Understanding the significance of cytopathological tests in evaluating various infectious processes have become very essential nowadays, as it is a safe, fast, and cost-effective procedure. We present a case of a 52-year-old male with Salmonella empyema where the causative organisms were initially identified on cytology, and subsequently confirmed by microbiological culture. Diff-Quik stained smears showed many colorless, slender, fat short bacilli, which were visualized against the blue-gray background of the smear. These bacilli were identified both intracellularly inside the histiocytes and neutrophils cytoplasm as well as extracellularly in the smear background. OBJECTIVE: To scrutinize the knowledge, attitude, and antimicrobial practices in Saudi Arabian Dentistry. METHODS: In this cross-sectional survey of dentists, a self-administered questionnaire comprising of 61 questions was dispersed to the participants randomly, which included their professional profile, awareness of the current scope of antimicrobial resistance, prescribing practice, frequency of antimicrobial prescription, and sources of continuing education of antimicrobials. OBJECTIVE: To determine the load and importance of respiratory viruses in hospitalized Saudi children with acute lower respiratory tract infections (ALRIs). METHODS: A retrospective study was performed at the Departments of Pediatrics, Pathology/Microbiology, King Khalid University Hospital, Kingdom of Saudi Arabia from January 2005 to December 2010. Nasopharyngeal aspirates were collected from 643 children with ALRI. Viruses were detected by direct immunofluorescence, respiratory syncytial virus (RSV), adenovirus (ADV), type 1 to 3 parainfluenza viruses (PIV), and type A and B influenza virus (flu). OBJECTIVE: To study the incidence and risk factors of the bacteria causing infectious keratitis among patients in Qassim province of Saudi Arabia. METHODS: This is a cross sectional study conducted at the Department of Optometry, College of Applied Medical Sciences, Qassim University, Qassim, Kingdom of Saudi Arabia from December 2010 to May 2011. One hundred patients suspected of keratitis were subjected to clinical examinations. A total of 115 corneal swabs from these cases were collected under aseptic conditions for bacteriological examinations. The Utility Of Platelet, Mean Platelet Volume, And Red Cell Distribution Width In The Diagnosis Of Active Crohn's Disease And Intestinal Tuberculosis, Sha Huang, Feng-Ming Yi, Rui Zhou, Min Chen, Yuan Lei, Jun-Zhang Zhao, Heng Zhang, Bing Xia

OBJECTIVE: To evaluate the diagnostic utility of platelet count (PLT), mean platelet volume (MPV), and red cell distribution width (RDW) in patients with active Crohn9s disease (CD) and intestinal tuberculosis (ITB). METHODS: This study was conducted in the Department of Gastroenterology, Zhongnan Hospital of Wuhan University, Wuhan, China. Sixty-eight patients with active CD, 35 with ITB, and 22 as control group were recruited. Blood routine test including white blood cell, red blood cell, PLT, MPV, RDW, and so forth was investigated. RESULTS: Patients with active CD and ITB have increased PLT and RDW (both p<0.001), and decreased MPV (p=0.002). The RDW performed preferably in predicting both active CD (odds ratio [OR]=2.390, p=0.007), and ITB (OR=2.338, p=0.017), and had better diagnostic value (area under the receiver operating characteristics curve [AUC] - 0.812; p<0.001) than CRP (AUC - 0.716; p=0.007) and ESR (AUC - 0.804; p<0.001) in ITB diagnosis. CONCLUSION: Among the laboratory markers, RDW not only possessed the favorable capability to predict active CD, but also showed outstanding predicting capability, and good diagnostic value in ITB.

Middle East respiratory syndrome coronavirus MERS-CoV is a newly emerging respiratory virus with a high case fatality rate among identified cases. The virus is thought to cause a severe disease in patients with underlying comorbidities. The identification of asymptomatic patients and mild cases among family and healthcare worker contacts of confirmed cases indicates a wider spectrum of clinical manifestation of the disease. The majority of patients presented with fever 98%, fever with cough 83%, and shortness of breath 72%. OBJECTIVE: To characterize, quantify, and assess the function of colostral secretory immunoglobulin A (sIgA) in Saudi women. METHODS: This prospective study was performed between March 2009 and February 2010 at King Khalid University Hospital, Riyadh, Saudi Arabia. Basidiobolomycosis is a rare fungal infection caused by basidiobolus ranarum. The vast majority of gastrointestinal basidiobolomycosis cases were reported from tropical and subtropical regions. We report a Saudi pediatric patient with ileal basidiobolomycosis and initial clinical presentation mimicking acute appendicitis before being misdiagnosed as Crohn9s disease. Our case is the first to report effective treatment of pediatric gastrointestinal basidiobolomycosis using voriconazole mono-therapy. In addition, we present extensive review of pediatric gastrointestinal basidiobolomycosis in medical literature.

