In the present day, high-throughput (HT) sequencing techniques provide in-depth data on how the transcripts are expressed and regulated and in this case shed light on how innate immunity responds to different stimuli. Acute Hepatopancreatic Necrosis Disease (AHPND) caused by Vibrio parahaemolyticusproducing Pir toxin (VPAHPND) is a current severe bacterial disease in shrimp. Suppression subtractive hybridization (SSH) was used to identify differentially expressed genes (DEGs) in hepatopancreas and hemocyte of VPAHPND-challengedPenaeus vannamei. The IKK-NF-κB signaling cascade is one of the crucial responsive mechanisms in inflammatory and immune responses. The key kinase proteins called inhibitor of kappa B kinases (IKKs) serve as the core elements involved in cascade activation. Here, the complete open reading frames of IKK homologs including PmIKKβ, PmIKKε1 and PmIKKε2, from the black tiger shrimp Penaeus monodon were identified and characterized for their functions in shrimp antiviral responses. The PmIKK transcripts were widely expressed in various examined tissues and the PmIKKε protein was detected in all three types of shrimp hemocytes. Rotavirus is an important cause of diarrhea in piglets and pigs worldwide, and group A (RVA) and group C (RVC) are mostly affected. Sulfonamide antibiotics are commonly used veterinary drugs in livestock to prevent or treat infectious diseases. Their residues in food are of concern since they can cause adverse health effects and development of bacterial resistance. In this work, a practical method for detection and quantification of sulfamethazine, one of sulfonamide antibiotics, in pig body fluid samples is developed via the combination of simple extraction and paper spray mass spectrometry. Background: Triple-drug antiretroviral prophylaxis of zidovudine (ZDV)/lamivudine (3TC)/nevirapine (NVP) for high risk HIV-exposed neonates is recommended within the Thai national program. However, there are limited data about the safety and drug concentration achieved with this regimen initiated at birth.

This research involved in the complete nitrogen removal in marine recirculating aquaculture system (RAS) through sequential nitrification and denitrification processes using internal biofilter within a single tank. The study was divided into two experimental parts. The first study was to estimate the effects of salinity (5, 15 and 25 PSU) and stocking density (50 and 100 shrimp m-2) on nitrification and denitrification efficiencies, as well as on microbial diversity in the biofilm. Macrophage plasticity is a process that allows macrophages to switch between two opposing phenotypes based on differential stimuli. Functional plasticity of macrophages contributes to disease pathogenesis such as cancer, metabolic diseases, autoimmune diseases and systemic infections. Epigenetics plays an important role in