Saudi Medical Journal
Sickle cell disease (SCD) is an inherited hematological disorder with multiple-organ involvement. The pulmonary complications of SCD are associated with significant morbidity and mortality. This article presents an important review of acute and chronic pulmonary complications, including acute chest syndrome, pneumonia, pulmonary thromboembolism, pulmonary fat embolism, chronic sickle cell lung disease, and pulmonary hypertension, in patients with SCD. Bronchial asthma and obstructive sleep apnea in relation to SCD are discussed in this article. Early recognition of pulmonary complications leads to early therapeutic interventions and improvement of the overall treatment outcome.

Effect Of Gut Microbiota On Colorectal Cancer Progression And Treatment, Glowi A. Alasiri
Saudi Medical Journal
Microbiota is a collection of bacteria, archaea, eukaryotes, bacteriophages, viruses, and fungi that cover human body surfaces and cavities. They characterize inside the body due to several factors such as diet, nutrition, xenobiotic substances, and microbial infections. Several studies have shown that gut microbiota can induce resistance against pathogens and regulate the immune system. In addition, their disruption is associated with several physiological and biochemical disorders, including inflammatory bowel disease (IBD), obesity, autoimmune diseases such as diabetes, hypertension, colon cancer, and cardiovascular disease. Colorectal cancer (CRC) is the third-deadliest cancer worldwide, accounting for approximately 900,000 deaths per year globally. Gut …

Saudi Medical Journal
Objectives: To evaluate the demographics, clinical presentation, laboratory data, chest radiographs, and outcomes of pediatric patients with critical coronavirus disease 2019 (COVID-19). Methods: This retrospective study included 34 children who were diagnosed with severe COVID-19 pneumonia between August 2020 and July 2021. Severe pneumonia was defined as fever, respiratory distress (tachypnea, chest retractions, and hypoxia [oxygen saturation <90% in room air]), and obvious infiltrations on chest radiography. Results: Ages of the patients ranged from newborns to 12 years old, with a median of 24 months (interquartile range: 12-72 months). Preschool-aged children were the most common age group (44%). Levels of inflammatory markers (C-reactive protein, ferritin, and procalcitonin) were elevated in most patients. A total of 13 patients developed severe acute respiratory distress syndrome (ARDS), while 4 developed multiorgan failure. Despite receiving supportive therapy, 2 (5.9%) patients died due to severe septic shock and multiorgan failure. One deceased patient was born prematurely at 30 weeks, while the other had chronic granulomatous disease. Conclusion: This study described a single-center cohort of pediatric patients with severe COVID-19 pneumonia. In this cohort, children with cardiopulmonary comorbidities and ARDS had a high mortality and long-term morbidity, as observed in other pediatric studies.

An Overview Of Pathophysiology And Treatment Options Of Osteonecrosis Of Femoral Head In Sickle Cell Disease, Mohammed L. Al-Otaibi
Saudi Medical Journal
Osteonecrosis of the femoral head (ONFH) is the most prevalent musculoskeletal pathologic manifestation of sickle cell disease (SCD) resulting in an osteonecrotic event. This review aimed to summarize mechanisms involved in pathophysiology of ONFH and treatment options available in Saudi Arabia to treat SCD patients with complication of osteonecrotic event. The pathophysiology of ONFH include genetic and micro particles involvement. The progression of osteonecrosis involves reduced levels of bioactive compounds in peripheral blood mononuclear cells and elevated CD4+T circulating levels to stimulate pro-inflammatory cytokines contributing to inflammation at target site. Initial treatment approach is pharmacological agents use to mitigate risk. …
