OBJECTIVE: To determine the predictive value of commonly used clinical and laboratory factors for mortality in patients with pneumonia in the Arab world. METHODS: We retrospectively analyze the data collected from all inpatients over the age of 16 years with a diagnosis of pneumonia in Tawam Hospital, Al-Ain, United Arab Emirates between the years 1997 and 2002. Patients were grouped into those who survived and those who died in the hospital. Clinical and laboratory factors on admission were used to predict outcome using simple, and multiple logistic regression analyses. RESULTS: Among the 236 patients admitted, 122 were females (age 56.9 …

Progress Towards A Leishmania Vaccine, Khaled S. Tabbara
Progress Towards A Leishmania Vaccine, Khaled S. Tabbara
Saudi Medical Journal
Leishmaniasis is a vector-born protozoan disease. Approximately 12 million individuals are affected worldwide with an estimated annual incidence of 1.5-2 million. Two clinical manifestations are recognized, cutaneous, and visceral, both of which are common in the Middle East. In both forms, infection is chronic, with potential deformities, persistence following cure, and lifelong risk of reactivation. Attempts to develop an effective human Leishmania vaccine have not yet succeeded. Leishmanization, a crude form of live vaccination historically originated in this part of the world. Experimental vaccination has been extensively studied in model animals in the past 2 decades. In this review, major …

OBJECTIVE: To study whether there will be a permanent lumbar nerve root scarring or degeneration secondary to continuous compression followed by decompression on the nerve roots, which can account for postlaminectomy leg weakness or back pain. METHODS: The study was performed at the Department of Anatomy, Faculty of Medicine, King Abdul-Aziz University, Jeddah, Kingdom of Saudi Arabia during 2003-2005. Twenty-six adult male New Zealand rabbits were used in the present study. The ventral roots of the left fourth lumbar nerve were clamped for 2 weeks then decompression was allowed by removal of the clips. The left ventral roots of the …

Saudi Medical Journal
OBJECTIVE: To eliminate malaria parasites in donors9 blood in vitro for eradication of transfusion-induced malaria. METHODS: We conducted the study at Ahmed Gasim Hospital in Khartoum, Sudan, between January 2005 and January 2006. Out of 4484 blood samples screened for malaria parasite microscopically, only 30 (200 ml each) satisfied the inclusion criteria of this study. The samples were subdivided equally into 4 portions. Three concentrations of quinine were separately added to 3 specimens while the fourth left without quinine (control). Blood specimens were tested on the day of collection by hematological and biochemical techniques simultaneously, and after 24 and 48 …

Asynchronous Adenoid Cystic Carcinoma Of The Prostate And Transitional Cell Carcinoma Of The Urinary Bladder, Luma M. Fayyad, Khalaf M. Al-Jader, Belal A. Al-Hawwari
Saudi Medical Journal
Histologic variants of prostatic carcinoma are readily recognized. In this report, we describe a rare variant, adenoid cystic carcinoma, in a 75-year-old man previously diagnosed to have transitional cell carcinoma of the urinary bladder. The diagnosis of adenoid cystic carcinoma was made by the characteristic microscopic features of the tumor morphologically and immunohistochemically. Two months later he was found to have metastatic disease. The patient9s treatment consisted of chemotherapy in combination with prednisone and hormonal therapy. Five and a half months after diagnosis, he died with metastatic disease. Making this case unique is the asynchronous occurrence of this variant with …

METHODS: Serum samples from patients presenting with AFI at 13 fever hospitals across Egypt between 1999 and 2003 were kept frozen at NAMRU-3 and used in this study. The assay was evaluated in 5 subject groups: brucellosis cases confirmed by blood culture (group I, n=202) 87% positive by standard tube agglutination test (TA), brucellosis cases exclusively confirmed by TA (group II, n=218), blood cultures from AFI cases positive for bacterial species …

Evaluation Of The Effect Of Castor Oil On Initiating Labor In Term Pregnancy, Sedighe Azhari, Sohaylla Pirdadeh, Marzieh Lotfalizadeh, Mohammad T. Shakeri
Saudi Medical Journal
OBJECTIVE: To determine the effect of castor oil on initiating labor in term pregnant women. METHODS: We conducted this randomized control clinical trial on 47 pregnant women from August 2003 to March 2004 in Mashhad University of Medical Sciences, Iran. RESULTS: There was a significant increase in labor initiation ratio in the castor oil group compared with the control group (54.2% compared with 4.3%, p<0.001). Also, the mean bishop score in the castor oil group increased from 2.50 ± 1.29 to 6.79 ± 3.20 (p<0.001). CONCLUSION: The probability of labor initiation increases during the first 24 hours after using castor oil, however, further studies are recommended to improve its efficacy and safety.

OBJECTIVE: To asses the commercial available enzyme-linked immunosorbent assays (ELISA) for differentiation of herpes simplex virus type 1 (HSV-1) and type 2 (HSV-2) antibodies. METHODS: The study was performed between January 1997 to November 2002 in the Division of Virology, Department of Pathological Sciences, Central Manchester Healthcare Trust and University of Manchester, Manchester, United Kingdom. Assays based upon type-specific glycoprotein G-1 (gG-1) for HSV-1, and glycoprotein G-2 (gG-2) from HSV-2 were evaluated to differentiate between HSV-1 and HSV-2 antibodies. Using 5 different ELISA tests, 2 panels of serum samples were tested. Panel one consisted of 88 sera, selected from the …
