In 1995, China’s State Family Planning Commission (SFPC), the governmental agency charged with developing and implementing China’s population policy, issued an official call for the reorientation of the family planning program from a focus on demographic targets to meeting clients’ needs. In support of this reorientation effort, the SFPC selected six rural counties and five urban districts with comparatively good socioeconomic conditions as pilot sites for a quality-of-care experiment. The material is drawn from a range of national surveys and medical research, as well as information gathered by nongovernmental organizations. Although adolescents make up a quarter of the population of Pakistan, they are still a new subject for research. The characterization of adolescents for this review is individuals ages 10–19, whether or not they are married, sexually active, or parents. This study reports on Bangladesh’s new program model for reproductive health service delivery and people's reactions to it. NGOs in Bangladesh have discontinued door-to-door contraceptive distribution in response to the government’s integrated, clinic-focused approach. The findings from this study strongly support these policy changes: clients and communities are responding favorably to many aspects of the new model, and there do not seem to be intractable social barriers to service utilization. As the NGOs and the Bangladesh government proceed with implementation of the integrated, essential health services model, additional strategies will be needed to erode the paternalistic service delivery culture that …

The African Population and Health Research Centre carried out this study, with support from FRONTIERS and USAID, in order to inform Benin’s Projet Intégré en Santé Familiale (Integrated Project on Family Health—PROSAF) about socio-cultural factors in Borgou that can impede health improvements. More specifically, the study aimed to identify key players in household decisionmaking processes, map out patterns of health-seeking behavior, elucidate how such patterns are associated with prevailing health services utilization, assess community valuation of existing health services and products, and recommend to PROSAF approaches to identified target groups for their intervention. The Population Council studied the acceptability of including sexuality issues in family planning (FP) in Egypt, a conservative society with social restrictions around discussions of sex. The study results showed that sexuality counseling is acceptable to family planning clients in Egypt; in fact, sexuality-related problems and concerns were found to be very common in the study group. Similarly, training family planning service providers on issues of sexuality is both feasible and acceptable to providers. This project, based on previous experiments of the FRONTIERS and POLICY projects, aimed to widen media coverage in Egypt of critical reproductive health issues and to communicate related research findings in a more systematic manner. Increasing coverage in the Egyptian press first required an assessment of the current coverage. Second, a network was formed of about 20 journalists from newspapers and magazines of different publishing houses, followed by four roundtable discussions on critical reproductive health issues. This study was carried out to describe the major NGO community-based distribution (CBD) programs in Ghana, and to assess in some detail the functioning, quality of care, and performance of the Planned Parenthood Association of Ghana (PPAG) program. The study showed that national coverage by the NGO programs is extensive—virtually all the 110 districts in the 10 regions have at least one program. However, coverage within districts is sparse, as each program covers only a few communities. This study’s objective was to improve understanding of Kenyan men’s actual and potential roles as supportive partners in various phases of reproductive health (RH), to help in the design of strategies to encourage men’s greater participation in a variety of RH initiatives in Kenya. The results of the study clearly show that, to a larger extent than anticipated, men in Kenya already participate in women-centered RH services. Overall the institutional barriers seemed to be more overwhelming than the cultural barriers, given that one of the reasons frequently given for nonparticipation was fear of non-acceptance by the health providers. Nearly 70 professionals from the research, policy, service delivery, human rights, and advocacy fields came together to grapple with some of the political aspects of reproductive technologies. The purpose of this paper is to examine the reasons behind Material Services’ acquisitions, the reasons why the Government contested the mergers, and how the Government erred in analyzing the case. We wish to show that the combination of the three coal firms—Material Services, Freeman Coal, and United Electric—occurred for reasons of economic efficiency and practicality, not based on any atavistic desire to monopolize and damage social welfare.

