Initiating Buprenorphine In Patients Hospitalized With Intravenous Drug-Use-Related Endocarditis, Naiska Y. Cheung Apr 2019

Initiating Buprenorphine In Patients Hospitalized With Intravenous Drug-Use-Related Endocarditis, Naiska Y. Cheung

Yale School of Medicine Physician Associate Program Theses

During the current opioid crisis, hospitalizations for the treatment of intravenous drug use-related infective endocarditis have increased. Despite this, opioid use disorder is inadequately treated resulting in continued drug use, higher surgical intervention, recurrence and mortality rates relative to non-drug use-related endocarditis. Buprenorphine* is a safe and effective medication-assisted addiction treatment in outpatient settings; however, buprenorphine remains underutilized, particularly among patients hospitalized for infective endocarditis. Emergency department-initiated buprenorphine combined with referral to ongoing treatment is associated with improved rates of addiction treatment engagement. Therefore, our objective is to investigate whether hospital-initiated buprenorphine combined with referral in patients with opioid use …

Psilocybin-Assisted Therapy: A Double-Blind Randomized Controlled Trial, Katherine Gruppo Apr 2019

Psilocybin-Assisted Therapy: A Double-Blind Randomized Controlled Trial, Katherine Gruppo

Yale School of Medicine Physician Associate Program Theses

Alcohol use disorder is one of the most prevalent mental health disorders worldwide, treatment options are limited, and relapse rates are high. There remains an urgent need for effective treatments. Research involving the therapeutic potential of classic hallucinogens has reemerged in recent decades and the hallucinogen psilocybin has shown some promise in early studies. We aim to assess the efficacy of psilocybin in treating alcohol use disorder with a 10-week double blind randomized double dummy crossover control trial comparing psilocybin-assisted psychotherapy to ketamine-assisted psychotherapy in individuals with alcohol use disorder. Our primary outcome will be the mean percent heavy drinking …

Effectiveness Of Telepsychiatry Among Geriatric Participants With Age-Related Hearing Loss, Stephanie Mock Apr 2019

Effectiveness Of Telepsychiatry Among Geriatric Participants With Age-Related Hearing Loss, Stephanie Mock

Yale School of Medicine Physician Associate Program Theses

Presbycusis (hearing loss that occurs with age) affects 30% of adults aged 65 to 74, yet hearing loss is rarely considered when developing novel treatment deliveries. For example, research shows that telepsychiatry improves depression symptoms among geriatric patients similarly to traditional forms of therapy, however, there is no literature on effectiveness of telepsychiatry in geriatrics with presbycusis. The objective of this study is to assess whether, compared to face-to-face psychotherapy, telepsychiatry produces superior outcomes assessed by the Beck Depression Inventory II, among depressed geriatric patients suffering from presbycusis. We will conduct a randomized controlled trial among elderly individuals with depressive …

Ketamine Plus Exercise For Major Depressive Disorder: A Randomized Controlled Trial, Sean J. Mccarthy Apr 2019

Ketamine Plus Exercise For Major Depressive Disorder: A Randomized Controlled Trial, Sean J. Mccarthy

Yale School of Medicine Physician Associate Program Theses

Major Depressive Disorder is the leading cause of disability among mental illnesses worldwide, but existing therapies fail to adequately treat approximately 30-50% of depressed patients. Ketamine has promising rapid antidepressant effects, but these effects are of short duration. Exercise yields moderate antidepressant effects both as a primary treatment and as an adjunct to standard therapies, but has not been investigated as an adjunct to ketamine. This study seeks to determine whether the addition of regular aerobic exercise regimens to ketamine therapy will potentiate or prolong ketamine’s antidepressant effects. We propose a single-blind, randomized controlled trial in which patients will be …

Wages, Experience And Training Of Women, Richard Blundell, Monica Costa Dias, David Goll, Costas Meghir

Cowles Foundation Discussion Papers

We investigate the role of training in reducing the gender wage gap using the UK- BHPS which contains detailed records of training. Using policy changes over an 18 year period we identify the impact of training and work experience on wages, earnings and employment. Based on a lifecycle model and using reforms as a source of exogenous variation we evaluate the role of formal training and experience in defining the evolution of wages and employment careers, conditional on education. Training is potentially important in compensating for the effects of children, especially for women who left education after completing high school.

Wages, Experience And Training Of Women Over The Lifecycle, Richard Blundell, Monica Costa Dias, David Goll, Costas Meghir

Cowles Foundation Discussion Papers

We investigate the role of training in reducing the gender wage gap using the UK-BHPS. Based on a lifecycle model and using tax and welfare bene t reforms as a source of exogenous variation we evaluate the role of formal training and experience in defining the evolution of wages and employment careers, conditional on education. Training is potentially important in compensating for the effects of children, especially for women who left education after completing high school, but does not fundamentally change the wage gap resulting from labor market interruptions following child birth.

