In addition, the complement fixing capability of circulating IgG subclasses was also investigated. METHODS: Seventeen skin biopsies and 25 serum samples obtained from 25 cases of bullous pemphigoid were analyzed by direct and indirect immunofluorescence using mice anti-human IgG subclasses monoclonal antibodies. Indirect complement immunofluorescence was used to measure the avidity with which antibodies activate the complement. The study was carried out between 1992 and 1996 at the King Fahd Hospital of the …

The separation was performed at the age of 4 months. The resulting chest wall defects in both twins were covered by dacron-reinforced silastic prosthesis. The abdominal wall defect was primarily closed in the "left twin," and covered with a dacron reinforced silastic sheet in the "right". Primary skin closure was achieved in both, following extensive mobilization of the …

Travel Epidemiology. Who Perspective, Hussein A. Gezairy
Saudi Medical Journal
International travel is undertaken by large, and ever increasing numbers of people for professional, social, recreational and humanitarian purposes, and this upward trend looks set to continue. Travelers are exposed to a variety of health risks of the unfamiliar environment of the visited area. The risk of exposure is determined by destination, duration of the visit, purpose of the visit, behavior of the traveler and underlying physical and biological status of the traveler. However, most risks can be minimized by suitable precautions taken before, during and after travel. Travelers can themselves, also, easily introduce any infectious agents they may harbor …

Traumatic Fat Embolism Syndrome, Tarig S. Al-Khuwaitir, Abdurahman M. Al-Moghairi, Suphia M. Sherbeeni, Hamed M. Subh
Saudi Medical Journal
Traumatic fat embolism syndrome occurs most often following fractures of long bones sustained in road traffic accidents and is a common cause of medical consultation from the orthopedic surgery department. The sub-clinical presentation is subtle and expresses itself by the presence of hypoxemia, while the full clinical syndrome compromises respiratory insufficiency, an altered consciousness and a characteristic petechial rash. Recognition is simple once the patient is viewed in the context of his or her clinical setting. Diagnosis is aided further by the presence of hematological and biochemical abnormalities including anemia, thrombocytopenia, an elevated erythrocyte sedimentation rate and fat macroglobulinemia. Imaging …

Single Coronary Artery Arising From The Left Sinus Of Valsalva, Ayfer Mavi, Alper Sercelik, Resat Ayalp, Erdem Gumusburun, Talantbek Batyraliev
Saudi Medical Journal
A single coronary artery is a rare congenital anomaly of the coronary arteries where only one coronary artery arises from the aortic trunk by a single coronary ostium, supplying the entire heart. We report a case of a 57-year-old woman with atypical chest pain, in whom coronary angiography showed a L-I subtype single coronary artery (arising from a single ostium in the left sinus of valsalva) without associated cardiovascular disease. The clinical significance and subtype of the single coronary artery are discussed.

Diabetes mellitus and cystic fibrosis (CF) have been reported before in the literature, but they have never been reported in the same patient in the Middle East. We present the first reported case of insulin dependent diabetes mellitus (IDDM) and CF in 2 siblings of the same family. Both siblings were diagnosed early in life with IDDM, and their diabetes was well controlled on insulin. Cystic fibrosis was diagnosed in the first case one year after IDDM was diagnosed due to history of chronic cough and in the 2nd case by family screening. Both had severe failure to thrive, recurrent …

Complex congenital anomalies of the mullerian ducts can occur in isolation or in association with other developmental disorders. They result from non-development or non-fusion of the mullerian ducts or the failure of reabsorption of the uterine septum. Early diagnosis is necessary to relieve symptoms, optimize preservation of the genital organs and prevent the development of endometriosis. We present a case report to highlight this phenomenon.

Saudi Medical Journal
The operative treatment of tuberculous spondylitis remains a challenge with regard to the surgical approach to the cervicothoracic junction. In addition, it is difficult to restore the resected vertebral bodies. Two cases of tuberculous spondylitis in this area are presented. The first case concerns a 25-year-old African patient suffering from the effects of tuberculous spondylitis: Destruction of vertebral bodies dorsal (D)1, D2, and D3, kyphosis coupled with compression of the spinal cord, as well as incomplete motor and sensory paraplegia (Frankel grade C). The operative treatment of tuberculous spondylitis that is described, approached the cervicothoracic junction by means of a …

Current Surgical Treatment Of Parkinsons Disease, Sohail A. Ansari, Antoine Nachanakian, Nabil M. Biary
Saudi Medical Journal
There has been a renaissance in the surgical management of Parkinson9s disease. This has been due to long-term effects of levodopa and a better understanding of the basal ganglia and its circuitry. Ablative surgery and neurostimulation are the only realistic surgical options at present. Although surgical treatments, such as ablation and stimulation are effective, they are not useful for stopping the progression or restoring the system. Neural transplantation helps restore the system by using a number of techniques. Targets mostly used are in the thalamus, globus pallidus and subthalamic nucleus. A number of factors must be considered including patient9s age, …
