The Case For Indexing: Echocardiographic Dimensions In The Arab Gulf, Adil I. Jumani, Shaini C. Mani, Ghada A. Rashwan, Hadiza O. Ibrahim, Jumaa S. Aldhaheri, Safaa M. Almohdar, Khaled M. Alfakih

Saudi Medical Journal

Objectives: To evaluate echocardiographic parameters in young Emirati males and compare them to American Society of Echocardiography/European Association of Cardiovascular Imaging reference ranges, highlighting the need for population-specific or indexed normal ranges. Echocardiographic reference ranges for cardiac dimensions are primarily derived from Western populations, which may not be suitable for smaller-bodied populations, such as Asians. Methods: Echocardiography was performed on 60 healthy Emirati males undergoing military recruitment from September to December 2024. Measurements included left ventricular (LV), aortic, and left atrial (LA) dimensions and volumes. Parameters were indexed to body surface area (BSA) and compared to established Western norms. Results: …

An Exploration Of Serum 25-Hydroxyvitamin D Levels After Unstable Femoral Intertrochanteric Fractures In Community-Dwelling Elderly, Hakan Zora, Gökhan Bayrak Apr 2025

An Exploration Of Serum 25-Hydroxyvitamin D Levels After Unstable Femoral Intertrochanteric Fractures In Community-Dwelling Elderly, Hakan Zora, Gökhan Bayrak

Saudi Medical Journal

ABSTRACT Objectives: Inadequate vitamin D is related to increased fall risk, which leads to hip fractures among the elderly. Therefore, this study aimed to explore vitamin D levels and the variables potentially influencing the total hospitalization time of community-dwelling elderly patients with unstable femoral intertrochanteric fractures. Methods: Between 2020-2024, 75 elderly patients treated with total hip replacement (THR) were retrospectively analyzed. Patients were divided into the deficient (<20 ng/mL, n=46), insufficient (20 to 29.99 ng/mL, n=16), and normal (≥30 ng/mL, n=13) serum 25-hydroxyvitamin D (25(OH)D) groups. Presence of delirium status, postoperative intensive care entrance, number of chronic diseases, preoperative waiting, and total hospitalization time after THR were recorded. Results: The cohorts’ mean serum 25(OH)D level was 20.11ng/mL. The deficient, insufficient, and normal serum 25(OH)D level groups displayed no significant difference in the postoperative intensive care entrance ( p =0.547) and the presence of delirium ( p =0.947). The multiple linear regression model indicated that elderly patients’ total hospitalization increases with the coefficients as the preoperative waiting time (β=0.466, p =0.001) and the number of chronic diseases (β=0.263, p =0.011) increase. Serum 25(OH)D level did not impact patients’ total hospitalization time (β=-0.072, p =0.474). Conclusion: This study indicated that community-dwelling elderly patients with unstable femoral intertrochanteric fractures had significantly decreased serum 25(OH)D levels. Reducing preoperative waiting time and monitoring and addressing chronic diseases may decrease total hospitalization. Efforts should focus on achieving and maintaining adequate vitamin D levels through supplementation post-hospitalization.

Predictors Of Hypoglycemia Among Children Admitted In The Emergency Pediatric Unit Of The University Of Maiduguri Teaching Hospital, Maiduguri, Nigeria, Ruqayyatu A. Imam, Hassan A. Elechi, Hauwa Y. Musa, Adamu I. Rabasa, Habiba B. Bakari

Saudi Medical Journal

Objectives: To determine the prevalence and pattern of hypoglycemia among children admitted to the Emergency Pediatric Unit (EPU) at the University of Maiduguri Teaching Hospital, Maiduguri, Nigeria. Methods: A cross-sectional study was conducted between February and September 2020. Blood glucose, along with other relevant laboratory investigations, was measured for each patient upon admission to the EPU using a point-of-care test glucometer (ACCU-CHEK with strips). Results: Of the 340 children recruited for the study, 54 patients had hypoglycemia (<2.2 mmol/L), giving a prevalence rate of 15.9%. Thirty-six (66.7%) of the children with hypoglycemia were under the age of 5 years (odds ratio [OR]: 6.218 [1.077–35.912], p =0.041) and 26 (48.1%) were severely underweight (OR: 3.692 [1.266–10.971], p =0.017). Factors such as not having eaten for at least 16 h, weakness, and coma at presentation all independently predicted hypoglycemia (OR: 5.696 [1.768–18.352], 6.556 [1.730–24.850], 9.479 [3.092–29.059], p =0.004, 0.006 and <0.001) respectively. Severe malaria was also independently related to hypoglycemia (OR: 2.720 [0.554–13.365], p =0.021). Conclusion: Hypoglycemia is a common occurrence among children admitted to the EPU. Factors such as being under five years old, in a coma, weakness, severe malaria, and prolonged fasting were all identified as independent predictors of hypoglycemia. Therefore, routine blood glucose monitoring of children admitted to the EPU, specifically those at higher risk, is recommended.

A moderate agreement was found between 3-MinNS and NRS-2002 (κ=0.496, p <0.001). The sensitivity, specificity, and AUC of 3-MinNS were 79.1%, 75.0%, and 0.851, respectively, and it was determined to be a moderately effective nutrition screening tool that can be used to identify malnutrition in patients with cardiovascular diseases. Conclusion: The 3-MinNS is a moderately effective nutrition screening tool that can be administered within the first 24 hours of hospitalization in patients with cardiovascular diseases.
