemia, 86% of the patients presented with fever. Porphyromonas gingivalis, an important periodontal pathogen, infects primary gingival epithelial cells (GECs). Despite the large number of bacteria that replicate inside the GECs, the host cell remains viable. We demonstrate that P. gingivalis triggers rapid and reversible surface phosphatidylserine exposure through a mechanism requiring caspase activation. However, after 1 day of infection, the bacteria no longer induce phosphatidylserine externalization and instead protect infected cells against apoptosis. Infection exerts its effect at the level of mitochondria, as P. gingivalis also blocks depolarization of the mitochondrial transmembrane potential and cytochrome c release. Investing time and energy into survival and reproduction often presents a trade-off to many species of animals. In parasitic wasps, both hosts and sugar sources contribute to the forager’s fitness but are often found in different locations. The decision to search for hosts or for food can have a strong impact on fitness when the forager’s lifetime is short and resources are not abundant. We investigated the tendency of flowers and hosts to attract 1-day-old female Cotesia rubecula Marshall (Hymenoptera: Braconidae) with different feeding histories in a wind tunnel. The polymorphic fungus Candida albicans is one of the most versatile opportunistic pathogens in humans. Many organs of the human body are potential targets for infection by this pathogen, but infection is commonly localized in the gastrointestinal tract, an environment providing anaerobic growth conditions. We describe a chemically defined anaerobic growth medium for four strains of Candida albicans (A72, SC5314, MEN, and 10261). It is a defined liquid glucose-phosphate-proline growth medium supplemented with oleic acid, nicotinic acid, and ammonium chloride. The cells did not require or respond to added ergosterol. OBJECTIVE: The purpose of this study was: 1. To estimate the prevalence of iron depletion and iron deficiency anemia in preschool Arabic children of the United Arab Emirates (UAE), and 2. To determine the risk factors associated with these conditions in this population of children. METHODS: From April through to October 2000 a questionnaire and capillary blood survey was carried out in one primary health care (PHC) centre of Al Ain, UAE.
