This capstone presentation depicts the impact of diabetic management with an occupational therapy approach on upper extremity motor function for the adult stroke population. This program utilizes dietary management, a home exercise program, and task modification strategies to improve ADL and IADL performance.

Diabetes is a chronic metabolic disease characterized by an inability of beta cells to produce or secrete insulin due to decreasing beta cell mass, a condition induced by beta cell death or overuse. Current treatment consists of daily administration of insulin to diabetic individuals. We have shown that Sirtuin 7 (Sirt7), a deacetylase located in the nucleus, directly interacts with Nkx6.1, a transcription factor essential for beta cell function and proliferation. We have shown that one of the post translational modifications that impinges on Nkx6.1 activity is acetylation. Given Sirt7’s role as a deacetylase, and published reports demonstrating its impact …

The pathogenesis of type 1 (T1D) and type 2 diabetes (T2D) is associated with progressive beta-cell death and loss of insulin secretion, making functional betacell mass preservation a priority in the treatment of diabetes. T1D beta-cell death is primarily mediated by autoimmune exposure of beta-cells to proinflammatory cytokines such as IL-1b, TNFa, and IFNg (1, 2). Beta-cell loss in T2D, however, is characterized by damage caused by a destructive metabolic state of persistent hyperglycemia and elevated free fatty acid (FFA) levels (2, 3). Grace Cottage Family Health & Hospital identified diabetes as a level one priority for community health improvement. VeggieVanGo is a program hosted at schools and hospitals hosted by the Vermont Foodbank which provides fresh produce to those in needs. Lifestyles modifications are essential for diabetes management including consuming fresh produce. This project sought to increase awareness of the role of diet in diabetes management for patients at Grace Cottage Hospital who may use the VeggieVanGo program.

Hispanic adults are 70 percent more likely than non-Hispanic white adults to be diagnosed with diabetes by a physician. In 2017, Hispanics were twice as likely to be hospitalized for treatment of end-stage renal disease related to diabetes, as compared to non-Hispanic whites. In 2018, Hispanics were 1.3 times more likely than non-Hispanic whites to die from diabetes. Lack of readily accessible educational material in different languages can lead to educational deficits in non-English speaking patients.

For patients with diabetes, screening for microvascular complications like retinopathy requires annual retinal photography or dilated fundoscopy to identify and mitigate processes that can cause preventable blindness. Yet, by measures at the state and federal level, the percent of patients achieving this care is below target. At the Community Health Centers of Burlington (CHCB), the barriers responsible for this include uncertainty among patients and providers about which local optometry offices accept Medicaid, are scheduling new patients and have interpreter services available. Food insecurity is a widespread issue plaguing roughly 12.3% of households in the United States, which is a total of roughly 15.6 million families. It is defined as the limited or uncertain access to adequate and nutritious food. At times, it is struggling to find any food at all and wondering when your next meal will be. Food insecure areas tend to have less supermarkets and convenience stores, resulting in lower availability of healthy food options like fresh fruits, vegetables, whole grains, and more.
