Transportation infrastructure—including buried utilities, drainage systems, and culverts—is frequently subjected to significant external stresses arising from existing conditions and new construction, especially in urban environments where deformation and failures can lead to significant operational and safety consequences. Lightweight fills offer a viable strategy to mitigate these stresses by reducing imposed loads. Freight and rail networks across the United States face mounting risks from weather-related hazards and other systemic disruptions that threaten operational continuity, economic stability, and community well-being. While state freight plans are federally mandated, local and metropolitan freight planning varies widely, leaving gaps in how resilience is addressed across scales. This study examines how resilience is being integrated into freight and rail planning through a two-part approach: (1) a content analysis of 43 recent state and metropolitan freight plans, and (2) a nationwide survey of 56 transportation professionals engaged in freight and rail planning. This article explores how the visceral acts of being bitten by and killing mosquitoes can underpin the formation of ecological citizenship. This form of citizenship is premised on human-nature entanglement and raises questions concerning how the rights and responsibilities stemming from such entanglement become distributed across the public and private domains. As a space in which mosquito cohabitation is contested daily, the home is a key site through which ecological citizenship can be forged and theoretical debates concerning urban political ecology can be extended. Isolated lesser trochanter (LT) fractures are uncommon in adults and may be the first clue to an underlying pathologic process. In contrast to adolescent apophyseal avulsions, the same finding in a skeletally mature patient has been linked to metastatic disease, primary bone tumors, hematologic malignancy, infection, and, less often, benign traumatic, stress-related, osteoporotic, or metabolic causes. This review examined the malignant, infectious, and benign etiologies, diagnostic pathways, treatments, and outcomes that have been reported in adults aged 18 years or older presenting with a radiographically isolated LT fracture or avulsion fracture. Coagulation factor IX (FIX) is a serine protease that is crucial for normal hemostasis in vertebrates. Unlike other soluble vitamin K-dependent procoagulant enzymes, there is an “extravascular reservoir” of FIX in various tissues. The extravasation of FIX and its subsequent maintenance outside of circulation have long been attributed to its ability to bind collagen IV. This property is conferred by a short, specialized region of its phospholipid-binding gamma-carboxyglutamic acid (Gla) domain. Collagen IV is a nonfibrillar extracellular protein found primarily in basement membranes that assembles into sheet-like supportive structures around blood vessels and epithelia, providing a convenient scaffold for this …

Background and objective: The global rise in the incidence of diabetes mellitus necessitates innovative management strategies to mitigate the severity of the disease. The mainstay of diabetes treatment is still insulin therapy, although there are drawbacks, such as the need to maintain ideal glycemic control while lowering the risk of hypoglycemia and user fatigue. The development, clinical usefulness, and constraints of insulin delivery systems are examined in this narrative review, with particular attention to sensor-augmented and closed-loop technologies.

