Objective : Dirofilariasis or heartworm disease, a mosquito-borne parasitic disease, continues to be a serious problem in canine (dogs) and feline (cats) species. The disease is caused mainly by the filarial nematode, Dirofilaria immitis. In canine dirofilariasis, the adult worms, present in the pulmonary arteries and the right ventricle, causing damages and leading to pulmonary hypertension and right-sided congestive heart failure. In addition, acute death in feline dirofilariasis causes lung injury resulting in respiratory distress. Wolbachia is a group of obligatory intracellular gram-negative bacteria. This bacterium is responsible for manipulation of host reproduction in many arthropod species and involved in embryonic and larval development, adult female fertility, and survival in filarial nematode. The applied biology of Wolbachia and modification of pest and vector species using Wolbachia have been studied in order to suppress or modify natural populations. In addition, Wolbachia has been used as a novel chemotherapeutic target for filariasis control program. Some kinds of eye drop such as Latanoprost, Chloramphenicol, Cefazolin, Ceftazidime, Vancomycin and Amphotericin B need to be kept at the temperature of 2-8 degree celsius to maintain their efficiency. Patients are usually recommended to keep these drugs in the refrigerator while dispensing. However, the temperature in the refrigerator is not consistent especially the refrigerator's door has higher temperature than other parts.

The estimation of the postmortem interval (PMI) is necessary for supporting justice to the dead. During the early stage of death, estimation of PMI of the corpses depends on the changing of muscle contraction and physical conditions, but this method can be used effectively in case of death within 24 hours. Flies are the first organisms which swarm on corpses. Consequently, accuracy of the estimation of PMI using fly larvae collected from corpses depends on identification of the species of flies. Objective : The biological roles of Wolbachia, a filarial intracellular bacteria, have promoted applied researches in lymphatic filariasis. Analysis of Wolbachia proteins will provide complementary information on the biology of Wolbachia, as well as the immune mechanisms against filarial nematodes. We therefore developed a Wolbachia isolation method from Dirofilaria immitis (dog heartworm) for protein analysis. Methods : Wolbachia bacteria were homogenized and isolated by fractioned centrifugation by using 0.85% NaCl supplemented with varying concentration of Nonidet P-40 (NP-40) as the homogenization buffer. Diethylcarbamazine (DEC) treatment for lymphatic filariasis can be accompanied by adverse drug reactions. Severe adverse reactions are usually uncommon but once happen, they could be acute and sometimes life-threatening. We report here a case of 15-year-old patient who was treated with DEC and developed severe adverse reactions including fever, headache, dizziness, chest pain, sore throat, cough, dyspnea, nausea and vomiting. A complete profile of clinical findings and blood parameters, including parasitological, hematological, biochemical, and immunological parameters was reported. Mid aortic syndrome (MAS) is characterized by an extensive narrowing of the abdominal aorta, with involvement of the renal and visceral arterial branches. Angiography is the gold standard for the diagnosis of MAS. However, computed tomography angiography (CTA) can effectively determine the location and the extent of the stenosis. Medical management of MAS generally means a lifelong, combination of multiple classes of antihypertensive medications. Surgical correction such as aortoaortic bypass with renal revascularization remains the definitive treatment of the idiopathic MAS. Objectives : To study the effectiveness of antibiotic drug use for the treatment of sepsis after transrectal ultrasound guide core biopsy of prostate (TRUS-Bx). Methods : This was a retrospective study from January 2004 to December 2008 of all men who received TRUS-Bx at the Department of Urology, King Chulalongkorn Memorial Hospital, for the elevation of prostatic specific antigen (PSA > 4.0 ng/ml) or abnormal digital rectal examination (DRE). ภาวะอ้วนในเด็กเป็นปัญหาที่พบมากขึ้นในปัจจุบัน ทำให้เกิดภาวะแทรกซ้อนในหลายๆ ระบบของร่างกายรวมทั้งระบบหายใจ ความผิดปกติทางระบบหายใจเป็นผลมาจากการสะสมของเนื้อเยื่อไขมันตามอวัยวะต่างๆ ที่เกี่ยวข้องกับการหายใจ เช่น ผนังทรวงอก ผนังหน้าท้อง ทางเดินหายใจ และกะบังลม ทำให้ความยืดหยุ่นของผนังทรวงอกและปอดลดลง ปริมาตรความจุปอดและความสามารถในการออกกำลังกายลดลง นอกจากนี้ไขมันที่สะสมอยู่บริเวณทางเดินหายใจส่วนต้นและลำคอยังทำให้ทางเดินหายใจตีบแคบโดยเฉพาะอย่างยิ่งในขณะหลับ เกิดภาวะ obstructive sleep apneaและ obesity hypoventilation syndrome ทั้งหมดนี้ทำให้เด็กอ้วนเสี่ยงต่อการเกิดภาวะออกซิเจนในเลือดต่ำหรือมีคาร์บอนไดออกไซด์ในเลือดสูงและเกิดภาวะหายใจล้มเหลวตามมาได้ โดยเฉพาะอย่างยิ่งหากมีการติดเชื้อในระบบหายใจหรือภายหลังได้รับยานอนหลับหรือยาสลบเพื่อทำการผ่าตัดนอกจากนี้ ยังพบว่า การที่มีระดับออกซิเจนในเลือดต่ำหรือคาร์บอนไดออกไซด์ในเลือดสูงแบบเรื้อรังร่วมกับภาวะ leptin resistance ซึ่งพบได้ในเด็กอ้วนจะทำให้เด็กเหล่านี้มีการตอบสนองทางระบบหายใจน้อยกว่าปกติต่อภาวะขาดออกซิเจนหรือคาร์บอนไดออกไซด์ในเลือดคั่ง ทำให้ภาวะดังกล่าวรุนแรงมากยิ่งขึ้น ซึ่งหากไม่ได้รับการดูแลรักษาก็จะนำไปสู่ภาวะการหายใจล้มเหลวหรือเกิดภาวะแทรกซ้อนทางระบบหัวใจและหลอดเลือดตามมาได้ นอกจากนี้ สารเคมีที่หลั่งจากเนื้อเยื่อไขมันยังมีผลทำให้เกิดการอักเสบในระบบหายใจ และอาจเกี่ยวข้องกับการเกิดโรคหืดและภาวะหลอดลมไวเกินในเด็กเหล่านี้ การดูแลทางด้านโภชนาการและส่งเสริมการออกกำลังกายอาจช่วยให้สมรรถภาพปอดดีขึ้นและลดการเกิดภาวะแทรกซ้อนต่างๆ เหล่านี้ได้.