OBJECTIVE: To determine the biochemical parameters of thyroid function in the cord blood of Saudi infants. METHODS: Cord blood samples sent to the Pathology Department for screening for congenital hypothyroidism were used to determine the reference ranges for thyrotropin, free thyroxine, free triiodothyronine, thyroxine-binding globulin and thyroglobulin. All the measurements were carried out by immunoassay (Elisa, microparticle enzyme immunoassay or chemiluminescence immunoassay). Reference ranges were calculated after exclusion of outliers. RESULTS: Reference ranges for thyrotropin, free thyroxine and thyroxine-binding globulin were similar to published values, whereas those for free triiodothyronine and thyroglobulin were different. OBJECTIVE: Clobazam is a newer 1,5-benzodiazepine used for the treatment of epilepsy. It is better tolerated and less sedating than other benzodiazepines. It has yet to gain wide use for epilepsy in the Middle East. Our objective is to report our experience with clobazam for the treatment of childhood epilepsy. METHODS: A cohort of children with intractable epilepsy, defined as recurrent seizures after at least 3 anti-epileptic medication trials, were included prospectively. Clobazam was added to a maximum dose of 2 mg/kg/day. Follow-up by two pediatric neurologists was performed. Therapeutic response was recorded as complete (no seizures), good (>50% …

Five children (3F:2M), in the age group 1 years to 11 years, with Munchausen syndrome by proxy are reported from the Sultanate of Oman. They were seen over a four years period from 1996-1999. In all these children, the mother came up with history of uncontrolled epilepsy. Carbamazepine was the most common antiepileptic drug used. One of these children remained hospitalized elsewhere for nearly 9 months, as a case of uncontrolled status epilepticus. OBJECTIVE: This study reports the results of a questionnaire design to elicit doctors views about epilepsy. METHODS: Sixty-two percent of medical staff working in different regions of Oman responded. The questionnaire covered certain topics regarding the source of the knowledge of the doctors on seizure disorders and the personalities and behavior of people with epilepsy. RESULTS: The study suggests that although doctors in Oman gained knowledge on epilepsy prior to medical education, more doctors judged people with epilepsy ‘negatively’ compared to ‘positively’ for normal people. OBJECTIVE: To collect basic data on multiple and diverse issues such as drugs of dependence, smoking, initiation age, injection related harm, nutritional status, trauma, accidents, abnormal laboratory parameters and co-morbid conditions. METHODS: Seven hundred and ninety nine subjects from a voluntary detoxification unit were studied. Four hundred and ninety seven charts from June 1995 to December 1995 were reviewed retrospectively. For the other 302 subjects admitted between September 1996 to December 1996, information regarding drug use, smoking, trauma, accidents and co-morbid conditions was recorded during the admission interview while the remaining data was later obtained from the charts. The goal of this article is to review childhood enuresis, which is a common behavioral condition reported in millions of children worldwide. An online computer search was made, and the literature up to 1997 was screened in order to include relevant data for this review. The prevalence of childhood enuresis varies across reviewed studies. This was attributed to a variety of sociodemographic and cultural dynamics of societies. Similarly, the pathophysiology of this ubiquitous symptom is determined by multiple risk factors including biological, psychological, social, and cultural, but the developmental/maturational delay influenced by genetics remains the most plausible explanation. The present decline in per capita expenditure on health in Saudi Arabia requires private funding to reduce pressure of health expenditure on the government budget. User charges would be an important source of revenue in the Kingdom where services cannot be cut and taxes are not imposed. OBJECTIVE: There is paucity of information on the driving pattern of epileptic patients in Saudi Arabia. The purpose of this study was to document the frequency of epileptic patients that drive and the factors associated with this. METHODS: A Hospital-based study with structured interviews of consecutively recruited patients who gave prior consent. Seizure types, extent of control and factors presumed to be associated with driving were documented. RESULTS: One hundred and thirty eight male epileptics (mean age: 32.7 years) were studied. METHODS: A prospective study of Saudi diabetics attending King Abdulaziz University Hospital out patient clinic from January 1998 until April 1999. Detailed information of each patients’ age, sex, body mass index, type and duration of diabetes mellitus, mode of treatment, degree of blood glucose control, presence of hypertension, hyperlipidemia, smoking, family history of diabetes mellitus and hypertension were recorded. Six patients with cerebral tuberculoma seen over a 2 year period are reported. Their clinical presentations, the result of investigations including radiological findings and their response to medical treatment are discussed and correlated with other reports in the literature. Eighty five percent of the patients were above the age of 30 years. All patients had negative past history of tuberculosis and both chest x-ray and erythrocyte sedimentation rate were within normal limits. Five patients presented with symptoms and signs of space occupying lesions but none had papilloedema.
