OBJECTIVE: Although the theoretical attractions of the elevated rim are obvious and have been widely accepted as a mean to improve the postoperative stability, the clinical advantages have not been demonstrated. The aim of this study is to further evaluate the elevated liners contribution to stability. METHODS: Forty-six patients with 50 hips undergoing primary total hip arthroplasty (THA) were enrolled in this study, conducted in Rush Hospital, Chicago, Illinois, United State of America, between March 2001 and February 2003. We tried to determine the amount of additional stability that can be provided by elevated-rim liner compared to the non-elevated liners …

Amlodipine Associated Hyperpigmentation, Zulal Erbagci
Amlodipine Associated Hyperpigmentation, Zulal Erbagci
Saudi Medical Journal
Amlodipine is a potent peripheral and coronary vasodilator with high selectivity for vascular smooth muscle, and is widely used in mild to moderate hypertension, chronic stable angina and vasospastic angina. Its most prevalent side effects are peripheral edema, flushing and headache. Cutaneous adverse reactions associated with amlodipine have been rarely reported. Herein, a male patient is described to develop oral mucosal and cutaneous hyperpigmentation one year after starting amlodipine, which became more noticeable with time. Although cutaneous hyperpigmentation was most prominent on the photoexposed areas, there was no history of previous photosensitivity, pruritus or flushing. To our knowledge, no case …

Ambulatory Blood Pressure Monitoring. A Pediatric Review, Aftab S. Chishti
Saudi Medical Journal
Hypertension is one of the major contributors to cardiovascular, renal and central nervous system morbidity and mortality. Although it is more prevalent in the adult population, hypertension and its sequelae are being seen in the pediatric population with increasing frequency. Blood pressure (BP) is not a static phenomenon. It is highly variable, changing constantly in response to various activities, stimuli, and stresses. Consideration of all of these factors make intermittent clinic BP measurements less effective in accounting for BP rhythmicity so ambulatory BP monitoring has emerged as a useful tool to give a detailed analysis of BP patterns during the …

Non-Langerhans Cell Histiocytoses Of The Skin, Naseem A. Ansari, Jihad T. Al-Ratrout
Saudi Medical Journal
Histiocytic proliferations may be reactive or neoplastic. The neoplastic proliferations (histiocytoses) are subdivided into a few categories. One of these categories is the non-Langerhan9s cell histiocytoses (NLCH). Cutaneous NLCH is a heterogeneous collection of conditions which can be sub grouped on the basis of the morphological appearance of the lesional histiocyte into xanthomatized, oncocytic, vacuolated, spindle-cell and polymorphous subtypes. The clinical characteristics and histological appearances of a variety of conditions within each subtype are discussed.

Isolated Unilateral Pleural Effusion As The Only Manifestation Of The Ovarian Hyperstimulation Syndrome, Abdulaziz S. Aldawood, Afaf A. Felemban
Saudi Medical Journal
Isolated unilateral pleural effusion is uncommon presentation of ovarian hyper stimulation syndrome. The pathogenesis of this syndrome involved an increased permeability of the ovarian capillaries and of the mesothelial vessels triggered by the release of vasoactive substances by the ovaries under human chorionic gonadotropin stimulation. Early recognition of this unusual presentation of ovarian hyperstimulation syndrome should allow for physicians to ensure a better and minimally invasive management of these potentially pregnant patients.

This is a detailed study of the technique of cystolithotomy as practiced by the Muslim surgeon Alzahrawi (Albucasis) in Cordova more than 1000 years ago. In addition to translating the relevant chapter in his book Al-Tasreef, his technique is critically evaluated comparing it with that of his predecessors and his successors. The study confirmed the originality of Alzahrawi who described operative steps and invented operative instruments not known in the Greco-Roman era. He was also the first to describe, in details, the operative technique in women and to recommend the 2-stage operation in complicated cases. His modifications and innovations greatly …

The histopathology of the cyst was a benign Mullerian type cyst. Bilateral tubal ligation performed at the same time revealed normal fallopian tubes. The patient was followed up 6 weeks and 6 months later, and she remains symptom free. The unusual anatomical location of …
