Spinal Versus Epidural Anesthesia For Transurethral Resection Of The Prostate, Nabil A. Jayousi

Saudi Medical Journal

OBJECTIVE: To compare spinal versus epidural anesthesia for transurethral resection of the prostate. METHODS: A total of 65 patients undergoing transurethral resection of the prostate between September 1996 and March 1997, from the King Hussein Medical Centre, Amman, Jordan, were enrolled in this study. RESULTS: Epidural anesthesia was successful in 30 patients using an initial dose of 15 ml of 0.5% bupivicaine; whereas spinal anesthesia was successful in 32, using 2.5 ml of 0.5% bupivicaine. Sensory blockade at the level of T8 was similar in both techniques as were hypotension and postoperative hemorrhage. Differences occurred in the degree of motor …

Subcutaneous Panniculitic T Cell Lymphoma Mimicking Histiocytic Cytophagic Panniculitis In A Child, Sulafa K. Ali, Nadir M. Othman, Alexander B. Tagoe, Asma A. Tulba Nov 2000

Subcutaneous Panniculitic T Cell Lymphoma Mimicking Histiocytic Cytophagic Panniculitis In A Child, Sulafa K. Ali, Nadir M. Othman, Alexander B. Tagoe, Asma A. Tulba

Saudi Medical Journal

Subcutaneous panniculitc T-cell lymphoma is a very rare malignancy in the pediatric age group, its association with hemophagocytic syndrome had been described but the association with the skin lesions mimicking histiocytic cytophagic panniculitis which is characteristic of hemophagocytic syndrome has not, to our knowledge, been described in children. We report a child with panniculitic T-cell lymphoma associated with bone marrow hemophagocytosis and subcutaneous histiocytic infiltration with active phagocytosis simulating histiocytic cytophagic. We stress the importance of searching for T-cell lymphoma in patients with panniculitis and hemophagocytic syndrome.

Bronchiolitis Obliterans Organizing Pneumonia Associated With Pseudomonas Aeruginosa Infection, Daad H. Akbar, Omer S. Alamoudi Nov 2000

Bronchiolitis Obliterans Organizing Pneumonia Associated With Pseudomonas Aeruginosa Infection, Daad H. Akbar, Omer S. Alamoudi

Saudi Medical Journal

Bronchiolitis obliterans organizing pneumonia is a rare disease characterized by the presence of granulation tissue within alveolar ducts and alveoli. Most cases are idiopathic, but it may also be seen during resolution of viral or bacterial pneumonia (mycoplasma, legionella and chlamydia). It may present as a community acquired pneumonia which does not respond to antibiotics, which make the diagnosis very difficult. We described a 53-year old patient who presented with Bronchiolitis obliterans organizing pneumonia during the resolution of pseudomonas aeruginosa pneumonia. Initially there was slight improvement on antibiotics but later he became severely hypoxic and placed on mechanical ventilator. Diagnosis …

Gastric Perforation. An Unusual Cause Of Ascites In A Newborn Baby, Omer B. Abdelbasit, Meeralebbae M. Shaheed, Mohammad S. Al-Omari, Abdullah M. Abomelha, Mirghani O. Babiker

Saudi Medical Journal

We report a case of a low birth weight asymmetrical small for gestational age baby, who presented at the age of 20 hours with sudden abdominal distension. Since birth he has been breastfed and was kept with his mother. Absence of radiological findings of necrotizing enterocolitis or perforation at the time of presentation delayed the diagnosis for 48 hours. At laparotomy the baby was found to have perforation of the stomach with no evidence of other gastrointestinal disorder.

The Zen Of Corporate Capital Structure Neutrality, Herwig J. Schlunk Nov 2000

The Zen Of Corporate Capital Structure Neutrality, Herwig J. Schlunk

Vanderbilt Law School Faculty Publications

Given the current tax rate structure - where the marginal tax rate of some persons exceeds the corporate tax rate and the marginal tax rate of others is exceeded by it - corporations are generally well advised to employ both debt and equity in their capital structures. The former will be held by low tax rate taxpayers and will serve to lower the effective aggregate tax rate6 on the corporation's taxable income. The latter will be held by high tax rate taxpayers and will serve to keep low the effective aggregate tax rate on the corporation's unrecognized economic income (such …

Correlation Of A Social Skills Self-Rating Survey And A Teacher Rating Inventory For Students At A Residential School For The Deaf, James Thomas Haley Nov 2000

Correlation Of A Social Skills Self-Rating Survey And A Teacher Rating Inventory For Students At A Residential School For The Deaf, James Thomas Haley

Student Work

Students aged 12 to 15 at a Midwestern residential school for the deaf completed the Matson Evaluation of Social Skills for Deaf and Hard-of-Hearing Youngsters (MESSY- DHH). Faculty at the school completed the Meadow Kendall Social Emotional Assessment Inventory (SEAI) to rate the behavior of the same students. Pearson’s correlations were conducted to determine if relationships existed between scores on the MESSY-DHH and the SEAI. Results indicated inconsistent correlations between scores on the MESSY-DHH and individual scale scores on the SEAI.