regulating this process. Sepsis is a severe response to systemic infection and one of the leading causes of death in patients world-wide. Bacterial endotoxin (lipopolysaccharide; LPS), is a potent inducer of inflammation that has been associated with the pathophysiology of sepsis. The repeated LPS exposure can induce a suppressive effect as refer to endotoxin tolerance. Wild animals such as elephants and non-human primate have been reported to be infected by M. tuberculosis (MTB) complex, which can cause tuberculosis in animals as well. Previous studies suggest that periodontitis may be associated with an increasing risk of hypertension. However, data from large epidemiological studies are limited. The aim of this cross-sectional study was to examine the association between periodontitis and hypertension using data from a cohort study in a Thai population. The study population comprised of 1,378 employees of the Electricity Generating Authority of Thailand (EGAT), aged 53 to 73 years old, were categorized into no/mild, moderate, or severe periodontitis groups. This research focuses on how to use natural fibers available from agricultural residues to produce biodegradable materials such as mulch films. Green composites were prepared from natural fibers (peanut shell and bagasse fiber) and polylactic acid (PLA) together with plasticizers. The presence of plasticizers improved PLA molecular mobility as seen by the reduction of glass transition, cold crystallization, and melting temperatures. The addition of natural fibers into PLA brought down decomposition temperature when compared to PLA. Flavobacterium columnare is Gram negative bacteria which caused columnaris disease in various kinds of freshwater fishes. This study is the first report of F. columnare infection in freshwater culturing Asian sea bass in Thailand. Phenotypic characterization, antimicrobial susceptibility test and quinolone resistant-associated genes of F. columnare were performed. A total 15 of F. columnare were isolated from diseased fishes and phylogenetic analysis revealed that these isolates were belonged to genetic group 2 and 4. In this study, we aimed to detect antimicrobial resistance determinants and virulence factor genes in V. parahaemolyticus isolates from cultivated oysters and estuarine waters in southern Thailand. A total of 594 V. parahaemolyticus isolates were collected from pooled oysters (n=361) and estuarine waters (n=233). The samples were collected monthly between March 2016 and February 2017 from Thap Put district, Phang Nga province in southern Thailand. การศึกษาแบบเก็บข้อมูลย้อนหลังนี้มีวัตถุประสงค์เพื่อสร้างแบบจำลองทางเภสัชจลนศาสตร์และหาปัจจัยที่มีผลต่อค่าพารามิเตอร์ทางเภสัชจลนศาสตร์ของยาแวนโคมัยซินในผู้ป่วยเด็กชาวไทย ค่าความเข้มข้นของ ยาแวนโคมัยซินในซีรั่มจานวน 348 ตัวอย่าง จากผู้ป่วยเด็กอายุระหว่าง 1 เดือน ถึง 18 ปี ที่เข้ารับการรักษา ณ โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย ระหว่างปี พ.