The global neurosurgical literature is dominated by high-income countries, with low- and middle-income countries (LMICs) contributing minimally despite bearing the most significant share of the neurosurgical disease burden. Underrepresentation limits equitable knowledge dissemination and the relevance of guidelines. The objective of this review was to assess neurosurgical publication disparities, identify deterrents to LMIC participation, and outline initiatives that foster equity and inclusion in research. This narrative review synthesized evidence on global disparities in neurosurgical literature through a structured search of PubMed, Scopus, and Google Scholar (2010–2025). Background: Lung adenocarcinoma (LUAD) is the most prevalent histologic subtype of non-small cell lung cancer (NSCLC) and exhibits considerable molecular heterogeneity. Among the most clinically significant driver alterations are mutations in EGFR and KRAS, both of which influence treatment selection and oncologic outcomes. The prevalence of these mutations varies by race, yet racial minority populations remain underrepresented in genomic studies. EGFR alterations are more frequently observed in Asian patients, while KRAS mutations predominate in non-Asian cohorts. The continuous identification of top-k maximal sum intervals using a sliding window over a data stream is a critical operation for applications in IoT and beyond. A maximal sum interval is a non-overlapping, contiguous subsequence with the maximal sum in a sequence of signed values. Existing algorithms are ill-suited for streaming contexts: they either exhaustively enumerate all intervals even for small k values, or depend on indexes that require frequent and costly restructuring. Multi-behavior recommendation improves target-behavior predic-tion by exploiting heterogeneous auxiliary feedback (e.g., view,collect, and cart), yet its robustness is often undermined by behavior-dependent noise and inconsistency. We argue that the key bottle-neck is not merely noisy behaviors, but a representation-level failurecaused by two coupled heterogeneities. First, intra-behavior rep-resentation entanglement arises when multi-hop propagationblends incidental signals with true preferences in the embeddingspace. This entanglement renders coarse spatial denoising inef-fective, since it cannot suppress noise without sacrificing weak-but-informative niche signals. Motivated by the observation that users in the real world often engage across multiple social networks simultaneously, we study the problem of influence maximization in multilayer social networks (Mlim), aiming to select a small set of nodes that maximizes the total influence spread across all layers. To this end, we introduce a hybrid propagation model that jointly captures layer-specific diffusion dynamics and probabilistic cross-layer propagation. Based on this model, we formally define the Mlim problem and establish its NP-hardness, monotonicity, and submodularity. Low-Light Image Enhancement (LLIE) aims to recover visually pleasing content and details from degraded low-light images. However, existing RGB-based methods often suffer from color bias and brightness artifacts due to inherent high color sensitivity. Although the HSV color space can decouple brightness and color, it introduces noticeable red and black noise artifacts. To address these challenges, we adopt the Horizontal/Vertical-Intensity (HVI) color space for LLIE, which is defined by the HV color map and learnable intensity. Speech synthesis systems can now produce highly realistic vocalisations that pose significant authenticity challenges. Despite substantial progress in deepfake detection models, their real-world effectiveness is often undermined by evolving distribution shifts between training and test data, driven by the complexity of human speech and the rapid evolution of synthesis systems. Existing datasets suffer from limited real speech diversity, insufficient coverage of recent synthesis systems, and heterogeneous mixtures of deepfake sources, which hinder systematic evaluation and open-world model training. As a fundamental data mining task, unsupervised time series anomaly detection (TSAD) aims to build a model for identifying abnormal timestamps without assuming the availability of annotations. A key challenge in unsupervised TSAD is that many anomalies are too subtle to exhibit detectable deviation in any single view (e.g., time domain), and instead manifest as inconsistencies across multiple views like time, frequency, and a mixture of resolutions. Current time series foundation models (TSFMs) primarily focus on learning prevalent and regular patterns within a predefined time or frequency domain to enable supervised downstream tasks (\eg, forecasting). Consequently, they are often ineffective for inherently unsupervised downstream tasks—such as time series anomaly detection (TSAD), which aims to identify rare, irregular patterns. Adverse haze conditions introduce complex degradations that obscure scene details and distort structural cues critical for object detection, posing persistent challenges for vision‐based sensing systems. Although existing haze removal methods have achieved notable improvements in visual clarity, their optimisation objectives are often misaligned with downstream detection requirements, leading to limited detection performance in real‐world scenarios. In Australia, despite overall declines in cardiovascular disease (CVD)-related mortality, Aboriginal