Malignant Transformation Of Persistent Endometriosis After Hysterectomy, Naif A. Bawazeer, Hatim M. Al-Jifree, Abdulrahim M. Gari
Saudi Medical Journal
The malignant transformation of persistent endometriotic implants into endometrioid adenocarcinoma is rare, especially after remote hysterectomy and salpingo-oophorectomy (TAH-BSO), and there are few cases reported in the English language literature. Patients receiving estrogen replacement therapy are common among the reported cases. We present a case that demonstrates the possibility of malignant transformation in a 53-year-old female, known case of endometriosis, who underwent total abdominal hysterectomy and bilateral salpingo-oophorectomy with no evidence of malignancy in the final pathology report. After 9 years, she presented with lower abdominal mass, and histopathological studies confirmed the diagnosis of well-differentiated endometrioid adenocarcinoma. The possibility of …

Does Cesarean Section Have An Impact On The Successful Initiation Of Breastfeeding In Saudi Arabia?, Afnan A. Albokhary, Jennifer P. James
Saudi Medical Journal
OBJECTIVES: To investigate whether the type of birth influenced breastfeeding outcomes. METHODS: This study used a quantitative descriptive correlation design study in a sample of 60 primigravida mothers. Participants were recruited over a 2-month period from June to July 2011 in the postnatal ward at King AbdulAziz University Hospital (KAUH) in Jeddah city, Kingdom of Saudi Arabia (KSA). RESULTS: The results of the study indicated that women who gave birth vaginally were more likely to breastfeed within the first hour, and at 24 hours after birth than those who had a cesarean section. The mothers who had cesarean section stated …

METHODS: This study was conducted at the Endocrine Unit, Dubai Hospital, Dubai, United Arab Emirates. We retrospectively collected data from patients with type 2 diabetes started on exenatide between November 2011 and February 2012. Data included demographics, clinical, laboratory results, and medications used. A general linear model adjusted by baseline characteristics (weight, HbA1C, age, use of statins, and duration of diabetes) was used to assess changes between …

Out of the 43,200 patients seen for different causes through that period, 1008 (2.3%) (females: 675 [67%]; males: 333 [33%]) had gastric complaints, and were subjected to an examination of blood and stool for H. pylori and parasites. Data regarding age and gender was also collected. RESULTS: The age of the patients ranged from 3-15 …

Pertussis. A Reemerging And An Underreported Infectious Disease, Muhammad A. Syed, Noureen F. Bana
Saudi Medical Journal
Pertussis or whooping cough is a highly infectious, vaccine preventable disease. The incidence of the disease has greatly been reduced since the introduction of the diphtheria, tetanus, pertussis vaccine. Pertussis resurgence has been observed in highly vaccinated populations of Western countries since 1990s. Poor vaccine quality, waning vaccine induced immunity, pathogen adaptation, and enhanced surveillance as well as advancements in diagnostic facilities are some of the reasons considered responsible for the increased reporting of pertussis cases. Pertussis may have been ignored and unnoticed due to its atypical manifestations in partially immunized population or people with waning immunity. We review the …

Radiation therapy of the head and neck area has resulted in numerous and undesired effects on the oral cavity tissue. The most frequent and most dangerous complications of radiotherapy of the head and neck area is osteoradionecrosis (ORN). The purpose of this study is to demonstrate the possibility of using ozone in the prevention of ORN of the jaw, and show the recent therapeutic protocols for treatment in patients receiving radiation therapy for the head and neck. We used an ElektroMagneTron device during the therapy. The patient underwent postoperative follow-up examination when bio-oxygenation therapy and clinical evaluation of wound healing …

Anal Fistula. Past And Present, Ahmad M. Zubaidi
Saudi Medical Journal
Anal fistula is a common benign condition that typically describes a miscommunication between the anorectum and the perianal skin, which may present de novo, or develop after acute anorectal abscess. Athough anal fistulae are benign, the condition can still negatively influence a patient’s quality of life by causing minor pain, social hygienic embarrassment, and in severe cases, frank sepsis. Despite its long history and prevalence, anal fistula management remains one of the most challenging and controversial topics in colorectal surgery today. The end goals of treatment include draining the local infection, eradicating the fistulous tract, and minimizing recurrence and incontinence …

Using The Theory Of Planned Behavior To Predict Infant Restraint Use In Saudi Arabia, Anna Nelson, Naomi N. Modeste, Helen H. Marshak, Joyce W. Hopp
Saudi Medical Journal
OBJECTIVES: To determine whether the theory of planned behavior (TPB) predicted intent of child restraint system (CRS) use among pregnant women in the Kingdom of Saudi Arabia (KSA). METHODS: In this cross-sectional study conducted in Dallah Hospital, Riyadh, KSA during June-July 2013, 196 pregnant women completed surveys assessing their beliefs regarding CRS. Simultaneous observations were conducted among a different sample of 150 women to determine CRS usage at hospital discharge following maternity stay. RESULTS: Logistic regression model with TPB constructs and covariates as predictors of CRS usage intent was significant (χ2=64.986, p<0.0001) and predicted 38% of intent. There was an increase in odds of intent for attitudes (31.5%, p<0.05), subjective norm (55.3%, p<0.001), and perceived behavioral control (76.9%, p<0.001). The 3 logistic regression models testing the association of the relevant set of composite belief scores were also significant for attitudes (χ2=16.803, p<0.05), subjective norm (χ2=29.681, p<0.0001), and perceived behavioral control (χ2=20.516, p<0.05). The behavioral observation showed that none of the 150 women observed used CRS for their newborn at discharge. CONCLUSION: The TPB constructs were significantly and independently associated with higher intent for CRS usage. While TPB appears to be a useful tool to identify beliefs related to CRS usage intentions in KSA, the results of the separate behavioral observation indicate that intentions may not be related to the actual usage of CRS in the Kingdom. Further studies are recommended to examine this association.
