Retinal Stem Cell Culture On Planar Scaffold For Transplantation In Animal Models Of Retinal Degeneration, Tina Xia Jan 2018

Retinal Stem Cell Culture On Planar Scaffold For Transplantation In Animal Models Of Retinal Degeneration, Tina Xia

Yale Medicine Thesis Digital Library

Successful differentiation of pluripotent stem cells into retinal cells and subsequent transplantation in animal models have made stem cell-based therapy a closer possibility for treating retinal degenerations. However, major limitations are low efficiency in deriving desired cell types and continued need for large animal studies to validate small animal studies. In this study, we modified a biopolymer scaffold with laminin-521 as a novel substrate to culture retinal progenitor cells (RPCs). We hypothesize that laminin-521 will increase cell retention and provide a developmental signal for RPCs to mature and self-organize. The RPC scaffold culture was transplanted into a RD10 mice, a …

Biodegradable Nanoparticles In The Treatment Of Cutaneous Malignancy, Emily Sara Yin Jan 2018

Biodegradable Nanoparticles In The Treatment Of Cutaneous Malignancy, Emily Sara Yin

Yale Medicine Thesis Digital Library

Keratinocyte-derived carcinomas represent the most common type of malignancy worldwide, and while surgical removal is a first-line therapy, surgery may be impractical for certain patients and does not completely eliminate risk of recurrence. Previous studies have demonstrated the advantages of biodegradable poly(lactic acid)-hyperbranched polyglycerol non-bioadhesive nanoparticles (NNPs) and bioadhesive nanoparticles (BNPs) as chemotherapeutic drug delivery vehicles for the treatment of internal solid tumors. We are now investigating the use of this delivery system for the treatment of cutaneous malignancies, with the goal of maximizing drug efficacy while minimizing systemic effects and treatment-associated morbidity. After topical application of fluorescent dye-loaded NNPs …

Datives, Data And Dialect Syntax In American English, Jim Wood, Raffaella Zanuttini Jan 2018

Datives, Data And Dialect Syntax In American English, Jim Wood, Raffaella Zanuttini

Linguistics Faculty Publications

In this paper, we present a detailed case study of a number of dative constructions that vary across speakers of American English. We show how geographical maps of acceptability judgments can be used to shed light on the syntactic structures underlying those judgments. Those structures can then be used to refine our understanding of syntax more generally, in this case relating to the features of argument-introducing heads. We provide novel support for the low applicative analysis of the Personal Dative construction, on the grounds that this analysis falls in line with a general, somewhat surprising conclusion about Southern American English: …

Assessment And Optimization Of Respiratory Syncytial Virus Prophylaxis Regimens In Connecticut, 1995-2013, Ben Artin Jan 2018

Assessment And Optimization Of Respiratory Syncytial Virus Prophylaxis Regimens In Connecticut, 1995-2013, Ben Artin

Yale School of Medicine Physician Associate Program Theses

BACKGROUND - Respiratory syncytial virus (RSV) causes seasonal respiratory infection with potentially serious complications in children, and leads to hospitalization rates as high as 50% in high-risk infants. Safe and effective immunoprophylaxis (palivizumab) is available, but costly. The American Academy of Pediatrics (AAP) recommends palivizumab only for infants at high risk of complications, and only during the RSV season. Although the current AAP guidelines acknowledge the existence of spatial and temporal variation in RSV incidence, they do not recommend spatial or temporal adjustments to immunoprophylaxis regimen outside of Florida. In this study, we investigate the value of using spatial and …

Rectified Flow In A Stratified Coastal Ocean, K. H. Brink Jan 2018

Rectified Flow In A Stratified Coastal Ocean, K. H. Brink

Journal of Marine Research

An idealized numerical model is used to explore the generation of mean flows by oscillating wind forcing in a stratified coastal ocean with no alongshore variability, i.e., where neither barotropic nor baroclinic instability is a factor. On the inner shelf, where surface-to-bottom mixing occurs, a mean cross-shelf flow develops, as examined by Castelao et al. (2010), and the present results suggest that this flow can remain two-dimensional if there is a nonzero cross-shelf density gradient. Offshore of the inner shelf, where the water column is stratified, a mean alongshore flow develops in the direction opposite to coastal-trapped wave propagation. This …