We present the clinical course of an 11-year-old child with septic pulmonary embolism secondary to community acquired methicillin-resistant Staphylococcus aureus (MRSA) septic deep venous thrombosis. The aim is to emphasize the non-specific symptoms of septic pulmonary embolism in pediatrics, the frequent association with septic deep venous thrombosis and osteomyelitis, and to highlight that MRSA is the most frequently isolated organism. Pediatricians should consider septic pulmonary embolism in cases of septic deep venous thrombosis even in the absence of respiratory symptoms. The initial antibiotic management should include glycopeptides, as community acquired MRSA is increasingly the isolated organism in this disorder.

The ocular involvement of tuberculosis is not uncommon but diverse. Rarely, patients initially present with ocular signs that simulate intraocular malignancy. We report 3 cases of isolated presumed choroidal tuberculoma masquerading as intraocular tumor. Two patients had no systemic evidence of tuberculosis with strongly positive purified protein derivative skin test and one patient had evidence of inactive old pulmonary tuberculosis demonstrated by chest computed tomography. Antituberculous regimen including isoniazid 5 mg/kg/day, rifampicin 600 mg/day, ethambutol 15 mg/kg/day, and pyrazinamide 30 mg/kg/day were employed in all cases for 9 months. We describe the third confirmed case of novel coronavirus infection in a resident of the Arabian Peninsula. Our patient presented, as did 2 prior cases, with severe pneumonia and renal dysfunction requiring intensive care support including assisted ventilation. However, unlike the earlier cases, and despite underlying chronic disease and a single kidney, he survived his infection and has been discharged home. The Ministry of Health continues active surveillance for additional cases. As this case report goes to press, 2 additional confirmed cases have been identified in Riyadh, Saudi Arabia. Current published data makes clear the relationship between genital human papillomavirus (HPV) infection and cervical cancer. Although there is an opportunity for screening programs that could obviate the disease, cervical cancer still remains the second most common cancer among women worldwide. The subtypes HPV 16 and 18 affect the anogenital tract mucosal surfaces, and accounts for nearly 70% of all cervical cancers, and 90% or more of anal cancer cases. METHODS: A cross-sectional study during November and December 2009 among hajjis registered while visiting Primary Health Care Centers of Riyadh, Kingdom of Saudi Arabia to get mandatory meningococcal meningitis vaccination. OBJECTIVE: To examine susceptibility of Pseudomonas aeruginosa (P. aeruginosa) and Acinetobacter baumannii (A. baumannii ) against carbapenems along with colistin and tigecycline as alternative therapeutic options. METHODS: A total of 117 strains of multidrug-resistant (MDR) non-fermenting Gram negative bacteria isolated from non-duplicate samples were collected consecutively. We included one sample from each patient (84 isolates of A. baumannii and 33 isolates of P. aeruginosa isolated from patients seen at King Khalid University Hospital, Riyadh, Saudi Arabia, from June to December 2010). OBJECTIVE: To determine the hepatitis B virus (HBV) genotypes and subgenotypes in patients with HBV infection in the Eastern Black Sea region of Turkey. METHODS: One hundred and thirty-seven patients9 samples collected over 5 years (January 2005 to January 2010) at Farabi Hospital in Karadeniz Technical University, Trabzon, Turkey were included in the study. OBJECTIVES: To evaluate the safety of Bacillus Calmette-Guerin (BCG) in Saudi infants and outline our management for BCG related lymphadenitis. METHODS: The records of infants who developed BCG related complications were retrospectively reviewed from March 2008 to March 2011 at the Maternity and Children Hospital, Dammam, Saudi Arabia for age, gender, birth weight, presentation, and outcome. All our patients were immunized with the BCG vaccine within 48 hours after birth, and the total number of vaccinated newborns was obtained from the vaccination registry.
