Increasing Distal-Proximal Temperature Gradient With Bed Socks To Improve Sleep In Analog Astronauts, Jessica Hockla May 2020

Increasing Distal-Proximal Temperature Gradient With Bed Socks To Improve Sleep In Analog Astronauts, Jessica Hockla

Yale School of Medicine Physician Associate Program Theses

Sleep quality and quantity in astronauts is a persistent problem, posing risks to mission success and crew safety. The prescription sleep aids used by astronauts to combat this issue have limited effectiveness. Furthermore, they may interfere with physical and cognitive function upon awakening, raising concerns about astronauts’ ability to respond in emergency situations. There is a need for an alternative effective sleep intervention to improve sleep in astronauts. Increasing the distal-proximal temperature gradient by warming the extremities has been shown to improve sleep. Therefore, we propose a randomized, crossover trial investigating the use of bed socks worn at bedtime to …

Platelet-Rich Plasma For Grade Ii And Iii Medial Collateral Ligament Tears In Patients Aged 18-45, Madeline Kratz May 2020

Platelet-Rich Plasma For Grade Ii And Iii Medial Collateral Ligament Tears In Patients Aged 18-45, Madeline Kratz

Yale School of Medicine Physician Associate Program Theses

Medial collateral ligament tears are the most common knee ligament injury. Extent of injury is graded on a three-point scale. The mainstay of treatment for isolated tears is physical therapy; however, grade II and III tears require more intensive therapy. Platelet-rich plasma therapy is an autologous concentration of platelets prepared as an injection to augment the healing process. We propose a randomized controlled trial investigating the efficacy of platelet-rich plasma injection for patients with acute grade II or III medial collateral ligament tears. Results will be quantified by mean decrease in Visual Analog Scale scores at two, four, and six …

Efficacy Of Rhythmic Auditory Stimulation On Ataxia And Functional Dependence Post-Cerebellar Stroke, Kaitlin Fitzgerald May 2020

Efficacy Of Rhythmic Auditory Stimulation On Ataxia And Functional Dependence Post-Cerebellar Stroke, Kaitlin Fitzgerald

Yale School of Medicine Physician Associate Program Theses

Ataxia, from Greek meaning, “lack of order,” is described as irregular movement and discoordination of body, gait, eyes, and speech. Ataxia is associated with cerebellar damage due to stroke and other cerebellar pathologies. Ataxia frequently results in functional impairment. Standard physical and occupational therapies in stroke rehabilitation facilitate motor recovery, especially within 90 days. However, many patients experience movement derangements beyond this time frame. Rhythmic auditory stimulation has been shown to be an effective intervention in chronic motor deficits like those observed after cerebellar stroke. Efficacy among patients with chronic stroke-induced ataxia is unexplored. This randomized control trial seeks to …

Topical Imiquimod As Primary Therapy Prior To Mohs Surgery For Penile Intraepithelial Neoplasia, Joseph Miller May 2020

Topical Imiquimod As Primary Therapy Prior To Mohs Surgery For Penile Intraepithelial Neoplasia, Joseph Miller

Yale School of Medicine Physician Associate Program Theses

Penile intraepithelial neoplasia is a rare malignant lesion commonly caused by the human papillomavirus or associated with lichen sclerosus. The definitive treatment for these indolent lesions is surgery, however topical treatments are often first line as an organ-preserving strategy. Currently, the evidence for topical therapies is based on literature reviews and case studies, with no randomized trials to best guide treatment. In this study, we will conduct a double-blind, randomized placebo-controlled trial to determine whether the use of imiquimod 5% cream as a primary therapy will have higher complete response rates in penile intraepithelial neoplasia compared to placebo cream prior …

Promoting Pre-Exposure Prophylaxis Among People Who Inject Drugs Accessing Syringe Exchange Programs, Elena Sullivan May 2020

Promoting Pre-Exposure Prophylaxis Among People Who Inject Drugs Accessing Syringe Exchange Programs, Elena Sullivan

Yale School of Medicine Physician Associate Program Theses

People who inject drugs are at increased risk of acquiring HIV and accounted for 10% of all new diagnoses in 2018. Pre-exposure prophylaxis has been shown to reduce HIV acquisition among at-risk populations, including people who inject drugs. However, pre-exposure prophylaxis is underutilized by people who inject drugs due to limited knowledge about its existence and purpose. The objective of this study is to determine the effectiveness of a patient navigation intervention in increasing initiation of pre-exposure prophylaxis among people who inject drugs. Specifically, we will carry out a randomized controlled study to compare the rates of pre-exposure prophylaxis initiation …

Video Colposcopy For Decreasing Anxiety In A Low-Income Setting, Jessica Zheng May 2020

Video Colposcopy For Decreasing Anxiety In A Low-Income Setting, Jessica Zheng

Yale School of Medicine Physician Associate Program Theses

Cervical cancer is the fourth most common cancer and fourth most common cause of cancer-related death in women in the world. Current guidelines for cervical cancer screening recommend a Pap and/or human papillomavirus test and a follow-up colposcopy if particular abnormalities are discovered. In many patients, colposcopies cause significant levels of anxiety, adversely impacting patients’ pain tolerance and adherence to follow-up. Previous studies have shown mixed results on whether video colposcopy decreases this anxiety. Moreover, these effects have not been assessed in low-income communities who are disproportionally affected by cervical cancer. In this study, we will conduct a single blind …