Antarctic Mode Water, K. Speer, J.-B. Sallee, V. Pellichero Jan 2018

Antarctic Mode Water, K. Speer, J.-B. Sallee, V. Pellichero

Journal of Marine Research

An estimate of Southern Ocean volume in temperature-salinity (T-S) classes is made with a focus on cold water masses south of the Polar Front, at depths shallower than 2,000 m. The volumetric diagram for waters below 5° C shows a deficit at temperatures near 0° C and 34.2 psu, surrounded by a ring of larger volumes, related to water mass formation near the Polar Front and near the continental margins as well as mixing effects. The cold, fresh extreme of the Antarctic Intermediate Water T-S relation shows a volumetric maximum, apparently distinct from the interior T-S relationship. We …

Multi-Decadal Trends And Variability In Temperature And Salinity In The Mid-Atlantic Bight, Georges Bank, And Gulf Of Maine, Elizabeth J. Wallace, Lev B. Looney, Donglai Gong

Journal of Marine Research

Increasing attention is being placed on the regional impact of climate change. This study focuses on the decadal scale variabilities of temperature and salinity in the Mid-Atlantic Bight (MAB), Georges Bank (GB), and Gulf of Maine (GOM) from 1977 to 2016 using hydrographic survey data from the National Oceanic and Atmospheric Administration (NOAA) Northeast Fisheries Science Center. The MAB (as defined by the shelf regions from Cape Hatteras to Cape Cod) experienced warming rates of 0.57 °C per decade during the Winter/Spring season (Jan–Apr) and 0.47 °C per decade during the Fall/Winter season (Sep–Dec). The GOM and GB, on the …

Abstract

SIMULATION AND SELF-ASSESSMENT: A MEDICAL-SIMULATION BASED EXPLORATION OF TRAINEE UNDERSTANDING OF EPA 6 (ORAL PRESENTATION) AND EPA 8 (HANDOFF)

Shuaib Raza, Katherine Gielissen and Tiffany Moadel.

Department of Pediatrics, Yale University School of Medicine, New Haven, CT

Introduction:

Entrustable Professional Activities are clinical tasks that are observable, executable, and reflect one or more clinical competency. The goal of “entrustment” is that trainees perform their work safely and effectively without supervision once they have demonstrated sufficient competency. Simulation offers a unique opportunity to assess EPAs and objectively measure skills without threat to patient safety. Dr. Gielissen and Dr. Moadel chose …

Sex Disparities In St-Elevation Myocardial Infarction Care And Outcomes: A Global Systematic Metaanalysis, Ido Haimi

Yale Medicine Thesis Digital Library

Background: Cardiovascular disease is the leading cause of death worldwide. Outcomes of patients with ST-segment–elevation myocardial infarction (STEMI) have improved through widespread implementation of systems-of-care, yet sex disparities continue to be reported. A comprehensive, global study of sex disparities in contemporary STEMI care and outcomes has not been undertaken.

Objective: To examine whether sex differences in STEMI management and mortality outcomes persist worldwide and by geographic region.

Methods: A systematic PubMed literature search was performed using search terms “sex” or “gender” and “STEMI” for studies in English from 2000 to present reporting sex-based STEMI mortality. Articles with primary data on …

Electrophysiology Of The Thalamus During Focal Limbic Seizures, William Angus Biche Jan 2018

Electrophysiology Of The Thalamus During Focal Limbic Seizures, William Angus Biche

Yale Medicine Thesis Digital Library

Temporal lobe epilepsy remains a common and complex clinical entity whose underlying disease pathology is incompletely understood. While many structures been identified in contributing to these seizures, particular note should be given to the thalamus. Previous studies with imaging techniques and neurostimulation have suggested certain thalamic nuclei of interest, but their precise activity during seizures has yet to be elucidated. The goal of this study was to perform population and single neuron recordings of several different thalamic nuclei during temporal lobe seizures, namely, the anterior (ANT), centrolateral (CL) and ventral posteriomedial (VPM). We performed these studies in an established rat …

Investigating The Effect And Mechanism Of Antiphospholipid Antibodies On Human Endometrial Endothelial Cell Function And The Impact Of Current Standard Therapies, Zola Chihombori Quao Jan 2018

Investigating The Effect And Mechanism Of Antiphospholipid Antibodies On Human Endometrial Endothelial Cell Function And The Impact Of Current Standard Therapies, Zola Chihombori Quao

