Loxosceles reclusa (L.reclusa) is known to bite humans, and its venom includes several enzymes that cause clinical symptoms. Loxoscelism, a condition due to being bitten by Loxosceles spiders, commonly known as recluses, can involve a range of clinical conditions, from local cutaneous lesions to severe systemic involvement. The diagnosis of loxoscelism is usually made by anamnesis and clinical findings. Magnetic resonance imaging is recommended for patients at high risk of necrotizing fasciitis. Treatment modalities are still controversial and there is no standardized treatment approach. Reported here, our case of loxoscelism involved a 24-year-old man presenting with a Loxosceles spider bite, …

The Emergence Of New Trends In Clinical Laboratory Diagnosis, Mohammed A. Alaidarous
The Emergence Of New Trends In Clinical Laboratory Diagnosis, Mohammed A. Alaidarous
Saudi Medical Journal
Diagnostic processes typically rely on traditional and laborious methods, that are prone to human error, resulting in frequent misdiagnosis of diseases. Computational approaches are being increasingly used for more precise diagnosis of the clinical pathology, diagnosis of genetic and microbial diseases, and analysis of clinical chemistry data. These approaches are progressively used for improving the reliability of testing, resulting in reduced diagnostic errors. Artificial intelligence (AI)-based computational approaches mostly rely on training sets obtained from patient data stored in clinical databases. However, the use of AI is associated with several ethical issues, including patient privacy and data ownership. The capacity …

Saudi Medical Journal
Objectives: To determine the factors associated with the development of methicillin-resistant Staphylococcus aureus (MRSA), hospital stay and mortality, and early versus late MRSA infection. Methods: Cases (n=44) were intensive care unit (ICU) patients admitted to King Fahd Specialist Hospital, Al-Qassim, Saudi Arabia between 2015 and 2019 who developed MRSA during their hospital stay. Controls (n=48) were patients from the same place and period who did not develop MRSA. Data were abstracted from hospital records. Results: Admission with sepsis (case: 46% vs. control: 2%, p <0.001) and having at least one comorbid condition (case: 95% vs. control: 46%, p <0.001) were significantly associated with the development of MRSA. Age (mean ± SD: case: 65±18, control: 64±18, p =0.7) and gender (% male, case: 52%, control: 56%, p =0.70) were not associated with the development of MRSA. Approximately 73% of all MRSA cases developed within the first 2 weeks of admission. Among the early cases, 44% died during their ICU stay; the corresponding percentage among the late cases was 42% ( p =0.69). There was no difference between early and late MRSA cases in terms of non-sepsis admissions (50% vs. 67%, p =0.32) or comorbid status (at least one: 97% vs. 92%, p =0.17). Conclusion: Sepsis and comorbid conditions were significant risk factors for MRSA development among hospital patients.

Western-Designed Total Knee Implants Mismatch Saudi Patient Knees: Anatomical Bony Measurements Documented By Computed Tomography, Mohammed L. Al-Otaibi, Ghazi A. Alshumrani
Saudi Medical Journal
Objectives: To determine the anthropometric features of Saudi knees compared with knees of other ethnicities and reveal mismatches among different knee implant systems. Methods: All knee computed tomography images obtained between January 2016 and September 2016 for varying medical reasons in a tertiary care hospital in Southwestern Saudi Arabia were retrospectively evaluated. Patients aged <18 years, with trauma or soft tissue or bone tumors were excluded. Eleven parameters were measured for each knee joint. Results: We evaluated 100 knees of 50 Saudi patients (25 men, 25 women) aged 28-85 (mean, 57.3) years. No statistically significant differences were found in age and Sasaki angle measurements between male and female patients. All other parameters were significantly larger in male than female knees. Our measurements were close to those published for Chinese, Thai, and Korean patients; however, published measurements were larger for Caucasian and Indian knees than our measurements. Conclusion: The Saudi population has morphological features of knee bone anatomy that do not match with the widely available prostheses designed for Caucasian knees. As several studies of different ethnicities have documented considerable prosthesis mismatch; our study further indicates the need for new implant designs that take these variations into account.

Association Between Preeclampsia And Prostasin Polymorphism In Pakistani Females, Saima Ejaz, Anwar Ali, Kamran Azim, Atif Mahmood, Asif I. Khan, Tuline A. Almazyad, Bushra Bilal
Saudi Medical Journal
Objectives: To investigate the relationship between a prostasin gene variations and the development of preeclampsia in a Pakistani female population. Methods: This was a case-control study carried out at University of Karachi, Karachi, Pakistan between May 2018 and 2019. A single nucleotide polymorphism (SNP) at rs12597511 locus was examined with polymerase chain reaction (PCR) and restriction fragment length polymorphism (RFLP) analyses in 76 preeclamptic and 74 normotensive expecting mothers. Results: We observed significantly increased risk of preeclampsia associated with the CC genotype of rs12597511 polymorphism as compared to TT ( p <0.001, OR=8.08, 95% CI:1.28-31.19) and TT/TC ( p <0.001, OR=14.66 and 95% CI: 3.31-65.07) genotypes carriers. Calculation of the allelic distribution revealed a higher frequency of the T allele (82%) among controls; however, the C allele was more prevalent in the preeclamptic group (36%) significantly. Conclusion: The significantly higher C allele frequency in the prostasin gene at the rs12597511 locus in the preeclamptic group indicates that the distribution of the C allele of the prostasin gene is a potential risk factor contributing to the development of preeclampsia.