and Torres Strait Islander women develop heart disease earlier and experience more than twice the rate of related morbidity and mortality compared with non-Indigenous women. This study explores Aboriginal women’s understandings of the factors that shape their heart health and lived experiences of CVD. We spoke with 28 Aboriginal women aged ≥18 years—15 semistructured interviews and two yarning circles with an additional 13 Aboriginal women—with/without pre-existing CVD, and 10 healthcare professionals (HCPs) working with Aboriginal communities in the Perth metropolitan region, between May 2024 and April 2025. Purpose: Persistent motor dysfunction after burn injury may be due to altered motor cortex and corticospinal tract function. This study examined manual dexterity, motor cortex neuroplasticity, and intracortical inhibition in former burn patients 1–3 years after minor injury (<10% TBSA). Method: Thirty former burn patients (TBSA: 0.78 ± 1.08%) and 30 non-injured controls participated. Manual dexterity was assessed using the Purdue Pegboard. Transcranial magnetic stimulation (TMS) was used to measure motor cortex excitability (motor evoked potential [MEP] amplitude) and short- and long-interval intracortical inhibition (SICI, LICI) before and after paired-associative stimulation (PAS) to induce neuroplasticity. Results: Former burn patients performed worse than controls on the bimanual assembly subtest of the Purdue Pegboard. PAS did not induce changes in MEP amplitude but increased LICI in both burn patients and control participants. In burn patients, baseline LICI was related to bimanual performance differently than controls. Conclusions: These findings suggest poorer bimanual motor function persists after minor burns, and differences in the association between inhibition and bimanual dexterity between burn and control groups. Further research should explore whether targeting motor cortex inhibitory circuits can improve motor performance following burn injury

This experimental study investigates the thermal-hydraulic performance of a parabolic trough collector (PTC) equipped with propeller-shaped turbulators, which were 3D printed and installed along the absorber tube to enhance convective heat-transfer by increasing flow mixing. Nine turbulator configurations were tested by varying the blade angle (β = 22.5° and 45°, and β = 67.5°) and the number of turbulators (Nt = 6, 12, and Nt = 18) over a Reynolds number range of 1000 to 6000. Musicians demonstrate advantages in acquiring motor sequences, showing faster learning and better explicit sequence knowledge than non-musicians. However, it is unclear whether this advantage extends beyond acquisition to the consolidation phase, which is when newly learned skills stabilize and become resistant to interference. Additionally, while interference from executing competing motor tasks is well-established, less is known about whether purely sensory information presented after learning can disrupt consolidation of a bimodal motor sequence. We investigated how post-acquisition sensory interference affects performance of a learned audio-visual sequence, and whether musical training moderates this vulnerability. Background: Ultrasound-based surveillance for HCC is limited by suboptimal sensitivity and low adherence. Blood-based biomarkers are potential alternatives for routine HCC surveillance. We aimed to define optimal sensitivity, specificity, and cost parameters under which blood-based biomarkers could serve as cost-effective alternatives in patients with cirrhosis.
Methods: We developed a microsimulation model of the natural history of HCC in individuals with compensated cirrhosis. We compared the cost-effectiveness of biannual surveillance using 2 blood-based biomarkers-HES V2.0 and GALAD-versus ultrasound-based surveillance across a range of sensitivity, specificity, test costs, and adherence levels. Background: Eosinophilic gastritis currently has no approved treatments and is postulated to be driven by type 2 inflammation. Dupilumab blocks type 2 cytokines IL-4 and IL-13 and has efficacy in multiple diseases characterised by type 2 inflammation, including eosinophilic oesophagitis. We aimed to assess the efficacy and safety of dupilumab in patients with eosinophilic gastritis.
Methods: DEGAS was a proof-of-concept, phase 2, multicentre, randomised controlled trial consisting of a 12-week, double-blind, placebo-controlled period, followed by a 24-week open-label extension period. Large-scale multiancestry genome-wide association studies have identified hundreds of loci associated with type 2 diabetes (T2D) and glycemic traits, yet imputed genotyping arrays limit the detection of low-frequency and rare variants. Whole-genome sequencing (WGS) offers a more complete view of genetic variation, especially across diverse populations. We analyzed high-coverage (38×) WGS data from 21,913 T2D case subjects, 61,036 control subjects, and up to 50,011 individuals with no diabetes with fasting glucose, fasting insulin, and HbA1c from the National Heart, Lung, and Blood Institute Trans-Omics for Precision Medicine Program. Objective To identify the barriers and facilitators that influence the dissemination of clinical practice guidelines (CPGs) and to explore the perceived usefulness, practical utility, expectations, and unmet needs of end users in relation to current dissemination strategies. Methods This qualitative evidence synthesis (QES) is a part of mixed-methods evidence synthesis.