Yale Medicine Thesis Digital Library

Women with pathologic levels of antiphospholipid antibodies (aPL) are at high risk for recurrent pregnancy loss and preeclampsia. Despite treatment with low molecular weight heparin (LMWH), either alone or with low dose aspirin (ASA), the incidence of late-term complications remains high for these patients. aPL recognizing β 2 glycoprotein I (β2GPI) are particularly harmful during pregnancy as they target the placenta causing insufficient spiral artery transformation and inflammation, giving rise to obstetric APS. There have been extensive studies into the mechanisms by which aPL affect the placental trophoblast which constitutively synthesizes and expresses β2GPI. However, aPL specific for β2GPI can …

Lag-3 Mediates Acute Rejection & Memory In Mouse Transplantation, Jeffrey Mark Erfe Jan 2018

Lag-3 Mediates Acute Rejection & Memory In Mouse Transplantation, Jeffrey Mark Erfe

Yale Medicine Thesis Digital Library

Lymphocyte Activation Gene-3 (LAG-3) is a soluble protein and transmembrane protein receptor expressed on lymphocytes such as immunosuppressive regulatory T cells. Our goal was to evaluate the effect of LAG-3 on transplantation outcomes, specifically rejection and cellular memory response to donor antigen, by comparing graft survival and IFN-l secretion to donor antigen in C57BL/6 LAG-3-/- versus wild-type mouse recipients of DBA/2 grafts. We found that LAG-3 deletion accelerates rejection time and enhances IFN-l secretion among heart or skin graft recipients. FACS analysis of memory T cells demonstrated disproportionate increases in effector T cell subsets, consistent with a heightened rejection response. …

Mitochondrial Calcium In Diabetic Platelets: Friend Or Foe?, Zainab O. Jaji Jan 2018

Mitochondrial Calcium In Diabetic Platelets: Friend Or Foe?, Zainab O. Jaji

Yale Medicine Thesis Digital Library

The global burden of diabetes mellitus (DM) is projected to keep escalating with a burgeoning of the incidence and prevalence of cardiovascular disease sequalae, which are its major cause of morbidity and mortality. Current antiplatelet therapies used for risk factor reduction have limited efficacy, as such, further investigation into mechanisms provoking platelet dysfunction is warranted. Cytosolic calcium (Ca2+) excess in DM platelets is a well-documented phenomenon in the literature, and various mechanisms have been postulated. However, the ramification of this observation on mitochondrial health has not been explored. We hypothesize that the increase in cytosolic Ca2+ will be absorbed by …

Using Art Observation In Museum Education To Broach Topics Of Bias And Power Among Health Professional Trainees, Robert Michel Rock Jan 2018

Using Art Observation In Museum Education To Broach Topics Of Bias And Power Among Health Professional Trainees, Robert Michel Rock

Yale Medicine Thesis Digital Library

The Liaison Committee on Medical Education has identified training on cross cultural interaction and bias as high-priority topics. There has been little published on the use of fine art, specifically painting or sculpture, as a means of entry into discussion on issues of bias in medicine relating to race, gender, and other identities. Making the Invisible Visible (MIV) is a 3-hour, guided museum tour that uses art observation to facilitate dialogue about biases embedded in Western culture as well as their influence on personal and professional interaction. The tour uses intersectionality and critical pedagogy as theoretical frameworks to approach multiple …

Validation Of A New Anatomic Severity Grading System For Acute Cholecystitis, Kenneth Vera Jan 2018

Validation Of A New Anatomic Severity Grading System For Acute Cholecystitis, Kenneth Vera

Yale Medicine Thesis Digital Library

The American Association for the Surgery of Trauma (AAST) established anatomic grading in 2015 to facilitate risk stratification and risk adjusted outcomes in emergency general surgery. This study validates the AAST anatomic grading system for acute cholecystitis (AC) at a tertiary, academic referral medical center.

This is a retrospective cohort study of 315 patients admitted for AC between 2013 and 2016. Cholecystitis severity was graded based on clinical, imaging, operative, and pathologic criteria in accordance with the published AAST anatomic grading scale. Grade I is acute cholecystitis, grade II is gangrenous or emphysematous cholecystitis, grade III is localized perforation, grade …

Long-Term Survival Of De Novo Stage Iv Human Epidermal Growth Factor Receptor 2 (Her2)-Positive Breast Cancers Treated With Her2 Targeted Therapy, Yao Wong Jan 2018

Long-Term Survival Of De Novo Stage Iv Human Epidermal Growth Factor Receptor 2 (Her2)-Positive Breast Cancers Treated With Her2 Targeted Therapy, Yao Wong

Yale Medicine Thesis Digital Library

Background: An increasing number of metastatic HER2 positive cancers are diagnosed as de novo stage IV disease. We hypothesize that a subset of these patients, who achieve complete clinical response or no evidence of disease (NED) status after multi-agent HER2-targeted treatments may have long progression free (PFS) and overall survival (OS).