Iodinated contrast media ถูกใช้ในปริมาณที่มากขึ้นตามการเติบโต และขยายตัวอย่าง รวดเร็วของการตรวจทางรังสีวิทยาวินิจฉัย โดยเฉพาะการตรวจด้วยเครื่องเอกซเรย์คอมพิวเตอร์ ในขณะเดียวกัน เราก็ได้เรียนรู้ผลข้างเคียงจากการใช้ iodinated contrast media มากขึ้น ผลข้างเคียง โดยรวมพบได้ 5 - 8 เปอร์เซ็นต์ และมีน้อยกว่า 0.1 เปอร์เซ็นต์ ที่มีอันตรายถึงชีวิต ผลข้างเคียงแบ่ง ได้ว่า เป็นผลข้างเคียงต่อไตหรือต่ออวัยวะอื่น ๆ หรืออาจจะแบ่งเป็นแบบเฉียบพลัน หรือแบบล่าช้า การประเมินผู้ป่วยที่มีความเสี่ยงรวมถึงการจัดการและการรักษาผู้ป่วยเหล่านี้อย่างถูกต้อง และทันท่วง ทีเป็นสิ่งที่สำคัญและเป็นหน้าที่สำหรับรังสีแพทย์และทีม ซึ่งรวมถึงแพทย์เจ้าของไข้ แพทย์ประจำบ้าน พยาบาล นักรังสีการแพทย์ และเจ้าหน้าที่ธุรการที่ทำหน้าที่นัดผู้ป่วย บทความพิเศษเกี่ยวกับแนวทาง ปฏิบัติทางด้านคลินิคสำหรับการฉีด contrast media ฉบับนี้ เกิดจากการรวบรวมทบทวนบทความ ต่าง ๆ ที่เกี่ยวข้องและได้สรุปแนวทางที่เหมาะสมในการจัดการและป้องกันผลข้างเคียงต่าง ๆ ที่จะเกิด จากการใช้ iodinated contrast media.

Introduction : Computed tomography (CT) has been frequently supplemented to stage intrathoracic disease, monitor response to treatment, evaluate recurrence, and diagnose complications. Geographic variations in the incidence of malignant lymphoma are well documented. However, there is no radiologic study of intrathoracic disease of lymphoma in Thailand. Objective : To describe intrathoracic diseases of both Hodgkin disease (HD) and non-Hodgkin's lymphoma (NHL) demonstrated on CT.
