The Patient Self-Determination Act passed in 1991 requires staff in health care facilities, such as skilled nursing facilities, hospitals, home health services and primary care offices to ask patients about advanced directives. Despite this, actual completions of advance directives in America remain low. To determine the barriers and facilitators to the completion of advance directives in healthy, non-health care employed individuals aged 45-65. It is believed healthy individuals in this age group both desire the advance directive, and have strong opinions regarding their end of life preferences.

Patient choice for caesarean delivery (CD) is complex. This choice poses a challenge to Health Care Providers (HCPs) in terms of resource allocation, economics, and surgical risks. Yet, women's understanding of choice is poorly understood. This study answers the question: how HCPs might understand women's choice for CD? Four primiparous women who chose a CD were recruited. Widespread human transmission of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) also presents a new set of challenges when considering viral spillover from people to naïve wildlife and other animal populations. This replication and expansion of Mason's work on seclusion used an ethnomethodological approach to examine the decision-making process of five Canadian registered nurses. Each nurse read two patient vignettes and then verbally explained whether they would use seclusion and why. The interview data were analyzed in structural, languaing, first-level and second-level analyses. Although all subjects arrived at different decisions, they structured and languaged their responses similarly. The first-level analysis indicated the subjects believed that experience was the most significant factor in their decision-making. The Pacific oyster, Crassostrea gigas, is currently in the relatively early stages of invasion in San Diego estuaries. Aim: The primary aim of this thesis was to investigate the relationship between nutrition and frailty in Irish older adults. The nutritional status, dietary quality and perceived barriers/motivators to nutrition were also examined, and a frailty intervention protocol was designed. Methods: Cross-sectional study in Irish community-dwelling adults aged ≥65 years. Failure of beef carcasses to achieve desirable carcass specifications represent inefficiencies within the supply chain, namely greater carcass processing costs and the inability of the resulting primal cuts to conform to high-value market specifications. Analysis of a representative sample of prime Irish beef cattle conducted in this thesis determined that 59% of cattle fail to achieve the desired carcass specifications of the supply chain at slaughter. Drug-resistant infections pose serious medical threats, and causative resistance genes spread through plasmid conjugation. The TraT protein is a virulence factor encoded in drug-resistance F-plasmids. TraT itself provides protection from conjugation, serum complement, and bacteriophages, but the protective mechanism is unknown. To facilitate mechanism studies, I developed improved purification protocols for isolation of the soluble domain of F-plasmid TraT from Escherichia coli. Wild-type TraT and two variants were obtained. TraT domain forms several types of oligomers in vitro, also observed in full-length TraT. This oligomerization is stable without membrane attachment and in broad range of salt concentrations. Purified …

Understanding how environmental racism uniquely affects marginalized communities is crucial to effectively develop public policies that will address the systematic racism that is rooted in many existing policies and practices. The town of Mossville, Louisiana provides a case study of a Black town that experienced devastating environmental pollution as well as displacement from the oil and petrochemical industries in the region (Rogers, 2015), with few residents still in the area.
