Methods: In this cohort study, we retrospectively investigated the seroprevalence of Chlamydia pneumoniae , Mycoplasma pneumoniae , and Legionella pneumophila among randomly selected 189 confirmed COVID-19 patients at their time of hospital presentation via commercial immunoglobulin M (IgM) antibodies against these bacteria. We also carried out quantitative measurements of procalcitonin in patients’ serum. Results: The seropositivity for L. pneumophila was 12.6%, with significant distribution among patientsolder than 50 years (χ2 test, p =0.009), while those of M. …

“Smart Optometry” Phone-Based Application As A Visual Acuity Testing Tool Among Pediatric Population, Lina H. Raffa, Nasser T. Balbaid, Mariam M. Ageel
Saudi Medical Journal
Objectives: To evaluate the validity of a smartphone-based application for visual acuity (VA) testing in children and to compare parent and clinician-performed VA to standard VA assessment. Methods: A cohort of 100 children aged <18 years old was recruited. Subjects were randomly assigned to either start with conventional distance VA chart screening or the smartphone VA assessment twice by both the clinician and the caregiver if applicable. Near VA scores were assessed using the near vision E chart. Accuracy and reliability values were analyzed. Results: One hundred patients with an average age of 9.92 ± 3.0 years old were enrolled. The difference between conventional distance and application logMAR values was -0.023, and the difference between near vision and application logMAR values was -0.004. “Smart Optometry” had a sensitivity of 89.3% in detecting subnormal VA compared with conventional vision testing methods. Sensitivity in detecting subnormal VA was found to be higher in younger age groups up to 91.7% in comparison with the older age groups. The interclass correlation of application-measured VA scores by the caregivers and the clinician were 0.77 (95% CI; 0.67-0.83) using single measures and 0.87 (95%CI; 0.8-0.9) using average measures. Conclusion: “Smart Optometry” phone application was found to be an acceptable home-based VA testing tool with good inter-rater reliability for young children showing good sensitivity in detecting subnormal VA, but lower sensitivity in detecting amblyopia.

Prevalence Of Preterm Birth And Risk Factors Associated With It At Different Gestational Ages: A Multicenter Retrospective Survey In China, Yi-Jie Zhang, Yan Zhu, Li Zhu, Cheng-Qiu Lu, Chao Chen, Lin Yuan
Saudi Medical Journal
Objectives: To investigate the prevalence of preterm birth (PTB) and the risk factors for different gestational age subgroups of preterm birth in China. Methods: We carried out a descriptive cross-sectional study encompassing all singleton live births (24 +0 to 41 +6 weeks) with completed data in 23 provinces in China from 2010 to 2017 during investigation period. We compared both the preterm group (24 +0 to 36 +6 weeks) and preterm subgroups (<32 weeks, 32 +0 to 33 +6 weeks, and 34 +0 t0 36 +6 weeks) with the term group (37 +0 to 41 +6 weeks). We collected information on maternal and fetal characteristics from medical records. Logistic regression was use. Results: The prevalence of PTB was 7.4% (15,833/215,254) in singleton births. After adjusting for maternal age, parity, and potential risk factors in univariate analysis, the high-risk factors for PTB at <32 weeks were placental abruption (aOR=41.52; 95% CI, 25.89–66.58), placenta previa (aOR=40.04; 95% CI, 32.00–50.09), chorioamnionitis (aOR=11.06; 95% CI, 8.738–14.02), and hypertension disorders in pregnancy (HDP) (aOR=3.564; 95% CI, 2.930–4.335). Intrahepatic cholestasis of pregnancy (ICP) was significantly associated with PTB at 34–36 weeks (aOR=5.763; 95% CI, 5.049–6.577), particularly with spontaneous PTB (aOR=10.04; 95% CI, 8.79–11.47). Gestational diabetes mellitus (GDM) was significantly associated with PTB at 34–36 weeks only (aOR=1.156; 95% CI, 1.054–1.267). Conclusion: Placental abruption, placenta previa, chorioamnionitis, and HDP were more predictive of early PTB; GDM and ICP were more predictive of late PTB.

Objectives: To analyze the clinical and echocardiographic changes in individuals with morbid obesity who underwent bariatric surgery. Methods: In total, 59 obese patients with body mass index >35 kg/m 2 were prospectively enrolled. We assessed baseline pre-operative and a 6-month post-operative lipid profile, hemoglobin A1c, echocardiography, lifetime, and a 10-year risks of atherosclerotic disease for all patients. Results: The mean patients’ age was 37±12 years, with 40 (67.8%) women. We found that the pre-operative total cholesterol (4.2±1.1 vs. 4.4±1.1, p =0.014) and triglyceride levels (1.4±0.7 vs. 1.8±0.8, p <0.0001) were significantly lower than post-operative levels, while post-operative high-density lipoprotein levels were significantly higher (1.5±0.5 vs. 1.2±0.3, p <0.0001). The calculated 10-year risk of atherosclerotic cardiovascular disease was significantly lower post-operatively (1.1±1.6% vs. 1.6±1.8%, p <0.0001). Echocardiography follow-up revealed that diastolic dysfunction was more prevalent pre-operatively than that post-operatively (41% vs. 10%, p <0.0001). Post-operative left ventricular (LV) mass was significantly lesser than the pre-operative mass (168±252 g vs. 187±255 g, p =0.019), whereas the post-operative LV diastolic (46.5±7 mm vs. 38.5±18 mm, p =0.002) and systolic dimensions (31±5 mm vs. 25±11 mm, p =0.001) were significantly smaller. Conclusion: Bariatric surgery resulted in a significant amelioration in lipid profile, reduction in LV mass, and LV cavity dimensions.