The Floodgates Of Strict Liability: Bursting Reservoirs And The Adoption Of Fletcher V. Rylands In The Guided Age, Jed Handelsman Shugerman Nov 2000

The Floodgates Of Strict Liability: Bursting Reservoirs And The Adoption Of Fletcher V. Rylands In The Guided Age, Jed Handelsman Shugerman

Faculty Scholarship

Part I presents an overview of Rylands v. Fletcher and then discusses the phases of the American response: the initial acceptance; the Northeastern rejections in the 1870s, which have been the basis for the erroneous scholarly conclusions; and the overlooked tide of acceptances across the country, beginning in the late 1880s and increasing in the 1890s. Part II places this wave of acceptance in its historical context of changing social forces, although these brief sketches are not the primary emphasis of this Note. First, during a period of rapid urbanization, a small number of courts sought to protect residential areas …

The Constitution Outside The Courts, James E. Fleming Nov 2000

The Constitution Outside The Courts, James E. Fleming

Faculty Scholarship

In this Book Review, Professor Fleming examines Professor Tushnet's arguments against judicial supremacy and in support of making constitutional interpretation less court-centered to pursue a populist constitutional law. The review concedes that Professor Tushnet's arguments that the “thick Constitution”--in particular, its commitments to federalism, states' rights, and separation of powers--is self-enforcing through the political processes are compelling. But it contends that he fails to make the case that the “thin Constitution”--for example, its fundamental guarantees of equality, freedom of expression, and liberty-- should be treated as similarly self-enforcing. Furthermore, Professor Fleming charges that Professor Tushnet does not adequately elaborate how …

Marine Planktonic Archaea Take Up Amino Acids, Cleber C. Ouverney, Jed A. Fuhrman Nov 2000

Marine Planktonic Archaea Take Up Amino Acids, Cleber C. Ouverney, Jed A. Fuhrman

Faculty Publications, Biological Sciences

Archaea are traditionally thought of as “extremophiles,” but recent studies have shown that marine planktonic Archaea make up a surprisingly large percentage of ocean midwater microbial communities, up to 60% of the total prokaryotes. However, the basic physiology and contribution of Archaea to community microbial activity remain unknown. We have studied Archaea from 200-m depths of the northwest Mediterranean Sea and the Pacific Ocean near California, measuring the archaeal activity under simulated natural conditions (8 to 17°C, dark and anaerobic) by means of a method called substrate tracking autoradiography fluorescence in situ hybridization (STARFISH) that simultaneously detects specific cell types …

Critical Reflective Practice And Workplace Learning : Impediments And Possibilities In Teacher Education, Barry Down, Carol Hogan Nov 2000

Critical Reflective Practice And Workplace Learning : Impediments And Possibilities In Teacher Education, Barry Down, Carol Hogan

Australian Journal of Teacher Education

Professional practica are an essential part of teacher education and other professional education programs, but university staff often express concern that prac. experiences are fundamentally conservative, emphasising preparation for the status quo rather than for what might be. In recent years other forms of workplace-based university learning have been devised, where staff have sought to build units of study around a core of reflective practice, action research and professional development. This paper describes one such initiative, a final semester internship for fourth year education students which enabled them to design and negotiate their own professional development plans in any one …

Transference And Psychological-Mindedness In Teachers, Erin Hanifin, Stephen Appel Nov 2000

Transference And Psychological-Mindedness In Teachers, Erin Hanifin, Stephen Appel

Australian Journal of Teacher Education

The article is an argument for the relevance for our understanding of the pedagogic relationship of Freud’s discovery of transference. Commonalities between teaching and psychoanalysis are reviewed prior to a discussion of how the concept of transference might be applied to teaching, particularly to improving the teacher’s ‘psychological-mindedness’. The article concludes by considering the moral/professional issue of teachers making use of the transference processes at work in their classrooms.

The Four Discourses Of Law: A Lacanian Analysis Of Legal Practice And Scholarship, Jeanne L. Schroeder Nov 2000

The Four Discourses Of Law: A Lacanian Analysis Of Legal Practice And Scholarship, Jeanne L. Schroeder

Articles

The article explores the evolving roles of modern lawyers, highlighting the tension between traditional duties like confidentiality and newer disclosure obligations, particularly in securities law. It employs Lacan's discourse theory to analyze legal roles, categorizing them into master, university, analyst, and hysteric discourses. The article argues that understanding these discourses can inform ethical frameworks and professional responsibilities, advocating for a nuanced approach to legal representation.

Spaceflight Effects On Single Skeletal Muscle Fiber Function In The Rhesus Monkey, Robert H. Fitts, Dominique Desplanches, Janell Romatowski, Jeffrey J. Widrick Nov 2000

Spaceflight Effects On Single Skeletal Muscle Fiber Function In The Rhesus Monkey, Robert H. Fitts, Dominique Desplanches, Janell Romatowski, Jeffrey J. Widrick

Biological Sciences Faculty Research and Publications

The purpose of this investigation was to understand how 14 days of weightlessness alters the cellular properties of individual slow- and fast-twitch muscle fibers in the rhesus monkey. The diameter of the soleus (Sol) type I, medial gastrocnemius (MG) type I, and MG type II fibers from the vivarium controls averaged 60 ± 1, 46 ± 2, and 59 ± 2 µm, respectively. Both a control 1-G capsule sit (CS) and spaceflight (SF) significantly reduced the Sol type I fiber diameter (20 and 13%, respectively) and peak force, with the latter declining from 0.48 ± 0.01 to 0.31 ± 0.02 …
