Risk factors that contribute to pressure ulcer formation include immobility and inactivity, age, malnutrition, fecal and urinary incontinence, and a decreased level of consciousness. Gerontological nurses should collaborate with other health disciplines (eg, the physician, physical therapist, and dietitian) to plan and provide preventive treatment measures for the elderly person with potential for impaired skin integrity: pressure ulcers. Gerontological nurses must continue the development and refinement of tools with which to predict the degree of risk for impaired skin integrity, as well as use a consistent method of grading pressure ulcers.

Critical care nurses face the challenge of helping anxious families cope with the critical illness of one of their members. The purposes of this exploratory descriptive pilot study were to identify whether adult family members of surgical intensive care unit (SICU) patients were willing and able to learn imagery during the time of their family member's critical illness, and which factors facilitate the learning. Of the 139 persons invited, 26 (18.7%) indicated willingness to participate, but only 10 participants completed both learning sessions. All 10 subjects achieved increased relaxation with the imaging. The primary purpose of this investigation was to determine the internal consistency of the 60+ functional fitness test battery for older adults. A secondary purpose was to determine whether the number of trials recommended in the testing manual was optimal. Subjects were used from an ongoing study of a fitness program for community-dwelling older adults. Internal consistency coefficients for subtests of flexibility, agility, coordination, and strength were excellent. Post hoc analysis across trials indicated significant differences in means between early trials and later trials. Mean scores stabilized after two or three trials, depending on the subtest of interest. Data provided …

Nurses who work with patients with Alzheimer's disease (AD) are especially vulnerable to the effects of stress and burnout. Many are poorly trained to cope with the emotional and behavioral problems of these residents and are repeatedly frustrated by their inability to manage these recurrent problems. Specialized care units (SCUs) reduced stress in the areas of staff knowledge, abilities, and resources. The SCU was designed specifically to provide the special environmental structures, support, and service systems required for the care of persons with AD that would enhance functioning and decrease associated behavioral problems. The purpose of this study was to identify and validate specific activities considered important in the implementation of three non-pharmacologic pain management interventions-relaxation, distraction and guided imagery. Using a two-round Delphi survey, nurses selected for their expertise in pain management were asked to validate definitions and activities considered important in the implementation of each of these strategies. Data were analyzed using a modification of Fehring's diagnostic content validity method. From this process, a list of activities for each intervention evolved that has implications for nursing research, practice and education.

Pain is of great clinical importance, often associated with disability, loss of independence, and reduced quality of life. The purpose of this study was to determine the effectiveness of a community-based strength training program for older adults in decreasing falls. Subjects who completed a strength training program and an age and gender matched comparison group of older adults who did not participate in the strength training program were questioned about falls during the preceding year. Functional fitness measures for coordination, balance, and strength significantly improved in strength training subjects after completion of a six week program. Furthermore, after controlling for gender effects, the findings revealed that the strength training group reported significantly fewer falls in the preceding year …

Objective: The specific objective for this research was to determine initial psychometric properties of the Faces Pain Scale (FPS) as a measure of pain intensity for use with the elderly. Design: The study was descriptive correlational in nature, with nonrandom sampling. A total sample of 168 community subjects (30-121, depending on task completed), aged 65 or older, participated in the research protocol. To determine the validity, reliability, and scaling properties of the FPS, rating and ranking procedures, placement tasks, and test-retest methods were used. Historically, graduate nurses are not attracted to older adults or geriatric care. Innovative strategies, such as the Young Gerontological Nurse Clinician Program (YGNCP) initiated by the John A. Hartford Center for Geriatric Nursing Excellence at the University of Iowa, are needed to promote geriatric nursing as a challenging and rewarding practice focus. The YGNCP, which provides early, positive, self-directed experiences for nursing students under the direction and supervision of faculty and expert clinical mentors, shows excellent potential for recruiting nurses to clinical and leadership positions in geriatric and geropsychiatric care.

Acute confusion (AC), also referred to as delirium (AC/delirium), is a common problem seen by health professionals who work in a variety of care settings. This is an evaluative report on the clinical usability of instruments to assess AC/delirium as a part of nursing practice. Specifically, five instruments [the Confusion Assessment Method (CAM), Delirium Rating Scale (DRS), Delirium Symptom Inventory (DSI), Mini-Mental State Examination (MMSE), and Neelon/Champagne (NEECHAM) Confusion Scale] are discussed. The work demonstrates how the cooperation of nurses in practice, education, and research can improve both patient and staff outcomes.

In this study, the authors explored the perceptions of effective teaching-learning strategies from the perspective of graduates of a second-degree prelicensure nursing program. Data were collected using interviews and analyzed using qualitative description. Seven themes were identified. These findings provide a unique perspective in identifying teaching strategies for use with these students and may provide insight and direction to others who have considered or are considering second-degree prelicensure programs.

While acute confusion (AC) is frequently studied in the hospitalized elderly population, this phenomena has been largely ignored in elders who are residents in long-term care (LTC) facilities. The purpose of this study was to estimate the prevalence of AC in older LTC residents, the antecedent conditions present at the time of the AC event and the recognition rate of AC when assessed by staff nurses in two LTC facilities. Despite the high prevalence of acute confusion among elders and the importance of its early detection, there are few reports of systematic efforts to increase staff competence. This article describes the development and evaluation of an 8-hour educational program designed to prepare staff nurses to perform in a new role, the unit-based acute confusion Resource Nurse (ACRN). Pre- and posttest scores were obtained for the 80 registered nurses who participated in the program. Research tabulating the existence and effects of pain related factors in the elderly is lacking, in spite of the continued growth of the aged population. Leg pain, in particular, may interfere with activities and interpersonal interactions. This study of 3,097 rural elderly examines the prevalence of complaints of leg pain and related factors in a geographically-defined population. Findings indicate that leg pain is clearly a common experience which impacts daily activities of the elderly.

Objective: To document the prevalence of depression an examine the relationship between depression and selected pain-related variables associated with chronic back pain among elderly and nonelderly samples. Design: Survey with mailed questionnaire. Setting: Patients seeking treatment at a spine diagnostic and treatment center. Patients: Consecutive sample of elderly subjects (n = 69). Nonelderly subjects (n = 59) were randomly selected to achieve a sample size comparable to the elderly. Evaluation of depressed mood was determined by Beck Depression Inventory. Nurses working with the elderly encounter many complex and potentially stressful care situations. Nowhere is this more true than for nursing home staff who work in highly demanding, labor- and client-intensive jobs.
