Hospital readmissions for Medicare beneficiaries with congestive heart failure (CHF) and chronic obstructive pulmonary disease (COPD) contribute to increased healthcare costs and adverse outcomes. Evidence-based interventions, such as discharge planning, multidisciplinary care coordination, and patient education, have been implemented to reduce ambulatory care-sensitive admissions (ACSA), yet challenges in care continuity persist. This integrative review examined strategies adopted by healthcare organizations to reduce ACSA among Medicare beneficiaries with CHF and COPD. This doctoral project aimed to develop clinical practice guidelines (CPG) to strengthen the assessment of poststroke patients for rehabilitation. The project sought to adopt an evidence-based approach to develop standardized recommendations that can help in the timely and appropriate rehabilitation of stroke survivors, thereby reducing the risk of long-term disability and readmissions. Mental health stigma, often resulting in misdiagnosis, delayed treatment, and decreased patient engagement, represents a significant barrier to effective nursing practice. This study was conducted to examine racial disparities in media portrayals of filicide cases involving African American and Caucasian women, exploring how systemic biases and societal narratives influenced public perception, policy, and the child welfare and legal systems. Using a qualitative content analysis of 42 cases involving African American women and 20 involving Caucasian women, framing techniques, language, and imagery were analyzed. The underuse of standardized mental health screening tools, such as the Patient Health Questionnaire-9 (PHQ-9), delays the diagnosis of major depressive disorder (MDD) in pediatric patients. This staff education project aimed to improve psychiatric providers' knowledge, confidence, and PHQ-9 utilization through an interactive training program. Over four weeks, three nurse practitioners and two medical assistants participated in an evidence-based educational intervention. Pre- and post-training assessments measured provider knowledge, confidence, and PHQ-9 usage through surveys and electronic health record (EHR) audits. Patient-centered care has become increasingly paramount in healthcare organizations (Thummak et al., 2023). A critical component of this model is patient adherence to scheduled appointments, as missed visits can negatively affect outcomes (Halladay et al., 2019), increase healthcare costs, waste resources, and disrupt workflow (Low et al., 2023). This qualitative case study investigated the factors contributing to patient nonadherence to scheduled visits in a multidisciplinary behavioral health organization (BHO) in the Southwestern United States.