ศ. 2555-2560 จานวน 212 คน วิเคราะห์ข้อมูลเภสัช จลนศาสตร์ประชากรด้วยโปรแกรม NONMEM® โดยวิธี first-order conditional estimation with interaction ลักษณะทางเภสัชศาสตร์ของยาแวนโคมัยซินอธิบายได้ด้วยแบบจำลองชนิดหนึ่งห้องที่มีการกำจัดยา แบบปฏิกิริยาอันดับหนึ่ง ค่าเฉลี่ยอัตราการกำจัดยาแวนโคมัยซิน (CL) มีค่า 0.13 ลิตร/ชั่วโมง/กิโลกรัม และ ค่าเฉลี่ยปริมาตรการกระจายยา (Vd) มีค่า 0.88 ลิตร/กิโลกรัม ปัจจัยที่มีผลต่อค่าพารามิเตอร์ทางเภสัชศาสตร์ ได้แก่ น้ำหนัก (กิโลกรัม) และอัตราการกรองของไต (มิลลิลิตร/นาที/1.73 ตารางเมตร) ดังแสดงในสมการ CL (ลิตร/ชั่วโมง) = 1.66*(น้ำหนัก/14)[superscript 0.75]*(อัตราการกรองของไต/108.9) และ V (ลิตร) = 12.7*(น้ำหนัก/14) ค่า ความผันแปรระหว่างบุคคลของ CL เท่ากับร้อยละ 34.8 ค่าความผันแปรระหว่างบุคคลของ V เท่ากับร้อยละ 39.6 และแบบจำลองสุดท้ายถูกตรวจสอบความสมเหตุสมผลด้วยวิธี bootstrap และ visual predictive check (VPC) ค่าพารามิเตอร์จากแบบจำลองสุดท้ายมีค่าใกล้เคียงกับค่าพารามิเตอร์ที่ได้จากการทำ bootstrap 1,000 ครั้ง และอยู่ในช่วงความเชื่อมั่นที่ร้อยละ 95 ผลการจำลองข้อมูลด้วยวิธี VPC 1,000 ครั้ง พบค่าความเข้มข้นของ ยาแวนโคมัยซินร้อยละ 6 ที่อยู่นอกช่วงความเชื่อมั่นที่ร้อยละ 90 แสดงถึงความเหมาะสมของแบบจำลองดังกล่าว ในการอธิบายเภสัชจลนศาสตร์ของยาแวนโคมัยซิน

การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อพัฒนาแบบประเมินสมรรถนะ และกำหนดเกณฑ์ตัดสินผลการประเมินสมรรถนะพยาบาลวิชาชีพอุบัติเหตุ-ฉุกเฉิน โรงพยาบาลจุฬาลงกรณ์ แบ่งการศึกษาเป็น 2 ระยะ คือ ระยะที่ 1 การสร้างแบบประเมิน มี 2 ขั้นตอน ขั้นตอนที่ 1 สังเคราะห์และวิเคราะห์ความสอดคล้องขององค์ประกอบและรายการสมรรถนะ โดยการสนทนากลุ่มกับผู้เชี่ยวชาญด้านบริหารและการปฏิบัติการพยาบาลในงานอุบัติเหตุ-ฉุกเฉิน จำนวน 10 คน ขั้นตอนที่ 2 การสร้างแบบประเมิน โดยนำรายการพฤติกรรมมาสร้างเกณฑ์การประเมินระดับสมรรถนะด้วยเกณฑ์แบบรูบริค (Rubric) 5 ระดับ วิเคราะห์ความตรงตามเนื้อหาของแบบประเมินโดยผู้ทรงคุณวุฒิ จำนวน 10 คน วิเคราะห์คุณภาพของแบบประเมินด้านความคงที่โดยประเมินพยาบาลวิชาชีพอุบัติเหตุ-ฉุกเฉิน จำนวน 5 คน ประเมินโดยพยาบาลหัวหน้าหอ 1 คน และ พยาบาลที่มีความเชี่ยวชาญในด้านการบริหารและปฏิบัติการพยาบาล จำนวน 5 คน และหาค่าความสอดคล้องภายใน โดยนำแบบประเมินไปใช้กับประชากร คือ พยาบาลวิชาชีพอุบัติเหตุ-ฉุกเฉิน จำนวน 63 คน ระยะที่ 2 การกำหนดเกณฑ์ตัดสินผลการประเมินสมรรถนะ โดยนำผลการประเมินที่ได้มากำหนดคะแนนจุดตัด โดยผู้ทรงคุณวุฒิด้านการบริหารและการปฏิบัติการพยาบาลด้านงานอุบัติเหตุ-ฉุกเฉิน จำนวน 10 คน ผลการศึกษาสรุปได้ดังนี้ 1. แบบประเมินสมรรถนะพยาบาลวิชาชีพอุบัติเหตุ-ฉุกเฉิน ประกอบด้วย 7 สมรรถนะ ข้อรายการสมรรถนะ 57 ข้อ ดังนี้ ด้านการคัดแยกอาการ (จำนวน 9 ข้อ) ด้านการฟื้นคืนชีพขั้นสูง (จำนวน 4 ข้อ) ด้านการปฏิบัติการพยาบาล (จำนวน 20 ข้อ) ด้านนิติเวชในงานอุบัติเหตุ-ฉุกเฉิน (จำนวน 5 ข้อ) ด้านการติดต่อสื่อสาร (จำนวน 5 ข้อ) ด้านการพัฒนาคุณภาพและการจัดการความรู้เชิงประจักษ์ (จำนวน 8 ข้อ) ด้านคุณลักษณะส่วนบุคคล (จำนวน 6 ข้อ) 2. การตรวจสอบคุณภาพของแบบประเมิน พบว่า แบบประเมินมีความตรงตามเนื้อหา …

