This study explored the traits and health state of African Americans in central Virginia in order to determine what traits put people at a higher probability of being diagnosed with diabetes. We also want to know which traits will generate the highest probability a person will be diagnosed with diabetes. Traits that were included and used in this study were cholesterol, stabilized glucose, high density lipoprotein levels, age(years), gender, height(inches), weight(pounds), systolic blood pressure, diastolic blood pressure, waist size(inches), and hip size(inches). There are many new medications for treatment of type 2 diabetes and these medications have many positive effects that go beyond lowering blood glucose. However, many of these medications are expensive and some insurance plans don’t cover them. Many patients struggle to pay for their diabetes medications and providers often are unsure of which medications will be covered by insurance. This project was designed to enhance patient education regarding diabetes medications and potential costs associated.

Diabetic retinopathy (DR) is a leading cause of blindness in adults. Early detection and subsequent treatment of DR has been shown to be effective at saving vision and medical costs. However, adherence to annual dilated eye exams is relatively poor among people with diabetes. This project in Waterbury, VT aimed to identify patients' potential knowledge gaps about the connection between diabetes and eye disease, educate patients about DR with a 2 min educational video in the primary care office, and then assess their level of interest in getting an eye exam after watching the video.

Exercise is a critical aspect of health maintenance, especially in the context of chronic medical problems such as diabetes, heart disease, and obesity. Weather and climate conditions in Vermont directly impact Vermonters' ability to exercise, and this results in negative impacts in their health maintenance and healthcare costs nationally and locally. This project explores the direct and indirect impact of decreased exercise and explore a modality for informing patients on how to stay active safely in the winter.

According to the Vermont Department of Health physical inactivity is one of the three main behaviors that predisposes patients to cancer, heart disease/stroke, lung disease, and type 2 diabetes, which result in over 50 percent of deaths among Vermonters. Washington County has a higher rate of overweight adults than the statewide average, making exercise an important component to healthier living. Barriers to exercise, especially among patients with diabetes, impose serious health ramifications for the residents of Washington County, and avenues must be made available to circumvent these barriers.

Many senior citizens in the Vergennes community are active in the spring, summer, and fall with various activities, community groups, and walking. However, in the winter with colder temperatures and less sunlight, these activities are difficult to maintain, even more exacerbated by the current Covid-19 pandemic. Encouraging maintenance of physical activity throughout the winter months in Vermont is difficult, but can provide many benefits to these individuals both physically and mentally.

An estimated 80% of Americans consume more than the recommended daily amount of added sugar. Added sugar consumption significantly increases risk for cardiovascular disease mortality, and is associated with body weight, type 2 diabetes, and metabolic syndrome. In the United States, added sugar consumption contributes to public health costs related to cardiovascular disease ($555 billion spent in 2016) and diabetes ($245 billion spent in 2012). Particularly in New York, nearly 40% of all New York deaths in 2014 could be attributed to cardiovascular disease and 10.5% of New York adults had diabetes in 2017.