Methods: 483 patients with de novo stage IV HER2 positive breast cancer diagnosed between 1998-2015 were identified through the medical records at Yale and MD Anderson Cancer Centers. Treatment, clinical variables and survival were extracted and compared between those who achieved NED status and those who did not.

Results: …

The Cost Of Saving A Child’S Life: A Cost-Effectiveness Analysis Of A Pediatric Operating Room In Uganda, Ava Yap Jan 2018

The Cost Of Saving A Child’S Life: A Cost-Effectiveness Analysis Of A Pediatric Operating Room In Uganda, Ava Yap

Yale Medicine Thesis Digital Library

This study examines the cost-effectiveness of constructing a dedicated pediatric operating room (OR) in Uganda, a country where access to surgical care is limited to 4 pediatric surgeons serving a population of over 20 million children under 15 years of age.

A decision tree model projected the cost and disability-adjusted life-years (DALY) averted by a Ugandan pediatric OR. OR cost data were collected by obtaining equipment price lists, anesthetic and operative reports, government salary scales. A patient family survey was administered over 6 months to collect out-of-pocket (OOP) costs. The OR case-log, pediatric surgical ward database and literature review informed …

Risk Prediction In Older Adults After Acute Myocardial Infarction: The Silver-Ami Study, David William Goldstein Jan 2018

Risk Prediction In Older Adults After Acute Myocardial Infarction: The Silver-Ami Study, David William Goldstein

Yale Medicine Thesis Digital Library

Older adults are at risk for functional decline after hospitalization for acute myocardial infarction (AMI). Our goal with this thesis is to explore two outcomes relevant to maintenance of physical function, falls and cardiac rehabilitation (CR) utilization in a cohort of adults over the age of 75 hospitalized with acute myocardial infarction. We aim to describe the risk of falls within six months of discharge and the rates of CR use, and to identify factors associated with these outcomes.

Our project uses data from the SILVER-AMI study, a prospectively designed cohort study which enrolled 3000 patients over the age of …

Comparison Of Hematologic Effects Of Coronary Artery Bypass Grafting Surgery Performed With And Without Use Of Cardiopulmonary Bypass, Simon Kigwana Jan 2018

Comparison Of Hematologic Effects Of Coronary Artery Bypass Grafting Surgery Performed With And Without Use Of Cardiopulmonary Bypass, Simon Kigwana

Yale Medicine Thesis Digital Library

Abstract: Coronary artery graft surgery (CABG) using a cardiopulmonary bypass (CPB)

pump to allow for stopping the heart, commonly designated as “on pump CABG” or

ONCAB, requires complete anticoagulation and is associated with significant

postoperative anemia. In addition, a reduction in postoperative platelet counts is relatively

common in large part due to heightened activation of hemostatic pathways and platelet

consumption secondary to blood passing through the CPB circuit. It has been

demonstrated that both anemia and nadir platelet counts after ONCAB are associated

with the incidence and severity of postoperative acute kidney injury (AKI). Over the past

several years, techniques …

Chemical And Phycological Structure Of Surface Microlayer And Subsurface Water In A Southern Baltic Sea Estuary, Józef Piotr Antonowicz, Anna Kozak Jan 2018

Chemical And Phycological Structure Of Surface Microlayer And Subsurface Water In A Southern Baltic Sea Estuary, Józef Piotr Antonowicz, Anna Kozak

Journal of Marine Research

The surface microlayer of water (SML) is a unique ecotone found on all water bodies. It is an interphase for the exchange of matter between the hydrosphere and the atmosphere. This ecotone is capable of accumulating chemicals and microorganisms in amounts as high as 100-fold greater than those observed in the pelagic zone. Here we report on the accumulation of chemicals and phytoneuston in the SML of the estuarial Łebsko Lake located at the southern coast of the Baltic Sea in the unpolluted region of the World Biosphere Reserve: the Słowínski National Park in Poland. The physicochemical parameters and phytoplankton …