โรคเลปโตสไปโรซิสเป็นโรคติดต่อจากสัตว์สู่คน เกิดจากเชื้อเลปโตสไปรา ทำให้เกิดภาวะไตวายเฉียบพลันในผู้ป่วยโรคเลปโตสไปโรซิส การศึกษานี้ได้นำผู้ป่วยที่สงสัยว่าเป็นโรคเลปโตสไปโรซิสจำนวน 330 ราย มาวินิจฉัยว่าเป็นโรคเลปโตสไปโรซิสตามเกณฑ์ขององค์การอนามัยโลก (WHO) ได้แก่ วิธีเพาะเชื้อในเลือด หรือวิธี Microscopic Agglutination Test (MAT) หรือวิธี real time PCR ที่วิธีใดวิธีหนึ่งให้ผลเป็นบวก วัตถุประสงค์เพื่อนำวิธี quantitative Polymerase Chain Reaction (qPCR) และวิธี Droplet Digital PCR (ddPCR) มาใช้หาปริมาณเชื้อเลปโตสไปราจากตัวอย่างปัสสาวะของผู้ป่วย และนำปริมาณเชื้อมาศึกษาความสัมพันธ์กับการเกิดภาวะไตวายเฉียบพลันในผู้ป่วยโรคเลปโตสไปโรซิส โดยใช้เกณฑ์ของ Kidney Disease: Improving Global Outcomes (KDIGO) เป็นเกณฑ์ในการแบ่งความรุนแรงของโรคไตวายเฉียบพลัน ที่แบ่งออกเป็น 2 กลุ่ม คือ กลุ่มคนไข้ที่มีภาวะไตวายเฉียบพลัน (AKI) และกลุ่มคนไข้ที่ไม่มีภาวะไตวายเฉียบพลัน (Non-AKI) ผลคือ คนไข้ได้รับการวินิจฉัยว่าเป็นโรคเลปโตสไปโรซิสจำนวน 207 รายตามหลักเกณฑ์ของ WHO มีปัสสาวะที่นำมาใช้ในการศึกษาด้วยวิธี qPCR ให้ผลบวก 53 ตัวอย่าง สรุปว่าวิธี qPCR ในกลุ่มAKI มีปริมาณเชื้อเฉลี่ย 12937 (1412, 101614) ตัว/มล. มีแนวโน้มของปริมาณเชื้อเลปโตสไปรามากกว่ากลุ่ม Non-AKI เฉลี่ย 3719 (1308, 15495) ตัว/มล. ค่า P-value 0.228 จึงไม่มีความสัมพันธ์กันอย่างมีนัยสำคัญทางสถิติและ วิธี ddPCR มีปัสสาวะที่นำมาใช้ในการศึกษา ให้ผลบวก 69 ตัวอย่าง สรุปว่าวิธี ddPCR ในกลุ่มAKI มีปริมาณเชื้อเฉลี่ย 2100 (645, 4300) ตัว/มล. มีปริมาณเชื้อเลปโตสไปรามากกว่ากลุ่ม Non-AKI เฉลี่ย 320 (150, 1700) ตัว/มล. ค่า …

ปัจจุบันพบเชื้อ Escherichia coli ที่สร้างเอนไซม์ extended-spectrum beta-lactamase (ESBL) แพร่กระจายในชุมชนอย่างกว้างขวาง เชื้อแบคทีเรียดังกล่าวสามารถพบได้ว่าเป็นเชื้อประจำถิ่นในลำไส้ของคน และสัตว์ รวมไปถึงสามารถพบได้ในสิ่งแวดล้อมทั่วไป ซึ่งอาจกลายเป็นแหล่งเก็บกักเชื้อแบคทีเรียดื้อสารต้านจุลชีพ ที่สามารถแพร่กระจายในชุมชนต่างๆ ทั่วโลก การศึกษาครั้งนี้ทำการจำแนกลักษณะทางโมเลกุลของเชื้อ E. coli ที่สร้างเอนไซม์ ESBL ซึ่งแยกได้จากสุกรในฟาร์มปศุสัตว์และประชากรในชุมชน ในพื้นที่อำเภอเมือง จังหวัดลำพูน เพื่อศึกษารูปแบบของยีนที่ควบคุมการสร้างเอนไซม์ ESBL และศึกษาหาความสัมพันธ์ของเชื้อในกลุ่มตัวอย่างทั้งสอง เชื้อ E. coli ที่สร้างเอนไซม์ ESBL จำนวนทั้งสิ้น 212 สายพันธุ์ แยกได้จากอุจจาระของสุกรและประชากรที่มีสุขภาพดี และทำการจำแนกยีนที่ควบคุมการสร้างเอนไซม์ ESBL ได้แก่ ยีน blaCTX-M ยีน blaTEM และยีน blaSHV ด้วยเทคนิค multiplex-PCR และเทคนิค whole genome sequencing และศึกษาความสัมพันธ์ของเชื้อ E. coli ที่สร้างเอนไซม์ ESBL ที่แยกได้จากทั้งสองกลุ่มตัวอย่างด้วยเทคนิค multilocus sequence typing ผลการศึกษาพบว่าเชื้อ E. coli ที่สร้างเอนไซม์ ESBL ส่วนใหญ่เป็นเชื้อดื้อยาหลายขนาน และมักดื้อยากลุ่ม fluoroquinolones ยากลุ่ม aminoglycosides และยา trimethoprim/sulfamethoxazole ในขณะที่เชื้อทุกสายพันธุ์ดื้อต่อยา ampicillin ยา ceftriaxone และยา cefotaxime ยีนที่ควบคุมการสร้างเอนไซม์ ESBL ที่พบมากที่สุดได้แก่ ยีน blaCTX-M ซึ่งพบได้ 95.75% รองลงมาคือยีน blaTEM และยีน blaSHV พบได้ 62.73% และ 2.40% ตามลำดับ โดย subgroup ของยีนที่ควบคุมการสร้างเอนไซม์ ESBL ที่พบมากที่สุด ได้แก่ ยีน blaCTX-M-55 …