Robust Tests For White Noise And Cross-Correlation, Violetta Dalla, Liudas Giraitis, Peter C.B. Phillips Apr 2019

Robust Tests For White Noise And Cross-Correlation, Violetta Dalla, Liudas Giraitis, Peter C.B. Phillips

Cowles Foundation Discussion Papers

Commonly used tests to assess evidence for the absence of autocorrelation in a univariate time series or serial cross-correlation between time series rely on procedures whose validity holds for i.i.d. data. When the series are not i.i.d., the size of correlogram and cumulative Ljung-Box tests can be significantly distorted. This paper adapts standard correlogram and portmanteau tests to accommodate hidden dependence and non-stationarities involving heteroskedasticity, thereby uncoupling these tests from limiting assumptions that reduce their applicability in empirical work. To enhance the Ljung-Box test for non-i.i.d. data a new cumulative test is introduced. Asymptotic size of these tests is unaffected …

Earnings Dynamics And Firm-Level Shocks, Benjamin Friedrich, Lisa Laun, Costas Meghir, Luigi Pistaferri Apr 2019

Earnings Dynamics And Firm-Level Shocks, Benjamin Friedrich, Lisa Laun, Costas Meghir, Luigi Pistaferri

Cowles Foundation Discussion Papers

We use matched employer-employee data from Sweden to study the role of the firm in affecting the stochastic properties of wages. Our model accounts for endogenous participation and mobility decisions. We find that firm-specific permanent productivity shocks transmit to individual wages, but the effect is mostly concentrated among the high-skilled workers; firm-specific temporary shocks mostly affect the low-skilled. The updates to worker-firm specific match effects over the life of a firm-worker relationship are small. Substantial growth in earnings variance over the life cycle for high-skilled workers is driven by firms accounting for 44% of cross-sectional variance by age 55.

The Lehman Brothers Bankruptcy H: The Global Contagion, Rosalind Z. Wiggins, Andrew Metrick Mar 2019

The Lehman Brothers Bankruptcy H: The Global Contagion, Rosalind Z. Wiggins, Andrew Metrick

Journal of Financial Crises

When Lehman Brothers filed for bankruptcy on September 15, 2008, it was the largest such filing in U.S. history and a huge shock to the world’s financial markets, which were already stressed from the deflated housing bubble and questions about subprime mortgages. Lehman was the fourth-largest U.S. investment bank with assets of $639 billion and its operations spread across the globe. Lehman’s clients and counterparties began to disclose millions of dollars of potential losses as they accounted for their exposures. But the impact of Lehman’s demise was felt well beyond its counterparties. Concern regarding its real estate assets, its large …

The Lehman Brothers Bankruptcy G: The Special Case Of Derivatives, Rosalind Z. Wiggins, Andrew Metrick Mar 2019

The Lehman Brothers Bankruptcy G: The Special Case Of Derivatives, Rosalind Z. Wiggins, Andrew Metrick

Journal of Financial Crises

When it filed for bankruptcy protection in September 2008, Lehman Brothers was an active participant in the derivatives market and was party to 906,000 derivative transactions of all types under 6,120 ISDA Master Agreements with an estimated notional value of $35 trillion. The majority of Lehman’s derivatives were bilateral agreements not traded on an exchange but in the over-the-counter (OTC) market. Because derivatives enjoyed an exemption from the automatic stay provisions of the U.S. Bankruptcy Code, parties to Lehman’s derivatives could seek resolution and self-protection without the guidance and restraint of the bankruptcy court. The rush of counterparties to novate …

The Lehman Brothers Bankruptcy F: Introduction To The Isda Master Agreement, Christian M. Mcnamara, Andrew Metrick Mar 2019

The Lehman Brothers Bankruptcy F: Introduction To The Isda Master Agreement, Christian M. Mcnamara, Andrew Metrick

Journal of Financial Crises

When Lehman Brothers Holdings, Inc. (LBHI) sought Chapter 11 protection, the more than 6,000 counterparties with which its subsidiaries had entered into over 900,000 over-the-counter (OTC) derivatives transactions faced the question of how best to respond to protect their interests. The existence of standardized documentation developed by the International Swaps and Derivatives Association (ISDA) for entering into such transactions meant that the counterparties likely thought that they were dealing with a well-defined and robust set of options in answering this question. Yet, in practice, the resolution of Lehman’s OTC derivatives portfolio ended up being less orderly than the existence of …

The Lehman Brothers Bankruptcy E: The Effects On Lehman’S U.S. Broker-Dealer, Rosalind Z. Wiggins, Andrew Metrick Mar 2019

The Lehman Brothers Bankruptcy E: The Effects On Lehman’S U.S. Broker-Dealer, Rosalind Z. Wiggins, Andrew Metrick

Journal of Financial Crises

Lehman’s U.S. broker-dealer, Lehman Brothers Inc. (LBI), was excluded from the parent company’s bankruptcy filing on September 15, 2008, because it was thought that the solvent subsidiary might be able to wind down its affairs in a normal fashion. However, the force of the parent’s demise proved too strong, and within days, LBI and dozens of Lehman subsidiaries around the world were also in liquidation. As a regulated broker-dealer, LBI was required to comply with the Securities and Exchange Commission financial-responsibility rules for broker-dealers, including maintaining customer assets separately. However, the corporate complexity and enterprise integration that characterized the Lehman …
