OBJECTIVE: Considering the hazards of exposure to cement dust, this study incorporated basic hematological parameters, erythrocyte sedimentation rate and the total leukocyte count. The idea was to identify a simple, readily available and cost effective screening test that could help in identifying the presence of disease, its severity, or both in cement mill workers potentially related to their work place. METHODS: This study was carried out in the Department of Physiology, Faculty of Health and Medical Sciences, Hamdard University, Karachi and the University of Karachi, Pakistan, during the time frame 1999-2000. In this study a group of 50, apparently healthy …

OBJECTIVE: To present the available susceptibility data of Mycobacterium tuberculosis (M. tuberculosis) isolates from the Kingdom of Saudi Arabia (KSA) published in peer-reviewed journals. METHODS: In a meta-analysis, studies published between 1966 and 2001 were included. Publication sites include Medline-indexed and non-indexed. Numbers of grown and resistant isolates were tabulated for first-line anti-tuberculosis agents. RESULTS: Twelve studies met the pre-set criteria. Data on 6,316 isolates between 1979 and 2000 were available. Resistance to at least one agent of the first-line anti-tuberculosis agents was 18.4%. Monoresistance to a single first-line agent was found in 10.9%, while polyresistance was noted in 7.6%. …

The increasing usage of cephalosporins, to which the enterococci are resistant, has resulted in the rising number of enterococcal infections worldwide. Enterococci are a normal part of the flora of the human gastrointestinal tract, buccal cavity, perineal skin, vagina, urethra and gallbladder, but may occur as pathogens in a number of sites causing urinary tract infections, intra-abdominal infections, fatal bacteremia, meningitis and endocarditis. A Saudi male who developed enterococcal endocarditis with vegetations on both aortic and mitral valves required mitral and aortic valve replacement. The attention of physicians is drawn to the increasing frequency of enterococcus as a cause of …

Tuberculosis Of The Knee, Umayya M. Musharrafieh, George F. Araj, Ghazi S. Zaatari, Ramzi S. Musharrafieh

Saudi Medical Journal

Tuberculosis is a worldwide problem that has persisted in developing countries and is re-emerging in developed ones. Infectious complications with Mycobacterium tuberculosis have also been reported with increasing frequency. Among these, skeletal and intra-articular infections continue to affect patients and pose a difficult diagnostic problem to physicians and orthopedic surgeons. This is so, as the non-specific clinical presentation which, overlaps with several infectious and non-infectious diseases, and the latency period of this bacteria that can persist up to several years after the initial infection, contribute to diagnosis and management delay. Thus due to the paucity of the disease, physicians should …

Ethical Considerations In The Prevention And Management Of Genetic Disorders With Special Emphasis On Religious Considerations, Mohammed A. Albar Jun 2002

Ethical Considerations In The Prevention And Management Of Genetic Disorders With Special Emphasis On Religious Considerations, Mohammed A. Albar

Saudi Medical Journal

Genetic diseases include not only single gene disoders, but multifactorial, somatic cell genetic disorders, mitochondrial and even chromosomal. One in 4 adults will suffer from a multifactorial or a somatic cell genetic disease. The common diseases in the community have a hereditary component namely diabetes mellitus, hypertension, ischemic heart diseases and many types of cancer. Even monogenic diseases which affect a small number of the newborns (2%-3%), have a greater impact on childhood diseases up to age 15 years. Therefore, it is imperative to scrutinize the available methods of prevention and management of genetic disorders, their ethical implications, and since …

Extrapulmonary Tuberculosis, Epidemiology And Patterns In Saudi Arabia, Abdulrahman A. Alrajhi, Ali M. Al-Barrak

Saudi Medical Journal

Annual incidence rates of extrapulmonary tuberculosis have been increasing over the last few years in the Kingdom of Saudi Arabia. True rates may even be higher due to incomplete reporting. Diagnosis of this condition requires high clinical suspicion, special diagnostic procedures, special staining, and culture media for acid fast bacilli. Delayed diagnosis results in increasing morbidity, mortality, and cost to the health care system. Particularly in areas of high endemicity of Mycobacterium tuberculosis, clinicians should be aware of the various forms of extrapulmonary tuberculosis. The available epidemiology and patterns of various forms of extrapulmonary tuberculosis in the Kingdom of Saudi …

Causes Of Infant Mortality In Jordan, Sami A. Khoury, Diana F. Mas'ad

Saudi Medical Journal

OBJECTIVE: Jordan lacks accurate information on mortality and related indicators. Reporting of infant deaths is defective. Infant mortality rate based on registered deaths was 6/1000 live births, while estimates of this rate based on indirect methods varied between 29-70/1000 live births. Causes of death in general are grossly misrepresented in death certificates. The objective of this paper is to explore, in-depth, causes of infant mortality and related indicators. METHODS: An assessment of causes of death by verbal autopsy was carried out at the Department of Family and Community Medicine, University of Jordan, Amman, Jordan. This was applied between November 1995 …

Diabetes And Infarcted Papillary Thyroid Cancer, Fares H. Haddad, Omar M. Malkawi, Ahmad A. Omari, Alia S. Izzat, Hana M. Khassrof, Luma M. Faiad, Abed-Latif Okla, Abdallah N. Jamil

Saudi Medical Journal

A young black Jordanian lady who has type one diabetes, chronic diabetic complication and ischemic heart disease, presented with a picture of diabetic keto-acidosis, precipitated by an acute neck swelling. This was suggestive of acute suppurative thyroiditis with abscess formation causing compressive symptoms. This unfortunate patient had an eventful course despite aggressive treatment by antibiotics and surgery and then succumbed of an acute cardiac event. The operative tissue biopsy revealed an abscess in an infarcted papillary thyroid cancer. We believe this is a rare presentation of such an association with a fatal outcome.

Saudi Medical Journal

Non menstrual staphylococcal toxic shock syndrome is rare in small infants. This is a 4-month-old infant presented to us with a picture of bronchiolitis and few postuler skin lesions, treated with antistapylococcal antibiotics in addition to other supportive medications. On the 4th day of therapy the patient developed sunburn like erythroderma, hypotension, and high grade fever. The dose of antibiotics was increased to the maximum possible dose, in addition to other supportive medications. The patient improved and developed extensive desquamation in both hands and feet on the 14th day of hospitalization, which confirms the diagnosis.

Combined Brucella Melitensis And Streptococcal Viridans Endocarditis, Tarig S. Al-Khuwaitir, Bashir A. Wani, Suphia M. Sherbeeni

Saudi Medical Journal

Infectious endocarditis is a potentially lethal inflammation of the hearts9 inner lining invaded by microorganisms. The mortality from this illness increases as the number of infective organisms rises to 2, due to involvement of the left side of the heart. These microorganisms usually arise from the patient9s own flora but can be acquired from the environment. Fever and heart murmurs are the principal clinical manifestations followed by a plethora of peripheral signs due to dissemination of microorganisms via the bloodstream. Echocardiographic imaging and sensitive culture techniques form the cornerstone of diagnosis. We report a patient with rheumatic heart disease who …
